Diabetes mellitus is the most common endocrine defect among children. Good diabetes management is important to give better health for both the children and the whole family. The goal of this study was to identify the level of mother involvement in children type·1 Diabetes Mellitus management, to identify factors related with mother involvement and to determine the relationships between mother
involvement and glycemic control. A cross·sectional study was carried out with 92 respondents selected by universal sampling from mothers who had diabetic children aged 5-15 years old and from medical visit appointment at University Kebangsaan Malaysia Medical Centre (UKMMC) and Putrajaya Hospital (HP1). Data was obtained through medical record and self reporting questionnaire. The study shown that 58.6% mothers had high involvement and 41.4% mothers had low involvement. Mothers' education level had a significant (p=0.0Z) relationship with a level of mother involvement. Most mothers gave high involvement in diet management (64.1%) and blood glucose monitoring (52. 7%), however most mothers gave low involvement in exercise management (63.0%) and insulin injection (60.9%). Mothers involvement in four management tasks were not statistically significant (p>0,05) with glycemic control. Mothers involvement in children diabetic management was high. There was high mother involvement in children's insulin injection and dietary intake and low mother involvement in children`s blood glucose monitoring and exercise, This study abo showed that there was no significant relationship between level of mother involvement and glycemic control.

A cost-utility analysis was performed desferrioxamine treatment in thalctssaemia patients at two tertiary hospitals in Malaysia in 2004. A hundred and twelve transfusion dependent thalassaemia patients were grouped according to the status of desferrioxamine; optimum and sulwptimum. Cost analysis was from a patient and hospital perspectives while Quality Adjusted Life Years (QALYs) was the health outcome of choice. Incremental Cost·Effectiveness Ratio (ICER) was also stipulated to show the difference in cost for an additional QALY if patient currently on suboptimum desferrioxamine to switch using optimum desferrioxamine. Results on cost analysis showed the mean cost of treatment for thalassaemia patients on optimum desferrioxamirie was higher than those on sub·optimum desferrioxamine; (RM14, 775.00+SDRM4,737.00 and RM10,780+RM3,655, respectively). QALYs were 19.186+6.591 and 9.859+5275 in the optimum and suboptirnum group, respectively. Costutility analysis showed the cost per QALYs in optimum desferrioxamine group was RM59,045.00 compared to RM44,665.00 in suboptimum desferrioxamine group. ICER of patients on sub-optimum desferrioxamine switching to optimum desferrioxarnine was only RM420.39. Sensitivity analysis showed that the results were robust in the best and worst scenarios. In conclusion, although it is expensive for thalassaemia patients to use optimum desferrioxamine compared to sub-optimum desferrioxamine, the cost per QALYs gained was undoubtedly low.

Cancer screening is an important part of any cancer control program and the success of any screening program is partly dependent on having large numbers of the high risk population availing themselves of the service. The purpose of this study was to assess the cancer screening behavior of fulbtime employed women staff of an institute of higher learning using the Health Belief Model (HBM) construct. The results showed that the rate of reported regular cancer screening behavior (Pap smear screening, breast self examination (BSE), clinical breast examination (CBE) and mammography screening) were low. The rates for regular screening were 42.5% (Pap smear), 20.9% (BSE), 15.5% (CBE) and 9.4% (mammography). There were differences in the dimensions of the HBM between the women who reported regular screening and those that did not. The perceived barriers", "perceived benefits” and “motivation” dimensions were different in Pap smear screening, whereas the "confidence” dimension was different in BSE. Recommendations were made for a health education program targeting both women and men to increase uptake of cancer screening services by women.

Home phototherapy treatment has been available in the Klang Valley (comprising Petaling ]aya, Kuala Lumpur; Shah Alam and Klang) since 2003. This study was conducted to create awareness of the existence of home phototherapy and its usage in Malaysia. This was a retrospective study using 1297 informed consent forms that parents had to read and sign prior to the commencement of home phototherapy. lt was found that the majority ofthe babies was males (41.2%), at or over 36 weelds gestation (97.2%), from areas in Selangor (57.4%), fully breastfed (53 %) and referred by doctors (98%). The mean age of the babies at initiation of lwme phototherapy was 6. 7 days. The mean bilirubin level at the start of home phototherapy was 243.8 umoVL and the mean bilirubin level at the end of home phototherapy 5 was 139.3 umoVL. The mean decrease in bilirubin level was 103.12 umoVL and the mean number of days of usage was 3.5 days which is a daily decrement of about 29 umoVL (29.46 + 13.8). Home phototherapy remains a viable clinical option for full term babies with physiological jaundice.
A cross-sectional study was conducted in May 2007 on stress and stress self management among the prisoners and wardens of a prison in Selangor. This study was conducted as part of an elective posting for second year medical students and the topic was chosen to add on to the body of knowledge on stress among prisoners and wardens. A self administered questionnaire was distributed by random sampling and the respondents consisted of 100 prisoners and 97 wardens. Majority of the prisoners did not have stress with reference to interpersonal factors such as conflict with cellmates, conflict with wardens and conflict with inmates, For interpersonal factors, majority of the prisoners experienced stress because they felt bored (78%), thought they had no bright future (63%) and they also had no visitors (61%). Stress resulted in physical effects, whereby 67% admitted that they experienced health problems and 64% experienced sleep disturbance. Emotional effects of stress experienced by the prisoners were sadness (67%), anxiousness (63%) and confusion (56%). The major spiritual effect was regret (84%) followed by feeling closer to God (78%) and feeling high spirited (59%). The major social effects experienced by
the prisoners were low self esteem (63%) and embarrassment (58%). Methods of stress self management were prayers and conduct of religious activities (87%), sharing problems with friends (78%), doing recreational activities (77%), sleeping (72%) , doing a hobby (68) and seeking help from doctors (57%). As for the wardens, with reference to interpersonal factors, high level of stress was experience with reference ta prisoners behavior (82.5%) and problems with higher authorities (83.5 %), For intrapersonal factors, sleep disturbances (64%), career problems (76.3%), financial problems (73.2%) and feeling bored (75.3%) highly contributed to stress. Wardens physical effects were 38.1% health problems and 29.9% sleep disturbances. For emotional effects, must of them were depressed (54.6%), anxious (67%), angry (63.9%), confused (64.9%), frustrated (59.8%), threatened (53.6%) and patriotic (51.5%). The spiritual effect were feeling closer to God (86.6%), feeling responsible for duties (82.5%), regret doing the job (74.2%), feeling high spirited (58.8%). For social effects, low self esteem is the highest effect experienced (68%). Wardens managed stress by praying and conducting religious activities (91.8%), recreational activities (88.7%), doing a hobby (85.6%), sleeping (74.2%), and sharing problems (68%).

Oral myofibroma is a rare tumour which usually occurs in children and has been reported in the mandible, tongue, buccal mucosa with only a few cases reported from the gingiva. It appears alarming clinically due to its fast growth which may mimic a malignancy. However, it is completely benign and is usually treated by complete surgical excision with excellent prognosis. Clinically, myofibroma presents as a single swelling when it occurs on the gingiva, and more common lesions such as fibrous epulis, pyogenic granuloma and peripheral odontogenic fibroma, myofibroma are usually considered in the differential diagnosis. We present 4 additional cases of gingival myofibroma in children. Their ages ranged from 7 to 14 years. Three were girls and 1 patient was a boy. All presented with solitary gingival growths, ranging from 3 weeks to 2 months in duration, and raised the clinical diagnoses of peripheral giant cell granuloma, pyogenic granuloma and fibrous epulis. Histopathology of incisional biopsies revealed proliferation of streaming and whorled fascicles of
spindle cells around slit-like vascular spaces. The spindle cells were cytologically bland and were
immunopositive for vimentin and smooth muscle actin, but were negative for desmin and S-100 protein. All were treated by surgical excision.
Two duplex PCR assays were established for the detection of C. trachomatis (Ct), N. gonorrhoeae
(GC), M. hominis (Mh), and U. urealyticum (Uu). These assays were used on clinical specimens
obtained from women with Premature Rupture of Membrane or Post Partum Fever, from preterm
infants, as well as from women with uneventful pregnancies and their babies delivered vaginally
at term. The analytical sensitivity of the duplex PCR assays with internal controls incorporated is 7.0,
19.0, 5x103 and 7x102 genome copies per reaction for Ct, GC, Mh and Uu respectively. Specificity
was demonstrated by the amplification of only target DNA in the presence of other organisms.
Among 40 women with normal, at term, deliveries, there were 6 positives for Ct, 2 for GC and 1
for Uu. None of these women had signs of genital tract infection. The Mh/Uu PCR was positive in
11 of 40 PROM cases, with 7 women positive for Uu, 2 for Mh and 2 others for both organisms.
Of 40 blood cultures taken from post-partum maternal infections, 6 were positive for Ct and 1 for
Mh. Respiratory secretions from 30 premature neonates yielded 5 positives for Uu and one each
for Mh and Ct. In contrast, there was only 1 positive result (for Mh) in 30 mature neonates. With 1
exception, all mycoplasma and ureaplasma positives were confirmed by culture and the concordance
between paired tracheal aspirates and nasopharyngeal swabs from neonates was 96.7%. These results show the potential use of the duplex PCR assays for the diagnosis of maternal and neonatal disease caused by the four urogenital pathogens.
All known field isolates of enterovirus 71 (EV71) can be divided into three distinct genogroups (A,
B, C) and 10 subgenogroups (A, B1-5, C1-4) based on VP1 gene sequences. We examined VP1
gene sequences of 10, 12 and 11 EV71 strains isolated in peninsular Malaysia during the outbreaks
of hand, foot and mouth disease in 1997, 2000 and 2005 respectively. Four EV71 strains isolated in
the hand, foot and mouth disease outbreak of 2006 in Sarawak (Malaysian Borneo) were included
to describe their genetic relationship.
Four subgenogroups (C1, C2, B3 and B4) of EV71 co-circulated and caused the outbreak of hand,
foot and mouth disease in peninsular Malaysia in 1997. Two subgenogroups (C1 and B4) were noted
to cause the outbreak in 2000. In the 2005 outbreak, besides EV71 strains of subgenogroup C1,
EV71 strains belonged to subgenogroup B5 were isolated but formed a cluster which was distinct
from EV71 strains of the subgenogroup B5 isolated in 2003. The four EV71 strains isolated from
clinical specimens of patients with hand, foot and mouth disease in the Sarawak outbreak in early
2006 also belonged to subgenogroup B5. Phylogenetic analysis of the VP1 gene sequences showed
that the four Sarawak EV71 isolates belonged to the same cluster as the EV71 strains that were
isolated in peninsular Malaysia as early as May 2005. The data suggested that the EV71 strains
causing the outbreak in Sarawak could have originated from peninsular Malaysia.

Orthogonal Frequency Division Multiplexing (OFDM) is a successful technique in wireless communication systems. Frequency offset in the OFDM system leads to loss of orthogonality among subcarriers which results in the occurrence of intercarrier interference (ICI). To improve the efficiency of bandwidth performance in the ICI self-cancellation scheme, frequency domain partial response signaling (PRS) was investigated. In this study, the integer polynomial partial response coefficients were exploited to enhance carrier-to-interference power ratio (CIR) in the OFDM system. CIR was enhanced up to 4.1 dB to 5 dB when the lengths of PRS polynomial, K was 2 and 5, respectively.
Ocimum basilicum seed extracts were found to possess significant anti-ulcer activity against ethanol-induced ulceration in experimental animal models. Three groups of adult male rats were used, with each group consisting of six rats. Oral administration of absolute ethanol to rats pre-treated with 10% Tween 20® (Group 1) produced extensive haemorrhagic lesions of the gastric mucosa. Silicon, as the most important electronic material, has a lot of applications in the electronic industry and this includes the use of silicon in solar cells. One of the solar grade silicon production processes is the use of acid leaching for the removal of metallic impurities from silicon. The main advantage of this process for silicon purification is that it is based on a low temperature process. The purification of metallurgical grade silicon by acid leaching was studied as a function of time, temperature and etching. Based upon experimental results and under optimum conditions, it was possible to remove 41%, 71% and 25% of iron, calcium and aluminum respectively, with the use of aqua regia.
Molecular dynamics reaction simulation showed that the rate constant is not constant over the concentration profile of reactants and products over a fixed temperature regime, and this variation is expressed in terms of the defined reactivity coefficients. The ratio of these coefficients for the forward and backward reactions were found to equal that of the activity coefficient ratio for the product and reactant species. A theory was developed to explain kinetics in general based on these observations. Several other theorems had first to be developed, most striking of all was the inference that the excess Helmholtz free energy was the thermodynamical function which had a direct relation to these activity factors than the Gibbs free energy. The theory is applied to a class of ionic reactions which could not be rationalized using the standard Bjørn-Bjerrum theory of ionic reactions.
The increased human demand for animal proteins in Malaysia is led by several factors: population growth, urbanisation, income growth and changing consumer preferences. Meeting the projected increased demand in the future is an awesome and challenging task. Presently, the non-ruminant poultry and pig industries, mainly private sector led, make the most significant contribution to total animal protein supplies, and inefficient ruminant production systems lag well behind. The strategy for promoting productivity growth to increase animal protein supplies from ruminants requires concerted efficient natural resource management that can target specific production systems. Two distinct economic opportunities are the development of oil palm-based cattle and goat production. The value addition to oil palm cultivation due to the beneficial crop-animal-soil interactions are enormous. The prerequisites are inter-disciplinary efforts, holistic systems, participatory community-based research and development that are needs-based and address constraints, increased research investments, institutional commitment and a policy environment that can enhance total factor productivity in the future.
Linear stability analysis was used to investigate the onset of Marangoni convection in a two-layer system. The system comprised a saturated porous layer over which was a layer of the same fluid. The fluid was heated from below and the upper free surface was deformable. At the interface between the fluid and the porous layer, the Beavers-Joseph slip condition was used and in the porous medium the Darcy law was employed to describe the flow. Predictions for the onset of convection were obtained from the analysis by the perturbation technique. The effect of surface deformation and depth ratio, z (which is equal to the depth of the fluid layer/depth of the porous layer) on the onset of fluid motion was studied in detail.

In this study, real-time imaging was monitored for flowing solid particles when various baffles were created to block certain areas of the pipe. The generated flow regimes were full-flow, three-quarter-flow, half-flow and quarter-flow. A vertical pneumatic conveyor was designed to hold a 85 mm inner diameter pipeline. The four projection optical tomography systems used, applied the parallel beam projection approach and use infrared light sources so that the sensor was free of noise from the surrounding visible light source. The two orthogonal and two rectilinear projections were axial, but ideally they should have been in the same layer. The sensor readings could be related to the varying light intensity effects of the dropping particles and were used to provide cross-sectional distribution information for the conveyor. By using computer programming, the information was reconstructed to produce coloured images and concentration was obtained by reference to a colour code. The results obtained from this study showed how imaged flow followed the artificial flow regime. This study could benefit industrial production lines in maintaining the desired flow rates.
Spatio-temporal datasets are a collection of datasets where data can vary in both space and time. Theoretically, such datasets can be considered as continuous and discrete. For example, specification of the function, F: Ed T Rn, where Ed denotes d-dimensional Euclidean space, T = R* ∩ {} the domain of time and Rn an n-dimensional scalar field. Examples of such data sets include time-varying simulation results, film and videos, time-varying medical datasets, geometry models with motion or deformation, meteorological measurements, and many more. It is therefore highly desirable to use visualisation to summarize meaningful information in higher dimensional spatio-temporal datasets. Our aim is to conceive an efficient visual study to facilitate scientists in identifying temporal association among complex and chaotic atom movements in ion trajectories. An application that uses a streamline for spatial motion of ion trajectories and Colour Number Coding Scheme for temporal encoding of high degree of timeline events among mobile ions is proposed. With an anthology of the visual examples, it was revealed that this application would be beneficial for scientists to visually mine any 3D spatio-temporal dataset.
Comet 17P/Holmes was discovered by Edwin Holmes on 6 November 1892 while he was conducting regular observations of the Andromeda Galaxy (M31). Calculations using observation of its orbits established the perihelion date as 13 June and the orbital period as 6.9 years. The 1899 and 1906 appearances were observed, but the comet was only seen again in 1964. The comet has since been
observed on every subsequent return.
All relevant and essential data of an existing vehicle seat assembly line such as the operating time and processes, material handling system, workstation layout, bill of materials, equipment and hand tools, were collected and analyzed. The time standards for each of the vehicle seat assembly elements were established using work study techniques. A simulation approach was used to determine the productivity and effi ciency of the existing and proposed lines. Simulation technique was also used to determine and identify bottle-necks in both existing and proposed systems. Comparison of the existing assembly line and the proposed assembly line in terms of their productivity and effi ciency are also highlighted.

The Ministry of Science Technology and Innovation (MOSTI) Angkasawan (Astronaut) Programme achieved its mission to send the first Malaysian astronaut Sheikh Muszaphar Shukur by Soyuz TM11 to the International Space Ship (ISS) on 10 October 2007. He returned to earth, landing safely on 21 October 2007. Such a momentous event has carved out yet another milestone in the country’s
history and development of a civilisation based on science and technology for its people. This mission has provided the educational sector with an opportunity to initiate a curriculum innovation. Together with the Ministry of Education (MOE), the initiative was undertaken to improve techniques of teaching and learning (TL) and to broaden its scope to include space science and technology.
The objective of this paper is to model the extraction of carotenoid with supercritical carbon dioxide as the solvent. Experimental data for the high pressure vapour-liquid phase equilibrium of the binary system carbon dioxide-carotenoid was reviewed for the elevated temperatures of 313.15, 323.15, 333.15 K and pressures up to 500 bar. The experimental data was correlated and modeled using Redlich-Kwong equation of state and regular solution methods. The use of the equation of state as an empirical correlation for collating and predicting liquid-liquid and liquid-dense fluid equilibria is discussed. It was concluded that the estimation of some of the parameters required for these calculations would be difficult if the solute (carotenoid) was a complex substance about which little was known apart from its structural formula. An alternative procedure is to apply activity coefficient expression of the regular solution theory type to each phase. Calculations along these lines are described and the physical basis for applying these methods under the relevant conditions is discussed. The regular solution theory approach in particular was found to be encouraging for the mutual miscibility calculations for heavy components (such as carotenoid) particularly for substances sensitive to temperature, though the interaction parameters for he prediction activity coefficients must be regarded as pressure dependent.
