    This paper explains Needlestick and Sharp Injuries among Healthcare Workers in Saudi Hospitals. Presently there are number of factors that are responsible for health care workers injuries. Lack of secure environment and ignorance to safety measures can lead to injuries due to Needlestick and Sharp objects. There is a significant need of providing better working environment for health care workers in Saudi Arabia. Following daily practices and World Health Organization’s measures for taking the preventive steps for these injuries is necessary. Trying to avoid utilizing needles whenever secure and efficient substitutes are present, providing needle containers, avoiding re-capping and wearing gloves on both hands are some of the measures that could be taken to make sure these problems do not recur.

    An effective sharps injury prevention program includes several components that must work in concert to prevent healthcare personnel from suffering needlesticks and other sharp related injuries. Sharp injury management program in Kementerian Kesihatan Malaysia (KKM) is currently at surveillance state outlined in the Manual of Sharp Injury Manual Surveillance 2007. Latest data showed that as much as 1405 cases reported in Kementerian Kesihatan Malaysia facilities in 2011. In Hospital Melaka, high sharp injury prevalence was recorded with 25 per 1000 healthcare worker being affected each year. However, according to World Health Organization, surveillance data tend to underestimate the true rate of sharp injuries between 18% and 70%. Although reasons for sharp injury underreporting are well documented in many studies, less in known on what motivates healthcare personnel to report their injury. In order to encourage healthcare workers engagement to prevention program, these motivators need to be determined. Various factors affecting reporting behavior can be divided into organizational factors and health belief. The objective of the study is 1) to determine the level of awareness and compliance on sharp injury reporting among healthcare workers in Melaka, 2) To identify the relationship between awareness of reporting, organizational factors and health belief with sharp injury-reporting behavior among healthcare workers, 3) To determine the impact of awareness on reporting, organizational factors and health belief with sharp injury reporting behavior among healthcare workers, 4) To suggest improvements in sharp injury prevention program. Healthcare workers who reported sharp injury between 2011 and 2015 were invited to answers questionnaires pertaining these factors. Descriptive statistics, mean scores and multiple logistic regression were used for the analysis. Results and recommendation are discussed.

    Medical students face the threat of needle stick injury with the consequent risk of acquiring blood-borne infection by pathogens such as HIV, Hepatitis B and Hepatitis C while performing their clinical activities in the hospitals. A cross-sectional study was conducted among 417 final year medical students from Universiti Kebangsaan Malaysia (UKM), University Malaya (UM) and Universiti Putra Malaysia (UPM). The aims of the study were to determine the incidence of cases and episodes of needle stick injury among them in the past year. This study was also done to find out the factors that might be associated with the occurrence of this problem. The factors were sociodemographic factors, duration of exposure, level of knowledge of blood-borne diseases and Universal Precaution, perception of risk of blood-borne diseases and level of practice of Universal Precaution. The incidence of needle stick injury among medical students was 14.1% (59 cases). The total number of episodes of needle stick injury was 87 and the incidence of episodes among respondents was high i.e. 20.9%. The highest incidence of episodes of needle stick injury occurred in Obstetric & Gynaecology postings, followed by Medicine and Surgery. For clinical procedures, venepuncture had the highest incidence followed by setting up drips and giving parenteral injections. The results showed the students who had needle stick injury (cases) had lower scores in the practice of Universal Precautions than non-cases (p<0.05). There was a significant association between the level of practice of Universal Precautions and the number of episodes of needle stick injury, i.e. the higher the score for the practice of Universal Precautions, the lower the number of episodes (beta=-2.03 x 10(-2), p<0.05). This study showed that medical students are at risk of needle stick injury and blood-borne infections during their clinical activities while performing procedures on patients especially for those who were poor at practising Universal Precautions. Therefore some preventive measures should be taken by the management of the universities and medical students to avoid the occurrence of these problems.

    Glove perforation during surgery has always been a matter of concern as it increases the infection rate and the risk of transmission of blood borne diseases. To determine the common causes, the site and the awareness of glove perforations in orthopaedic surgery, a prospective study was conducted to assess the rate of glove perforation during 130 consecutive orthopaedic operations. All gloves worn by the surgical team were assessed after the surgery using the water-loading test. A total of 1452 gloves were tested, and the rate of perforation was 3.58%. Most of these perforations (61.5%) were unnoticed. The main surgeons had the most perforations (76.9%), followed by first assistants (13.5%) and second assistants (9.6%). Most perforations occurred at the non-dominant hand. The commonest site of perforation was the index finger followed by the thumb. Shearing force with instruments accounted for 45% of the noticed perforations. Majority of these occurred during nailing procedures (33%) and internal fixation without the use of wires (19%). Our rate of glove perforation is similar to other series. Most of them went unnoticed and were mainly due to shearing injuries rather than perforation by sharps. Therefore, there is an increased risk of contamination and break in asepsis during surgery.

    A questionnaire survey on cross-infection control was conducted among 1371 professionally trained dentists whose names appeared in the Malaysian Government Gazette of 1990. A 73.1 percent response rate was obtained. About 13 percent of the dentists routinely did not wear gloves during treatment of patients as opposed to 54 percent who routinely did. About 83 percent and 52 percent of dentists wore a mask and eyewear or glasses respectively when carrying out dental procedures. About 93 percent of dentists would use a new sterile needle for each patient and about 40 percent would wipe working surfaces with disinfectant after each patient. The practice of sterilizing handpieces was found to be uncommon as opposed to the sterilization of hand instruments. Variations were observed in some of the infection control measures by gender, seniority in service and employment status. More than one-third of the respondents had experienced puncture wounds during the last month prior to the survey.

    Nurses are an integral component of the health care delivery system. In discharging their duties, nurses encounter a variety of occupational health problems which may be categorized into biological hazards, chemical hazards, physical hazards, and psychosocial hazards. A review of some examples of each of these four types of hazards is presented in this article. Particular attention has been devoted to hepatitis B, acquired immunodeficiency syndrome, tuberculosis, cytotoxic drugs, anesthetic agents, needlestick injury, back pain, and stress.

    Health care workers (HCW) are constantly exposed to blood-borne illnesses through needle stick injuries (NSI). Despite the increasing trend of NSI, evidence regarding the actual practice of universal precautions among these HCWs is lacking. This study assessed the practice of universal precautions towards prevention of NSI among HCWs in a teaching hospital setting.
    Methods: This cross-sectional survey involved a newly-designed self-completed questionnaire assessing demographic data, exposure to NSI and practice of universal precautions. Questionnaires were distributed to every ward and completed questionnaires were collected after a period of 7 days.
    Results: A total of 215 HCWs responded to the survey. 35.8% were exposed to bodily fluid, with 22.3% had NSI in the last 12 months. Blood taking was the commonest procedure associated with NSI. Of practices of universal precautions, recapping needle and removing needle from syringe were still wrongly practiced by the HCWs assessed.
    Conclusion: NSI among HCW are still common despite the introduction of universal precautions in our hospital. Incorrect practices in handling sharps should be looked into in order to reduce the incidence of blood-borne illnesses through NSI in the hospital.

    Background: The occupationally acquired accident and injuries in Malaysian medical laboratories are still largely unexplored prior to this survey. Some of these questions are attempted in this survey and act as source of reference for the number and accident injuries in medical laboratories in the area of Klang Valley and also in Malaysia.
    Methods : This survey was carried out based on recordable cases throughout the calendar year of 2001 to 2005 from 3 main medical laboratories of Hospital Kuala Lumpur (HKL), Hospital Universiti Kebangsaan Malaysia (HUKM) and Pusat Perubatan Universiti Malaya (PPUM).
    Results : The average annual incident rate for this three medical laboratories is 2.05/100 full time equivalent (FTE) employees. The annual incident rate in individual medical laboratory is 2.04/100 FTE (HKL), 2.07/100 FTE (HUKM) and 2.04/100 FTE (PPUM) employees, respectively. The most common injury that is 25.3% of the total cases reported was due to cuts by sharp objects and the second most common injury was exposure to biohazard and chemical substances which constitutes 19.9% of the total cases. . Needle prick injury (16.8%), fire (8.4%), fall/slip (6.3%) and gases leak and locked in cold room were reported as one case each.
    Conclusion : The average incident rate from this study is remarkably similar compared with the incident injury rate reported by BLS (2006) which is 2.1/100 FTE in the average size of medical laboratory and diagnostic. Besides this incident rate of injury and illness as a comparison, it also can be used as a benchmark to evaluate the safety performance among medical laboratories in Malaysia.

    Needle Stick Injury is injuries caused by suture needle and hollow bore needle and does not include injuries via scalpel blades, lancet and glass pieces or by other means for the purpose of this training program. This problem needs to be prevented or minimized by embarking on an effective training program. The applicability of this program will be determined by the response of the participants to the contents and teaching methods which will in future effect the long term out come and the success of the Needle Stick Injury prevention and Universal Precautions training program. This Health Education program was designed to be used for the induction training of new workers as well as those already in service and is applicable to all levels of Health Care Workers especially those directly dealing with or handling syringes and needles. This Needle Stick Injury and Universal Precautions Training Program was carried out in one of the health facility under the Ministry of Health in Sabah and the outcome evaluation and analysis using paired t-test has shown a significant difference in their effectiveness in increasing the knowledge of the Health Care Workers after being exposed to the program (p < 0.01). The process evaluation was able to show that this approach was instrumental in creating awareness among the Health Care Workers who had attended this program. From the study, 98% (49) had not attended any formal training since joining the service, followed by 90% who had given a good overall rating for the program. For the lecture and its relevance, 90% and 74% for the hands–on training. About 77% of the participant was satisfied with the discussion. The program can be developed further and with it a comprehensive training module can be produced. The long-term benefits of such a training program will need to be evaluated in a future study.

    Needlestick injury has been recognized as one of the occupational hazards which results in transmission of bloodborne pathogens. A cross-sectional study was carried out among 136 health care workers in the Accident and Emergency Department of two teaching hospitals from August to November 2003 to determine the prevalence of cases and episodes of needlestick injury. In addition, this study also assessed the level of knowledge of blood-borne diseases and Universal Precautions, risk perception on the practice of Universal Precautions and to find out factors contributing to needlestick injury. Prevalence of needlestick injury among the health care workers in the two hospitals were found to be 31.6% (N = 43) and 52.9% (N = 87) respectively. Among different job categories, medical assistants appeared to face the highest risk of needlestick injury. Factors associated with needlestick injury included shorter tenure in one's job (p < 0.05). Findings of this study support the hypothesis that health care workers are at risk of needlestick injury while performing procedures on patients. Therefore, comprehensive infection control strategies should be applied to effectively reduce the risk of needlestick injury.

    Background and Aim: Healthcare workers (HCWs) routinely experience occupational blood and body fluid exposure (OBBE), including percutaneous injury and splash exposure to non-intact skin. The objective of this study was to determine the prevalence of OBBE and identify associated risk factors.

    Methods: A cross-sectional study was performed on 334 HCWs at 9 workstations. Data were collected with a self-administered questionnaire that consisted of four parts: sociodemographic variables, work-related information, knowledge about needle stick and sharps injury and splash exposure, and information regarding previous OBBE incidents. The data were analyzed by SPSS version 22.0 software.

    Findings: The prevalence of OBBE was 25.1% (95% confidence interval: 20.6-30.2), mostly due to percutaneous injuries, which were not reported to authorities. The highest proportions were among nurses and those working in the medical ward. Needle recapping practices were associated with almost a four-times higher risk of OBBE compared to no-recapping practices. HCWs who did not have any infection prevention training had a three-times higher risk of OBBE.

    Conclusion: Factors associated with OBBE are unsafe work practices, inadequate infection prevention training, and lack of knowledge regarding blood-borne infection. There is a need for more training and increased awareness about the risks of OBBE to reduce unsafe practices.

    Introduction: Clinical and sharp bins are commonly found in clinical settings. The usage of these bins is vital to ensure that health care facilities remain hygienic, free from any microbial transmissions and incidences of needle stick injuries. ‘oQ-Ba’ is a Japanese phrase that means bin. In real clinical settings, it is difficult to gather all bins at once while performing procedure due to limitations of space, frequency and mobility of bins. Therefore, ‘oQ-Ba’ the Universal Bins Trolley is developed to improve the waste management practice and increase the efficiency and effectivity of care delivery. Methods: A total of 40 respondents comprised of staffs from Emergency and Trauma De- partment and Haemodialysis Unit at Serian District Hospital were selected to use “oQ-Ba” the Universal Bins Trolley. They were given self-administered questionnaires pre and post-trials whichcomposed of ‘yes’ or ‘no’ answers to ob- tain feedbacks on the effectiveness of this project. Descriptive statistics include frequencies, means and percentages were used to analyze the data using the SPSS version 22. Results: The staffs found that the prototype helps to ensure the availability of the three bins as the bins are all attached together under one compartment. The staffs also found that the prototype is more convenient compared to the existing waste bins because of its mobility and unique design which consumed little amount of space. Conclusion: ‘oQ-Ba’ the Universal Bins Trolley provides handful of benefits to medical practitioners as it can ensure the availability of the three bins during a procedure.

    Accidental needle-stick injuries (NSIs) are a hazard for health-care workers and general public health. Orthopaedic surgeons may be more prone to NSIs due to the prevalence of bone spikes in the operative field and the use of sharp orthopaedic instruments such as drills, saws and wires. A hospital-based cross sectional study was conducted in the orthopedic wards of Melaka General Hospital. The prevalence of NSIs was 32 (20.9%) and majority of it occurred during assisting in operation theatre 13(37.4%). Among them six (18.8%) were specialist, 12(37.5%) medical officer, 10 (31.2%) house officer and four staff nurses (12.5%). Among the respondents 142 (92.8%) had been immunized against Hepatitis B and 148 (96.7%) participants had knowledge regarding universal precaution. The incidence of NSI among health care workers at orthopaedics ward was not any higher in comparison with the similar studies and it was found out that the prevalence was more in junior doctors compared with specialist and staff nurses and it was statistically significant.

    Future healthcare providers are facing the threat of needle stick injuries (NSIs) with the consequent risk of acquiring blood borne diseases. A cross sectional study was conducted from May 2017 to September 2017 among 327 students including medical, nursing and paramedic students from Faculty of Medicine UKM. This study aimed to determine the level of knowledge and awareness among students during their clinical years. A validated questionnaire was delivered via convenience sampling. Data was analyzed using SPSS 23 software and one sample t-test was used to compare with the standard setting value. The mean age of respondents was 23.32±2.37. The knowledge and awareness were significantly higher in both paramedic (knowledge (10.71 ± 1.64, p<0.001), awareness (5.50 ± 0.86, p<0.001), and nursing (knowledge (9.94 ± 1.38, p<0.001), awareness (5.35 ± 1.07, p<0.001) students as compared to standard setting, respectively. Meanwhile for medical students there was no significant difference in knowledge (9.95 ± 1.39, p=0.589) but significant difference in awareness (5.87 ± 1.03, p<0.001) compared to standard setting. The prevalence of NSIs was 8.3% with majority having at least one incident (85.2%) and main exposure is during medical posting (77.8%) during procedure needle recapping (70.4%) using hollow needle (63%). Incident of NSIs were mostly not reported (74.1%). Despite a significant good result on knowledge and awareness, the incidence of NSIs is alarming. Therefore, preventive steps need to be taken by the university to avoid the occurrence.

    BACKGROUND: Needlestick injury (NSI) is a significant occupational health issue among healthcare workers (HCWs).

    AIMS: To determine the national self-reported incidence and risk factors for NSI among Malaysian Ministry of Health (MOH) HCWs.

    METHODS: Using data from the MOH national sharps injury surveillance programme, information on reported NSIs over a 1-year period (2016) for different HCW subgroups were extracted and analysed.

    RESULTS: A total of 1234 NSI cases were reported in 2016, giving an overall incidence of 6 injuries per 1000 HCWs. Medical doctors recorded the highest incidence (21.1 per 1000 HCWs) followed by dental staff (7.5), pharmacy staff (4.2), nurses (3.7), medical assistants (3.4) and allied and auxiliary staff (1.0). Doctors had significantly increased risk of NSI compared with allied and auxiliary staff (relative risk [RR] = 20.7, 95% confidence interval [CI] 15.5-27.5), medical assistants (RR = 6.1, 95% CI 4.5-8.2), nurses (RR = 5.7, 95% CI 5.0-6.6), pharmacy staff (RR = 5.0, 95% CI 3.7-6.6) and dental staff (RR = 2.8, 95% CI 2.2-3.5). Significant differences were found in age and sharps- handling experience between occupational subgroups (P < 0.001 for both variables). Male employees had higher risk than females (RR = 1.33, 95% CI 1.18-1.50), with a significant difference seen in their sharps-handling experience (P < 0.01). Important risk factors included unsafe practices such as recapping of needles and their improper disposal.

    CONCLUSIONS: The national incidence of NSI amongst Malaysian HCWs was lower compared with other countries, but unsafe practices remain an important concern. There is a need to formulate, implement and monitor safe and consistent practices for the different healthcare professionals.

    This study aimed to assess the knowledge of blood-borne diseases transmitted through needle stick injuries amongst health-care workers in a tertiary teaching hospital. We also aimed to assess the practices of universal precautions amongst these workers and its correlation with the facts. We carried out a cross-sectional study from January to July 2008 involving various levels of health-care workers in Serdang Hospital, Selangor, Malaysia. A self-administered questionnaire assessing knowledge of blood-borne diseases and universal precautions, and actual practice of universal precautions was used. Two hundred fifteen respondents participated in this study; 63.3% were staff nurses. The mean knowledge score was 31.84 (SD 4.30) and the mean universal practice score was 9.0 (SD 2.1). There was a small, positive correlation between knowledge and actual practice of universal precautions (r = 0.300, n = 206, p < 0.001) amongst the cohort studied. Factors such as age and years of experience did not contribute towards acquisition of knowledge about blood-borne illnesses or the practice of universal precautions.

    Occupational risk of Human Immunodeficiency Virus (HIV) infection is a matter of concern for health care workers. We conducted a study to gauge the level of awareness amongst HCW (doctors and nurses) working in Hospital Sungai Petani regarding the post-exposure prophylaxis in case of needle stick injuries from confirmed or suspected cases of HIV. Nineteen Doctors (56%) and 13 nurses (25%) were aware of correct risk of transmission. None identified all the four risk fluids correctly. 94% of doctors and 98% of nurses correctly stated that washing the site with soap and water is the initial procedure, but only few (1/10 of doctors and 1/3 of nurses) knew whom to contact immediately after injury. Twenty three doctors (67%) and 41(78%) nurses were aware of the use of Zidovudine but only 10 participants were aware of the use of second drug. Only 6 doctors (17%) and 8 nurses (15%) knew the correct duration of post-exposure prophylaxis. Twenty-three doctors (67%) and 35 nurses (67%) knew that the drugs were available in Hospital Pharmacy and 11 doctors and 12 nurses knew the approximate cost of therapy. On the average about 50% of doctors and nurses have fair knowledge of post exposure prophylaxis against HIV. Ongoing awareness and training are necessary to improve the same.

    Introduction: Needlestick injuries (NSIs) are common occupational hazards against healthcare workers (HCWs). NSIs led to serious economic burden as there were high costs forhealth system and the society, as well as the psycholog-ical impact on exposed workers post injury. Methods: A retrospective cohort study was conducted to determine the incidence, contributing factors of NSIs and to estimate the cost of PEM in Sabah’s tertiary hospitals. Descriptive statis-tics were used to analyse and describe the trends of NSIs. Chi-Square test was performed to determine the association between variables, relative risk was calculated. Results: 145 cases out of 7075 employees were reported from 2017 to 2018. Results in this study showed that the highest incidence of NSIs was 2.4/100 employees in Hospital Queen Elizabeth. 1.1/100 employees in 2018, which was higher compared to year 2017. Most common among younger age group between 20-29 years old, among doctors, with median working experience of 2 years. NSIs occurred com-monly at the wards, during blood taking, disposal of devices and common device used were hypodermic needles.     Sharps injury among health care workers in HUKM has received serious attention lately due to the possible transmission of diseases like HIV, Hepatits B and Hepatitis C, which can cause serious consequences for the victim. The aim of the study was to gain a deeper understanding of the emotional problems experienced by health care workers who had incurred a sharps injury and the coping strategies adopted by them to overcome the problem. A 28 item semi-structured questionnaire was designed by the supervisors and the researcher as there was no existing questionnaire to be adopted, as no research had been carried out in this area prior to this. The target group consisted of 64 health care workers of different categories working in HUKM. Data was collected over a period of five months from November 2000 to March 2001. The results of the study revealed that the most frequently injured were the staff nurses, nearly 90% of whom were emotionally upset and were worried that they might contract a disease. There was no significant difference in the emotional stress experienced by the different categories of the health care workers, all were equally upset about the injury but each category of staff adopted different methods to overcome the tragedy. Open comments and opinions about prevention and the management of the sharps injury in HUKM as given by the respondents here are taken into account. Several views given by the participants were explored and discussed. Finally, several implications for nursing and recommendations have been presented.
