    Two hundred and thirty soil samples from different localities were examined for the presence of geophilic keratinophilic fungi. Six species namely Microsporum gypseum--34 isolates, Chrysosporium keratinophilum--29, C. tropicum--20, Keratinophyton terreum--4, Trichophyton terrestre--8 and Chrysosporium species--3--were isolated. Most of these fungi were recovered from garden, field and river bank soil. The importance of these findings is briefly discussed.

    BACKGROUND: Tinea imbricata is a chronic superficial mycosis caused mainly by Trichophyton concentricum. The condition mainly affects individuals living in primitive and isolated environment in developing countries and is rarely seen in developed countries. Physicians in nonendemic areas might not be aware of this fungal infection.

    OBJECTIVE: To familiarize physicians with the clinical manifestations, diagnosis, and treatment of tinea imbricata.

    METHODS: A PubMed search was completed in Clinical Queries using the key terms "Tinea imbricata" and "Trichophyton concentricum". The search strategy included meta-analyses, randomized controlled trials, clinical trials, observational studies, reviews, and case reports. The information retrieved from the above search was used in the compilation of the present article.

    RESULTS: The typical initial lesions of tinea imbricata consist of multiple, brownish red, scaly, pruritic papules. The papules then spread centrifugally to form annular and/or concentric rings that can extend to form serpinginous or polycyclic plaques with or without erythema. With time, multiple overlapping lesions develop, and the plaques become lamellar with abundant thick scales adhering to the interior of the lesion, giving rise to the appearance of overlapping roof tiles, lace, or fish scales. Lamellar detachment of the scales is common. The diagnosis is mainly clinical, based on the characteristic skin lesions. If necessary, the diagnosis can be confirmed by potassium hydroxide wet-mount examination of skin scrapings of the active border of the lesion which typically shows short septate hyphae, numerous chlamydoconidia, and no arthroconidia. Currently, oral terbinafine is the drug of choice for the treatment of tinea imbricata. Combined therapy of an oral antifungal agent with a topical antifungal and keratolytic agent may increase the cure rate.

    CONCLUSION: In most cases, a spot diagnosis of tinea imbricata can be made based on the characteristic skin lesions consisting of scaly, concentric annular rings and overlapping plaques that are pruritic. Due to popularity of international travel, physicians involved in patient care should be aware of this fungal infection previously restricted to limited geographical areas.

    The aqueous and ethanol extracts of Stichopus chloronotus Brandt were investigated for their effectiveness against guinea pig dermatophytosis caused by Microsporum canis and Trichophyton mentagrophytes using the hair root invasion test. The ethanol extract at 10 mg/ml showed 82.8 % efficacy against T. mentagrophytes while the aqueous extract at similar concentration showed 84.8% efficacy against M. canis infection, as compared to econazole which showed 100% efficacy against both infections. No adverse effect on the skin was observed in the treated animals. In conclusion, aqueous and ethanol extracts of S. chloronotus showed high antimycotic activity against experimentally induced dermatophytosis in guinea pigs.

    Trichophyton rubrum is a common pathogenic fungal species that is responsible for causing infection on
    human skin, hair and nail. The antifungal-resistant strains complicate the treatment regime.
    Hydroxychavicol (HC) is one of the main compounds from Piper betel leaf that have antifungal potential and
    its mechanism of action has not been studied yet. The objective of this preliminary study to determine the
    antifungal properties of HC against T. rubrum using transmission electron microscope (TEM) on gross and
    ultrastructure of T. rubrum hypha. T. rubrum was treated with HC and miconazole (MI) at concentrations of
    1.25, 2.5, 5 and 10 mg/mL for 1, 3, 5 and 7 days continuously. Generally, fungi structures became more
    severely damaged at increasing treatment duration. Microscopically, the fungi’s cell wall treated with HC
    showed a rough surface, shrinkage and demolition similar to the MI treated group. The fungi organelles were
    also demolished and disorganized. This study revealed that HC has the ability to inhibit T. rubrum growth
    and has potential to be an antifungal agent for skin infections.

    The authors describe a case of tinea faciei occurring in a sixteen-year-old boy. The rash was localized to the face and upper chest and resembled a typical photosensitive disorder, resulting in inappropriate treatment for six months. Results of a potassium hydroxide preparation and culture from the surface scale confirmed the clinical diagnosis.

    The common etiological agents of onychomycosis are dermatophytes, molds and yeasts. A mycological nail investigation of onychomycosis using direct microscopy and culture was conducted by the Mycology Unit, Department of Medical Microbiology, University of Malaya from March 1996 to November 1998. The study involved 878 nail clippings or subungal scrapings from subjects with onychomycosis. On direct microscopy examination, 50% of the specimens were negative for fungal elements. On culture, 373 specimens had no growth; bacteria were isolated from 15 nail specimens. Among the 490 specimens with positive fungal cultures, 177 (36.1%) were dermatophytes, 173 (35.5%) were molds and 130 (26.5%) were Candida. There were 2% (10/490) mixed infections of molds, yeasts and dermatophytes. Trichophyton rubrum (115/177) and Trichophyton mentagrophytes (59/177) were the main dermatophytes isolated. The molds isolated were predominantly Aspergillus niger (61/173), Aspergillus nidulans (30/173), Hendersonula toruloidea (26/173) and Fusarium species (16/173). 96.9% of the Candida species identified were Candida albicans.

    The objective was to screen and evaluate the anti-fungal activity of lactic acid bacteria (LABs) isolated from Malaysian fermented foods against two Trichophyton species. A total of 66 LAB strains were screened using dual culture assays. This showed that four LAB strains were very effective in inhibiting growth of T. rubrum but not T. interdigitale. More detailed studies with Lactobacillus plantarum strain HT-W104-B1 showed that the supernatant was mainly responsible for inhibiting the growth of T. rubrum. The minimum inhibitory concentration (MIC), inhibitory concentration, the 50% growth inhibition (IC50) and minimum fungicide concentration (MFC) were 20 mg/mL, 14 mg/mL and 30 mg/mL, respectively. A total of six metabolites were found in the supernatant, with the two major metabolites being L-lactic acid (19.1 mg/g cell dry weight (CDW)) and acetic acid (2.2 mg/g CDW). A comparative study on keratin agar media showed that the natural mixture in the supernatants predominantly contained L-lactic and acetic acid, and this significantly controlled the growth of T. rubrum. The pure two individual compounds were less effective. Potential exists for application of the natural mixture of compounds for the treatment of skin infection by T. rubrum.

    Trichophyton rubrum (T. rubrum) is one of the most common dermatophytes worldwide. This fungus invaded skin appendages of humans and animals. Recently, resistance to antifungal drugs as well as appearance of side effects due to indication of these kinds of antibiotics has been reported. Besides, using some plant extracts have been indicated in herbal medicine as an alternative treatment of these fungal infections. The aim of this study was to investigate the effects of Garlic (Allium sativum) and pure allicin on the growth of hypha in T. rubrum using Electron miscroscopy.

    The leaf and bark oils of Cinnamomum verum J.S. Presl. were examined for their antifungal activity against 6 dermatophytes (Trichophyton rubrum, T. mentagrophytes, T. tonsurans, Microsporum canis, M. gypseum and M. audouini), one filamentous fungi (Aspergillus fumigatus) and 5 strains of yeasts (Candida albicans, Ca. glabrata, Ca. tropicalis, Ca. parapsilosis and Crytococcus neoformans) by using the broth microdilution method. The antifungal activities of 4 standard compounds (cinnamaldehyde, eugenol, linalool and a-terpineol) which were major constituents in the oils were also investigated in an effort to correlate the effectiveness of the oils with those of the components of the oils. The combined antifungal effect of the oils against M. canis, M. gypseum and Cr. neoformans was investigated by the checkerboard assay. Isobolograms were constructed and Fractional Inhibitory Concentrations Index (FICI) were calculated to determine the combination effects between the oils. The chemical composition of the oils was analyzed by gas chromatography (GC) and gas chromatography- mass spectrometry (GC-MS). The oils showed strong activity against all the tested fungi with Minimum Inhibition Concentration (MIC) values ranging from 0.04 to 0.31 mg/ml. Cinnamaldehyde which was the most abundant component of the bark oil of C. verum showed the strongest activity against all the fungi studied. Based on the results of the assay on standard samples, it may be that the high levels of cinnamaldehyde and eugenol in the oils and in combination with the minor components could be responsible for the high antifungal activity of the oils. The antifungal effect of the leaf and bark oils of C. verum in combination against the tested fungi was not synergistic. However, the effect was additive against M. gypseum and antagonistic against Cr. neoformans and M. canis.

    This study was performed to evaluate the antifungal activities of methanolic fractions from the stem bark of Entada spiralis Ridl. against human dermatophytes and yeast-like fungus in vitro. Three types of human dermatophyte, Trichophyton mentagrophytes ATCC 9533, Microsporum gypseum ATCC 24102 and Trichophyton tonsurans ATCC 28942, and one yeast-like fungus, Candida glabrata ATCC 66032, were tested against the methanolic fractions labelled FA1, FA4 and FA5. T. mentagrophytes, T. tonsuran and M. gypseum were susceptible to all tested fractions in a concentration-dependent manner whereas C. glabrata was resistant. Fraction FA1 at a concentration of 400 mg/mL was found to exhibit the highest antifungal activity with the inhibition zone diameter of 22 mm (T. mentagrophytes). This fraction showed a minimum inhibitory concentration MIC of 0.097 mg/mL while the MIC value for the fraction FA4 and fraction FA5 was 3.12 mg/ml and 1.56 mg/ml respectively. Agar overlay bioautography assay results showed that most of the bioactive compounds were found in the fraction FA1. Based on these findings, it can be concluded that the stem bark extracts of E. spiralis can be a future source of potent natural antimicrobial drugs for superficial skin diseases.

    Prevalence of dermatomycoses varies from one centre to another due to many factors. Knowledge of local prevalence is useful to aid clinical diagnosis and treatment. Due to lack of data in Malaysia, this study aimed to look at the causes of dermatomycoses in Kuala Lumpur, Malaysia. Dermatological specimens including skin scrapings, hair and nail clippings were collected carefully from clinically suspected cases of dermatomycoses between 2008 and 2010. All cultures of skin, hair and nails that yielded positive fungal growth were included. Any fungal growth outside the streaking area, duplicate and incomplete data were excluded from the study. Three-hundred-fifty-eight patients were included. Male patients were slightly more than females with a ratio of 1.2:1. The median age was 53 years old with interquartile range of 38-64 years. More than half (53.6%) belonged to 20-60 years age group. Rates of culture isolation were 89.0% for nails, 56.2% for hair and 55.6% for skin. Five-hundred-twenty-two fungi were isolated from 358 clinical specimens. Non-dermatophyte moulds (NDMs) represented the largest group (50.5%; mainly Aspergillus species 18.7%), followed by yeasts (41.6%; mainly Candida species 26.8%) and dermatophytes (7.9%; mainly Trichophyton species 7.7%). In conclusion, NDMs and yeasts were more commonly isolated than dermatophytes from dermatological specimens in this centre. Current treatment regime that focuses on dermatophytes may be ineffective to treat dermatomycoses caused by NDMs or yeasts. Antifungal susceptibility study may be needed to guide therapy in recalcitrant cases.

    Ethanolic extract of Cassia alata leaves was investigated for its antimicrobial activities on several microorganisms including bacteria, yeast, dermatophytic fungi and non-dermatophytic fungi. In vitro, the extract exhibited high activity against various species of dermatophytic fungi but low activity against non-dermatophytic fungi. However, bacterial and yeast species showed resistance against in vitro treatment with the extract. The minimum inhibitory concentration (MIC) values of the extract revealed that Trichophyton mentagorphytes var. interdigitale, Trichophyton mentagrophytes var. mentagorophytes, Trichophyton rubrum and Microsporum gypseum had the MIC of 125 mg/ml, whereas Microsporum canis had the MIC of 62.5 mg/ml. The inhibition can be observed on the macroconidia of Microsporum gypseum which resulted in structural degeneration beyond repair. The mechanism of inhibition can be related to the cell leakage as observed by irregular, wrinkle shape and loss in rigidity of the macroconidia.

    Trichophyton rubrum is one of dermatophytes that penetrates keratinized tissues such as skin, hair and nail of human and animals. Recently, antifungal drugs such as imodazole and triazole was found to cause side effects, toxicity to patients and also not very efficient due to resistance to these drugs. As an alternative, some plants extract had been used to treat dermatophytes. This studies was done using Garlic extract (Allium sativum) to evaluate its effects on the growth of hypha of Trichophyton using Electron miscroscopy. Garlic had been known to posses antimicrobial, antiinflammatory, antithrombotic and antitumor activities. This studies found that garlic extract as low as 4 mg/mL inhibit the growth of hypha. Scanning electron microscopy studies revealed that hypha treated with garlic extract showed shrinkage, flat and cell wall demolition, similar to hypha treated with allicin (positive control) having rough surface, shrinkage and distortion. The tip of hypa became large after treatment with garlic extract. Transmission electron microscopy studies also found that hypha treated with allicin display cell wall thickening, local thickening, destruction of cytoplasmic content, mean while hypha treated with garlic extract exhibited cell wall thickening, disordered hyphal tip and desolution of cytoplasmic compartments and similar with hypha treated with allicin. These results showed that garlic extract and pure allicin could be use as an alternative to treat dermatophytes.
