    Microleakage testing has been used to determine the possible clinical performance of a restorative material. Many microleakage testing materials have been developed and performed through the years. There has been no agreement as to which testing methodology would give the most accurate results. Attempts have been made to simulate the oral conditions and to give a more quantitative representation of micro leakage. The different micro leakage testing methodologies are presented in this paper.

    The lifespan of dental restorations is limited. Longevity depends on the material used and the different characteristics of the dental piece. However, it is not always the case that the best and longest lasting material is used since patients may prefer different treatments according to how noticeable the material is. Over the last 100 years, the most commonly used material has been silver amalgam, which, while very durable, is somewhat aesthetically displeasing. Our study is based on the collection of data from the charts, notes, and radiographic information of restorative treatments performed by Dr. Vera in 1993, the analysis of the information by computer artificial intelligence to determine the most appropriate restoration, and the monitoring of the evolution of the dental restoration. The data will be treated confidentially according to the Organic Law 15/1999 on 13 December on the Protection of Personal Data. This paper also presents a clustering technique capable of identifying the most significant cases with which to instantiate the case-base. In order to classify the cases, a mixture of experts is used which incorporates a Bayesian network and a multilayer perceptron; the combination of both classifiers is performed with a neural network.

    Raising the thermal expansion coefficient (CTE) of dental porcelains is important to match the CTE of the ceramic material with the higher CTE of the metal inlay in dental restorations. The higher thermal expansion of the leucite phase increases the overall thermal expansion coefficient of the dental porcelain. Potassium nitrate (KNO3) additions in controlled percentages to the base dental porcelain formulation help in the formation of a leucite phase. The percentage added was 5,10 and 20 weight percent of leucite, respectively, to the total base frit composition. The change in CTE values was then investigated using a Linseis Dilatometer. A 20wt% KNO3 addition resulted in a CTE of 9.0 microm/m-K compared to the 7.7 microm/m-K CTE of the base composition. The microstructures observed under the scanning electron microscope (SEM) show a multiphase material with the leucite phases dispersed within a glassy matrix. The results suggest that higher CTEs in the dental porcelain are possible by increasing the KNO3- additions within the limits tested.

    OBJECTIVE: To investigate the current and future teaching of posterior composite restorations in undergraduate curricula in Malaysian dental schools.

    METHODS: A 24-item validated questionnaire including closed and open questions on the teaching of posterior composites was emailed to faculty members in all 13 Dental Schools in Malaysia. Responses were compiled on Excel and analysed.

    RESULTS: All 13 dental schools responded to the survey yielding a 100 % response. All schools indicated the use of posterior composites for 2- and 3-surface cavities in premolars and molars. The didactic teaching time devoted to composites was greater than for amalgam (38 h vs 29 h). Clinically, most posterior restorations placed by students were composites (average 74.1 %, range 10 %-100 %); the remaining 25.9 % were amalgams (range, 0 %-50 %). Slot-type cavities were the preparation techniques most commonly taught (n = 11,84.6 %). The use of rubber dam for moisture control was mandatory in most schools (n = 11, 84.6 %). History of adverse reaction to composites was found to be the most common contraindication to composite placement. The phase down of teaching and use of amalgam in Malaysia is expected to occur within the next six years.

    CONCLUSION: The trend to increase the teaching of posterior composites reported for other countries is confirmed by the findings from Malaysian dental schools. Notwithstanding this trend, the use of amalgam is still taught, and future studies are required to investigate the implications of the phase down of amalgam in favour of posterior composites.

    CLINICAL SIGNIFICANCE: Notwithstanding the increase in the teaching of posterior composites there is a pressing need to update and refine clinical guidelines for the teaching of posterior composites globally.

    This study was carried out to evaluate the microleakage of Class II cavities restored with various types of lining materials. Four types of composite resins (Esthet-X-Denstply, USA, FiltekTMZ350-3M ESPE, USA, Beautifil- Shofu, Japan and Solare P-GC, Japan) were used and the lining were the Fuji IXGP (GC, Japan), the Beautifil flow (Shofu, Japan), the FiltexTMZ350 flow (3M ESPE, USA) and the Esthet-X flow (Denstply, USA). All the specimens were thermocycled and immersed in 0.5% basic fuschin dye for 24 h. The microleakage was scored using the ISO microleakage scoring system. The data were entered using SPSS version 12.0 and analyzed using STATA software programme. This study showed that none of the materials used in this study was able to eliminate microleakage. However, it was shown that the glass ionomer cement was better in reducing the incident of microleakage at the cervical margin. Among the flowable composite resin, FiltexTMZ350 flow showed less microleakage at the cervical margin.

    The objective of this study is to share cost analysis methodology and to obtain cost estimates for posterior restorations in public sector dental clinics. Two urban and 2 rural dental clinics in Selangor state were selected. Only cases of 1 posterior restoration per visit by dental officers were included over 6 months. One capsulated amalgam type, 1 capsulated tooth-colored, and 1 non-capsulated tooth-colored material were selected. A clinical pathway form was formulated to collect data per patient. Annual capital and recurrent expenditures were collected per clinic. The mean cost of an amalgam restoration was RM 30.96 (sdRM 7.86); and tooth-colored restorations ranged from RM 33.00 (sdRM 8.43) to RM 41.10 (sdRM 10.61). Wherein 1 USD = RM 2.8. Restoration costs were 35% to 55% higher in clinics in rural areas than in urban areas. The findings demonstrate economy of scale for clinic operation and restoration costs with higher patient load. Costs per restoration were higher in rural than in urban dental clinics. More studies are recommended to address the dearth of dental costs data in Malaysia.

    Denture marking is accepted as a means of identifying dentures and persons in geriatric institutions, or post-mortem during war, crimes, civil unrest, natural and mass disasters. Labelling on the acrylic resin component of the denture can easily be damaged or destroyed by fire but on cobalt-chromium components it would be more resistant. A copper vapour laser (CVL) can be used to label the cobalt-chromium components of dentures and metal restorations easily, and legibly, and miniaturised for the incorporation of more personal particulars necessary for the identification of the deceased person. The CVL beam is focussed by its optics and delivered to the material surface by the two-axis scanner mounted with mirrors. A personal computer controls the movement of the scanner and the firing of the CVL. The high peak power of the pulsed CVL is focussed to very high energy density producing plasma ablation of the alloy surface. Very fine markings of a few microns width can be produced enabling the storage of detailed information of the deceased person on a metal surface for the purpose of rapid identification.

    This study assessed the nature of the adhesion in repaired glass-ionomer restorative materials. Two chemically different glass-ionomer cements, Ketac Fil and Chemfil II Cap, and three different methods of conditioning the surface for repair were employed. Specimens of each material were prepared and the cut surfaces were then treated with either 35% phosphoric acid, 35% polyacrylic acid or a combination of phosphoric acid followed by polyacrylic acid. Freshly mixed material was injected against these treated surfaces and allowed to set under simulated intraoral conditions. The specimens were tested to failure in flexion after seven days storage. Assessment of the fractured surfaces was then carried out using the scanning electron microscope. The results showed the occurrence of both adhesive and cohesive failure.

    This case report illustrates the successful management of a case of mutilated maxillary incisors in a 13 year old patient through a sequence of therapy designed to closely coordinate the various disciplines, endodontics, restorative dentistry and orthodontics, involved. Endodontics was directed towards the control of the infected pulp, restorative treatment was to reconstruct the broken down teeth in stages which were essential in the sequence of treatment, and orthodontics was involved in the achievement of functional occlusion and alignment. The comprehensive treatment approach combined to achieve satisfactory aesthetics and function.

    The aim of this study was to determine the periodontal status in relation to the quality of the adjacent proximal tooth surface. The gingival index (GI) and probable pocket depth (PD) were measured in 144 contra-lateral pairs of sound vs. restored and 95 contra-lateral pairs of sound vs. carious surfaces in 124 patients. The restorations studied were silver amalgams and tooth-coloured filling materials, the margins of which were supragingival or level with the gingival margins. The results showed that the GI and PD were greater for both restored and carious tooth surfaces than for the contra-lateral sound surfaces. When all the teeth were grouped together, the differences in the mean values of GI and PD were statistically significant at the P less than or equal to 0.05 level. A comparison between restored and carious (non-paired) tooth surfaces revealed higher mean values of GI and PD for the latter, which were highly significant at the P less than 0.001 level. Thus the present study shows that proximal tooth surface quality influences the health status of the adjacent periodontium.

    AIM: To evaluate caries spectrum among 12- and 15-year-old Indian children using the Caries Assessment Spectrum and Treatment (CAST) index.

    MATERIALS AND METHODS: An epidemiological survey of the schoolchildren was carried out in a district in India. A stratified cluster random sampling method was used to select the sample. The caries prevalence of the surveyed population was obtained by using this CAST tool. Chi-square test was used to verify the association between dental fluorosis, socioeconomic status, and age and caries experience. Mann-Whitney U-test was used to compare the caries experience between the two age groups.

    RESULTS: A total of 2,610 children were examined. The majority of the schoolchildren (12 years: 74.2%, 15 years: 75.5%) were healthy concerning their caries experience and the prevalence of the other codes was minimal. The prevalence of dentin carious lesions and the percent of restorable teeth was greater among the 15-year-olds. The mean decayed, missing, and filled teeth (DMFT) of 12- and 15-year-old subjects was calculated to be 0.22 and 0.29 respectively.

    CONCLUSION: The currently surveyed population showed a low caries prevalence and the use of the tool highlighted the caries spectrum in an impressive way.

    CLINICAL SIGNIFICANCE: A well-designed tool to assess the carious spectrum of an individual or a community, thus enabling the responsible stakeholders to plan an appropriate care that is necessary.

    BACKGROUND: The use of psychological testing to indicate the potential for dissatisfaction with dental treatment has many potential patient and clinician benefits but has been rarely investigated. The study aimed to explore the use of the Symptom Checklist-90-Revised (SCL-90-R) psychological testing instrument in describing the relationship between pre-treatment psychological traits and aesthetic restorative treatment satisfaction.

    METHODS: Thirty patients requiring aesthetic restorative dental treatment completed three questionnaires, namely 1) a pre-treatment expectation assessment, 2) an SCL-90-R analysis pre-treatment and 3) an outcome assessment post-treatment to assess patient's expectations and satisfaction of the proposed dental treatment relating to function, aesthetics, comfort and tissue preservation. Logistic regression models were used to assess the impact of psychological variables on patient satisfaction after adjusting for baseline expectations (P dental treatment. The ability to indicate aesthetic restorative treatment dissatisfaction is of great benefit to clinicians in maximising success and mitigating risk.
