    Malignant transformation of recurrent respiratory papillomatosis (RRP) is a rare entity occurring in only less than 5% of patients with RRP. The risk of transformation can arise even in the absence of risk factors such as smoking and radiation. We describe a patient with juvenile RRP diagnosed since childhood, who suffered malignant transformation into a squamous cell carcinoma after 29 years.     Poultry meat consumption is increasing worldwide but the overuse of antimicrobials for prevention and treatment of diseases has increased antimicrobial resistance (AMR), triggering a major public health issue. To restrict AMR emergence, the government supports the optimization of natural products that are safe and easy to obtain with minimal side effects on poultry, humans, and the environment. Various studies have explored the potential of herbs in animal health for their antiviral, antibacterial, antifungal, antiparasitic, immunomodulatory, antioxidant, and body weight gain properties. Therefore, this study reviewed plants with potential application in avian species by summarizing and discussing the mechanisms and prophylactic/therapeutic potential of these compounds and their plant origin extracts.

    Background: Oral drug delivery is the most preferred route for drug administration in the world, with tablets being one of the most common dosage forms. However, some people, particularly children and the elderly, have difficulty swallowing the tablets. Chewable tablets are the dosage form that can address the issue while also providing a valuable masking effect on drug taste, allowing patients to swallow the drugs more easily.

    Materials and Methods: In this study, the chewable tablets were manufactured using the melt granulation method, which resulted in tablets with a chewy texture. The tablets contained paracetamol as well as Arabic gum, starch, agar, and mannitol.

    Results: The drug release profiles for the fragmented form showed that 50% of the drug was released within 4 min and 100% was released within 30 min of the dissolution process. The intact form released nearly 90% of the drug within 2 h.

    Conclusion: Formulation 2 was determined as the best formulation. This tablets' formulation had passed all characterization tests and displayed a moderate hardness and chewy texture.

    Objectives: This study aimed to assess the completeness and quality of adverse drug reaction (ADR) reports that were submitted to the Pharmacovigilance Unit (PVU) in clinical training center (CTC), Faculty of Medicine, UiTM Sungai Buloh Campus.

    Materials and Methods: A retrospective study was conducted using all ADR reports that were submitted to the PVU in CTC from December 31, 2000, to December 31, 2018. The completeness was assessed by reviewing all the required elements to be filled in the ADR reports. The quality was assessed by investigating the required information in the ADR reporting form. Descriptive statistics have been used to present the findings.

    Key Findings: In a total of 31 reports that were submitted to the PVU in CTC, 98.9% of patient's information and 100% of ADR descriptions were completed. Suspected drug information and the reporter's details were completed by 52.2% and 79.6%, respectively. Of 58.0% of the information about seriousness recorded, 38.9% (n = 7) is mild, 44.4% (n = 8) is moderate, and 16.7% (n = 3) is severe. Among all the suspected medicines, drug class of antibiotics (32.4%, n = 12) is the most reported suspected drugs that caused ADR, followed by opioid analgesic (8.1%, n = 3) and nonsteroidal anti-inflammatory drugs (8.1%, n = 3).

    Conclusion: Further efforts and relevant interventions should be considered to increase the reporting frequency and to enhance the completeness and the quality of the ADR reports in the study setting.

    BACKGROUND AND OBJECTIVES: According to the global cancer situation, which is very alarming, with over 10 million new diagnoses and more than 6 million deaths each year globally, cancer is one of the most prominent causes of morbidity and mortality today. One of the cancers is oral cancer. Oral cancer is the irregular development of malignant cells in the oral cavity. The study's objective was to decide the mortality of cross-tabulation among patients treated for oral carcinoma from Hospital Universiti Sains Malaysia (USM), Kelantan, Malaysia.

    MATERIALS AND METHODS: This chapter summarizes the medical history for 7 years from January 2011 to December 2018 of patients who have been treated for oral carcinoma in the Hospital USM, Oral and Maxillofacial Surgery (OMFS) Unit. Each patient's complete medical record was checked, and data gathered were based on age, gender, site lesion, clinical diagnosis, and mortality. Version 26.0 of the SPSS software was used to evaluate the correlation and distribution of patient survival.

    RESULTS: This was a retrospective cross-sectional review of the medical evidence of 117 patients infected for oral carcinoma at OMFS (Hospital USM). Sixty-seven (57.26%) of the patients were male and fifty (42.74%) were female. Patient age ranged from 25 to 93 years. Malay has the highest prevalence (85.5%) in oral carcinoma, followed by a second ethnic group, Chinese (7.7%). The result indicates that the majority of oral carcinoma patients were over 60 years old.Cases of oral squamous cell carcinoma have proved to be the most prevalent malignant tumour in the mouth cavity. The largest number of cases collected is 91% of the data collected. Mucoepidermoid carcinoma (10%) is the second most common small salivary gland tumor.

    CONCLUSION: OSCC is the most prevalent kind of oral cancer. According to the data review, the most popular site for oral cancer is the tongue.

    Introduction: Little is known about the correlation between chronic obstructive pulmonary disease (COPD) and heroin smoking. Heroin smoking is a recent underinvestigated problem. The goal of this study is to study the impact of heroin smoking among COPD patients.

    Methods: This is a descriptive clinical study. A combination of self-reporting questionnaires and data extraction tools were used to collect information during baseline tests, interviews, and follow-ups. Patients' medical, clinical, and socioeconomic history were recorded. Participants were recruited using random sampling from multiple centers.

    Results: Out of 1034 COPD patients, heroin smokers represented the vast majority of addiction cases (n = 133). Heroin smokers were leaner than non-addicts (19.78 ± 4.07 and 24.01 ± 5.6, respectively). The most common type of comorbidities among heroin smokers was emphysema (27%). Both the forced expiratory volume (FEV1)/forced vital capacity ratio and FEV1% predicted were lower among heroin smokers than non-addicts (52.79 ± 12.71 and 48.54 ± 14.38, respectively). The majority of heroin smokers (55%) had advanced COPD, and at least 15% of heroin smokers suffered from frequent respiratory failure. The mean ± SD for COPD onset age among heroin smokers was 44.23 ± 5.72, and it showed a statistically significant correlation (P < 0.001).

    Conclusion: Heroin smoking might be linked to the onset of COPD. Heroin smokers showed a significantrespiratory impairment compared to tobacco smokers of the same age group.

    BACKGROUND: Periodontitis is a chronic inflammatory disease with conglomerate etiology making it difficult to diagnose at the early stages. Potential biomarkers in gingival crevicular fluid (GCF) would determine the presence of the current disease activity, predict sites vulnerable for future breakdown, and assess the response to periodontal interventions. Merely elevated levels of inflammatory soft-tissue markers do not indicate bone destruction. Since there is no single ideal biomarker established, bone-related biomarkers such as telopeptide of type I collagen, osteocalcin, calprotectin, osteopontin, and tartrate-resistant acid phosphatase (TRAP) seem to hold great promise as predictive markers to determine bone destruction and active phases in the disease progression. The present study is intended to explore the biologic plausibility of the levels of TRAP in health and chronic periodontitis.

    MATERIALS AND METHODS: The present cross-sectional clinico-biochemical study comprised 30 systemically healthy subjects with 15 periodontally healthy and 15 chronic periodontitis subjects who were age and gender matched. GCF and blood samples were collected from all the patients. TRAP estimation was done in both the samples using an enzyme-linked immunosorbent assay kit. The data were analyzed using independent t-test and Pearson correlation test.

    RESULTS: Serum and GCF TRAP levels in chronic periodontitis subjects were significantly higher when compared to the periodontally healthy group. There were no significant correlations found among serum and GCF TRAP levels with increasing age and gender in both the groups. An increase in disease severity, i.e., increase in probing pocket depth and clinical attachment level, did not show correlation with the GCF and serum TRAP levels in the chronic periodontitis group.

    CONCLUSION: Based on the findings of the present study, increased GCF TRAP levels in chronic periodontitis seem to be a potential marker for identifying ongoing periodontal destruction.

    Introduction: In the facial bones, the angle of the mandible is the common site of fractures. Furthermore, it is the site with the highest number of complications after fracture and hence needs an efficient fixation. The right approach is still debatable for the angle fractures. In the light of these factors, we evaluated the transoral and the transbuccal approaches for the treatment of fractures at the angle of the mandible.

    Materials and Methods: Twenty patients were equally divided into two groups of transoral and transbuccal methods. The parameters such as ease of access, surgical time, occlusion, postsurgical infection, fracture gaps, scarring, and complications were noted, and the values that were compared were statistically analyzed. P < 0.05 was considered statistically significant.

    Results: No significant variations were seen in the variables such as ease of access, occlusion, postsurgical infection, and fracture gaps. Surgical time was significantly less for the transoral method. Negligible scarring was noted in the transbuccal method.

    Conclusion: Although both the methods were comparable, the transbuccal approach was more efficient for the mandibular angular fracture treatment.

    Introduction: The viral infection COVID-19 is highly infectious and has claimed many lives till date and is still continuing to consume lives. In the COVID-19, along with pulmonary symptoms, cardiovascular (CV) events were also recorded that have known to significantly contribute to the mortality. In our study, we designed and validated a new risk score that can predict CV events, and also evaluated the effect of these complications on the prognosis in COVID-19 patients.

    Materials and Methods: A retrospective, multicenter, observational study was done among 1000 laboratory-confirmed COVID-19 patients between June 2020 and December 2020. All the data of the clinical and laboratory parameters were collected. Patients were randomly divided into two groups for testing and validating the hypothesis. The identification of the independent risk factors was done by the logistic regression analysis method.

    Results: Of all the types of the clinical and laboratory parameters, ten "independent risk factors" were identified associated with CV events in Group A: male gender, older age, chronic heart disease, cough, lymphocyte count <1.1 × 109/L at admission, blood urea nitrogen >7 mmol/L at admission, estimated glomerular filtration rate <90 ml/min/1.73 m2 at admission, activated partial thromboplastin time >37 S, D-dimer, and procalcitonin >0.5 mg/L. In our study, we found that CV events were significantly related with inferior prognosis (P < 0.001).

    Conclusions: A new risk scoring system was designed in our study, which may be used as a predictive tool for CV complications among the patients with COVID-19 infection.

    BACKGROUND: Patients around the world when attending the dental clinic are prone to undergo the feelings of fear and anxiety. This may impact the dental follow-up and prognosis.

    AIM: The aim of the study is to evaluate the dental anxiety in the patients attending for the dental treatment in a college, in India.

    MATERIALS AND METHODS: A sample of 500 adults with ages 15-60 years was considered for the study. Modified dental anxiety questionnaire was used in the present study to assess the anxiety faced by the patients attending for the dental treatment. ANOVA and Chi-square test were used to test for the significance between the variables, and the correlation between these variables was assessed using the Spearman's correlation. Likert scale was used to know the cutoff for the severity of the dental anxiety.

    RESULTS: Only in the 4% of the participants in the present study had dental anxiety. Gender, age, financial status, education impacted the dental anxiety and the regularity toward the appointments.

    CONCLUSION: It can be concluded from our study that there was a low anxiety in the dental patients toward the dental treatment and also anxiety decreased with the age. However, regular dental visits are advised to prevent the overall impact of the oral health on the general health.

    Congenitally missing anterior teeth alters the patient's physiognomy and diction in a big way. Maxillary incisor agenesis, prominently the lateral incisor is the most common congenitally missing permanent tooth, in the anterior maxillary region, which is the esthetic zone, representing approximately 20% of all dental anomalies. Treatment planning is inclusive of; smile design, preparation, perception of the patients, and their expectations in relation to esthetics, interdisciplinary alliance that meets the functional, health, and esthetic needs. A critical factor for the overall success is that of choosing a suitable restorative recourse. The case report describes the esthetic rehabilitation for congenitally missing maxillary lateral incisor and early traumatic loss of central incisor using an all ceramic-fixed prosthesis.

    Rice tungro bacilliform virus (RTBV) belongs to genus Tungrovirus within the family Caulimoviridae harbors circular double-stranded DNA (dsDNA). Rice tungro disease (RTD) caused by RTBV, responsible for severe rice yield losses in South and Southeast Asia. Here, we performed a systematic evolutionary and codon usage bias (CUB) analysis of RTBV genome sequences. We analysed different bioinformatics techniques to calculate the nucleotide compositions, the relative synonymous codon usage (RSCU), and other indices. The results indicated slightly or low codon usage bias in RTBV isolates. Mutation and natural selection pressures have equally contributed to this low codon usage bias. Additionally, multiple factors such as host, geographical distribution also affect codon usage patterns in RTBV genomes. RSCU analysis revealed that RTBV shows mutation bias and prefers A and U ended codons to code amino acids. Codon usage patterns of RTBV were also found to be influenced by its host. This indicates that RTBV have evolved codon usage patterns that are specific to its host. The findings from this study are expected to increase our understanding of factors leading to viral evolution and fitness with respect to hosts and the environment.

    Burkholderia pseudomallei is a Gram-negative soil-dwelling bacillus that causes melioidosis, a frequently fatal infectious disease, in tropical and subtropical regions. Previous studies have identified the overall genetic and evolutionary characteristics of B. pseudomallei on a global scale, including its origin and transmission routes. However, beyond its known hyperendemicity foci in northern Australia and Southeast Asia, the distribution and genetic characteristics of B. pseudomallei in most tropical regions remain poorly understood, including in southern China. Here, we sequenced the genomes of 122 B. pseudomallei strains collected from Hainan, an island in southern China, in 2002-2018, to investigate the population structure, relationships with global strains, local epidemiology, and virulence and antimicrobial-resistance factors. A phylogenetic analysis and hierarchical clustering divided the Hainan strains into nine phylogenic groups (PGs), 80 % of which were concentrated within five major groups (group 1: corresponding to minor sequence types [STs], 12.3 %; group 3: ST46 and ST50, 31.1 %; group 9: ST58, 13.1 %; group 11: ST55, 8.2 %; group 15: mainly ST658, 15.6%). A phylogenetic analysis that included global strains suggested that B. pseudomallei in Hainan originated from Southeast Asian countries, transmitted in multiple historical importation events. We also identified several mutual transmission events between Hainan and Southeast Asian countries in recent years, including three importation events from Thailand and Singapore to Hainan and three exportation events from Hainan to Singapore, Malaysia, and Taiwan island. A statistical analysis of the temporal distribution showed that the Hainan strains of groups 3, 9, and 15 have dominated the disease epidemic locally in the last 5 years. The spatial distribution of the Hainan strains demonstrated that some PGs are distributed in different cities on Hainan island, and by combining phylogenic and geographic distribution information, we detected 21 between-city transmission events, indicating its frequent local transmission. The detection of virulence factor genes showed that 56 % of the Hainan strains in group 1 encode a B. pseudomallei-specific adherence factor, boaB, confirming the specific pathogenic characteristics of the Hainan strains in group 1. An analysis of the antimicrobial-resistance potential of B. pseudomallei showed that various kinds of alterations were identified in clinically relevant antibiotic resistance factors, such as AmrR, PenA and PBP3, etc. Our results clarify the population structure, local epidemiology, and pathogenic characteristics of B. pseudomallei in Hainan, providing further insight into its regional and global transmission networks and improving our knowledge of its global phylogeography.

    Chronic lymphocytic leukaemia (CLL) is a heterogeneous disease with a highly variable clinical outcome. There are well-established CLL prognostic biomarkers that have transformed treatment and improved the understanding of CLL biology. Here, we have studied the clinical significance of two crucial B cell regulators, BACH2 (BTB and CNC homology 1, basic leucine zipper transcription factor 2) and BCL6 (B-cell CLL/lymphoma 6), in a cohort of 102 CLL patients and determined the protein interaction networks that they participate in using MEC-1 CLL cells. We observed that CLL patients expressing low levels of BCL6 and BACH2 RNA had significantly shorter overall survival (OS) than high BCL6- and BACH2-expressing cases. Notably, their low expression specifically decreased the OS of immunoglobulin heavy chain variable region-mutated (IGHV-M) CLL patients, as well as those with 11q and 13q deletions. Similar to the RNA data, a low BACH2 protein expression was associated with a significantly shorter OS than a high expression. There was no direct interaction observed between BACH2 and BCL6 in MEC-1 CLL cells, but they shared protein networks that included fifty different proteins. Interestingly, a prognostic index (PI) model that we generated, using integrative risk score values of BACH2 RNA expression, age, and 17p deletion status, predicted patient outcomes in our cohort. Taken together, these data have shown for the first time a possible prognostic role for BACH2 in CLL and have revealed protein interaction networks shared by BCL6 and BACH2, indicating a significant role for BACH2 and BCL6 in key cellular processes, including ubiquitination mediated B-cell receptor functions, nucleic acid metabolism, protein degradation, and homeostasis in CLL biology.

    The fabrication of Nano-based shielding materials is an advancing research area in material sciences and nanotechnology. Although bulky lead-based products remain the primary choice for radiation protection, environmental disadvantages and high toxicity limit their potentials, necessitating less costly, compatible, eco-friendly, and light-weight alternatives. The theme of the presented investigation is to compare the ionization radiation shielding potentialities of the lead acetate (LA), lead nitrate (LN), and bismuth nitrate (BN)-doped zinc oxide nanorods-based thin films (ZONRs-TFs) produced via the chemical bath deposition (CBD) technique. The impact of the selected materials' doping content on morphological and structural properties of ZONRs-TF was investigated. The X-ray diffractometer (XRD) analyses of both undoped and doped TFs revealed the existence of hexagonal quartzite crystal structures. The composition analysis by energy dispersive (EDX) detected the corrected elemental compositions of the deposited films. Field emission scanning electronic microscope (FESEM) images of the TFs showed highly porous and irregular surface morphologies of the randomly aligned NRs with cracks and voids. The undoped and 2 wt.% BN-doped TFs showed the smallest and largest grain size of 10.44 nm and 38.98 nm, respectively. The linear attenuation coefficient (µ) values of all the optimally doped ZONRs-TFs measured against the X-ray photon irradiation disclosed their excrement shielding potency. The measured µ values of the ZONRs-TFs displayed the trend of 1 wt.% LA-doped TF > 1 wt.% LN-doped TF > 3 wt.% BN-doped TF > undoped TFs). The values of μ of the ZONRs-TFs can be customized by adjusting the doping contents, which in turn controls the thickness and morphology of the TFs. In short, the proposed new types of the LA-, LN- and BN-doped ZONRs-TFs may contribute towards the development of the prospective ionization radiation shielding materials.

    This study presents the rheological behavior of water-based GO-TiO2-Ag and rGO-TiO2-Ag ternary-hybrid nanofluids. The impact of nanoparticles' volumetric concentration and temperature on the rheological properties were studied. All experiments were performed under temperatures ranging from 25 to 50 °C in the solid volume concentration range of 0.5-0.00005%. The data optimization technique was adopted using the Taguchi method. The types of nanomaterials, concentration, temperature, and shear rate were chosen to optimize the viscosity and shear stress. The effect of shear stress, angular sweep, frequency sweep, and damping factor ratio is plotted. The experimental results demonstrated that the rheological properties of the ternary hybrid nanofluid depend on the ternary hybrid nanofluid's temperature. The viscosity of ternary hybrid nanofluids (THNf) change by 40% for GO-TiO2-Ag and 33% for rGO-TiO2-Ag when temperature and shear rates are increased. All the ternary hybrid nanofluids demonstrated non-Newtonian behavior at lower concentrations and higher shear stress, suggesting a potential influence of nanoparticle aggregation on the viscosity. The dynamic viscosity of ternary hybrid nanofluid increased with enhancing solid particles' volume concentration and temperature.

    The object of this research is concrete-filled steel tubes (CFST). The article aimed to develop a prediction Multiphysics model for the circular CFST column by using the Artificial Neural Network (ANN), the Adaptive Neuro-Fuzzy Inference System (ANFIS) and the Gene Expression Program (GEP). The database for this study contains 1667 datapoints in which 702 are short CFST columns and 965 are long CFST columns. The input parameters are the geometric dimensions of the structural elements of the column and the mechanical properties of materials. The target parameters are the bearing capacity of columns, which determines their life cycle. A Multiphysics model was developed, and various statistical checks were applied using the three artificial intelligence techniques mentioned above. Parametric and sensitivity analyses were also performed on both short and long GEP models. The overall performance of the GEP model was better than the ANN and ANFIS models, and the prediction values of the GEP model were near actual values. The PI of the predicted Nst by GEP, ANN and ANFIS for training are 0.0416, 0.1423, and 0.1016, respectively, and for Nlg these values are 0.1169, 0.2990 and 0.1542, respectively. Corresponding OF values are 0.2300, 0.1200, and 0.090 for Nst, and 0.1000, 0.2700, and 0.1500 for Nlg. The superiority of the GEP method to the other techniques can be seen from the fact that the GEP technique provides suitable connections based on practical experimental work and does not rely on prior solutions. It is concluded that the GEP model can be used to predict the bearing capacity of circular CFST columns to avoid any laborious and time-consuming experimental work. It is also recommended that further research should be performed on the data to develop a prediction equation using other techniques such as Random Forest Regression and Multi Expression Program.

    The use of different sustainable materials in the manufacture of ultra-high-performance concrete (UHPC) is becoming increasingly common due to the unabating concerns over climate change and sustainability in the construction sector. Reactive powder concrete (RPC) is an UHPC in which traditional coarse aggregates are replaced by fine aggregates. The main purpose of this research is to produce RPC using dune sand and to study its microstructure and mechanical properties under different curing conditions of water curing and hot air curing. The effects of these factors are studied over a long-term period of 90 days. Quartz sand is completely replaced by a blend of crushed and dune sand, and cement is partially replaced by using binary blends of ground granulated blast furnace slag (GGBS) and fly ash (FA), which are used alongside silica fume (SF) to make a ternary supplementary binder system. Microstructural analysis is conducted using scanning electron microscopy (SEM), and engineering properties like compressive strength and flexural strength are studied to evaluate the performance of dune sand RPC. Overall, the results affirm that the production of UHPC is possible with the use of dune sand. The compressive strength of all mixes exceeded 120 MPa after 12 h only of hot air curing (HAC). The SEM results revealed the dense microstructure of RPC. However, goethite-like structures (corrosion products) were spotted at 90 days for all HAC specimens. Additionally, the use of FA accelerated the formation of such products as compared to GGBS. The effect of these products was insignificant from a mechanical point of view. However, additional research is required to determine their effect on the durability of RPC.

    The 5083 and 6061(T6) aluminum (Al) alloys are widely used in transportation industries and the development of structural designs because of their high toughness and high corrosion resistance. Friction stir welding (FSW) was performed to produce the dissimilar welded joint of Al5083-Al 6061(T6) under different welding parameters. However, softening behavior occurred in the friction stir welded (FSWed) samples because of grain coarsening or the dissolution of precipitation-hardening phases in the welding zone. Consequently, this research intended to investigate the effect of the post-weld heat treatment (PWHT) method on the mechanical property improvement of the dissimilar FSWed Al5083-Al6061(T6) and governing abnormal grain growth (AGG) through different welding parameters. The results showed PWHT enhanced the mechanical properties of dissimilar joints of Al5083-Al6061(T6). AGG was obtained in the microstructure of PWHTed joints, but appropriate PWHT could recover the dissolved precipitation-hardening particle in the heat-affected zone of the as-welded joint. Further, the tensile strength of the dissimilar joint increased from 181 MPa in the as-welded joint to 270 MPa in the PWHTed joint, showing 93% welding efficacy.

    Aerospace equipages encounter potential radiation footprints through which soft errors occur in the memories onboard. Hence, robustness against radiation with reliability in memory cells is a crucial factor in aerospace electronic systems. This work proposes a novel Carbon nanotube field-effect transistor (CNTFET) in designing a robust memory cell to overcome these soft errors. Further, a petite driver circuit to test the SRAM cells which serve the purpose of precharge and sense amplifier, and has a reduction in threefold of transistor count is recommended. Additionally, analysis of robustness against radiation in varying memory cells is carried out using standard GPDK 90 nm, GPDK 45 nm, and 14 nm CNTFET. The reliability of memory cells depends on the critical charge of a device, and it is tested by striking an equivalent current charge of the cosmic ray's linear energy transfer (LET) level. Also, the robustness of the memory cell is tested against the variation in process, voltage and temperature. Though CNTFET surges with high power consumption, it exhibits better noise margin and depleted access time. GPDK 45 nm has an average of 40% increase in SNM and 93% reduction of power compared to the 14 nm CNTFET with 96% of surge in write access time. Thus, the conventional MOSFET's 45 nm node outperforms all the configurations in terms of static noise margin, power, and read delay which swaps with increased write access time.
