    Objective: To evaluate the early results of patients in a single institution who underwent total knee arthroplasty using an implant system. Methods: We retrospectively reviewed 76 total knee replacements using the Genesis II prosthesis (Smith and Nephew, Memphis) in 60 patients (16 were bilateral cases) performed between February 2005 andFebruary 2008. Patient related outcomes and clinical evaluations were done by an independent observer. Knee function and patient satisfaction were evaluated using the American Knee Society (ASK) score and the Western Ontario and Mac Master University Osteoarthritis Index (WOMAC). Results: The mean age of the patients at the time of operation was 63.5years. The mean follow up period was 1 year 5 months. At the final follow up, the mean Knee Society Knee Score was 87.9 with 77.3% (58 knees) rated excellent, 21.3% (16 knees) rated good and1.3% (1 knee) rated fair; none were rated poor. The mean Function Score was 87.1 with 64% (48 knees) rated excellent, 29.3% (22 knees) rated good, 6.7% (5 knees) rated fair, no knee was rated poor. The mean WOMAC Score was 94.4 (standard deviation 6.59). Conclusion: Most patients treated for knee osteoarthritis with total knee replacement showed high rates of excellence and good early functional outcomes.

    Fracture of the femoral neck is one of the most common types of osteoporotic fractures. Hemiarthroplasty continues to be a good option despite current calls for total hip arthroplasty in this subset of patients. The hemiarthroplasty is less expensive and easier to perform, and can be adequately carried out in smaller hospitals with basic orthopaedic facilities staffed by general orthopaedists. Functional demands of the elderly among Asians are often less compared to similar subsets of Western population on which most comparative hip studies are based. This study was undertaken to investigate the outcome for femoral neck fractures treated with conventional hemiarthroplasty. The authors hope to provide information based on local data as a reference for our doctors and patients.

    Infection after joint replacement surgery is problematic and difficult to treat. The utility of antibiotic laden bone cement for reduction of risk of infection in primary and revision joint replacement surgery has already been established in many studies. In this study, we examined the efficacy of bone cement containing cefuroxime, employing a modified in vitro Kirby-Bauer susceptibility model for investigation of 13 strains of organisms that are found in orthopaedic infections. Organisms investigated were broad spectrum and effective for Gram-positive, Gram-negative, aerobic organisms and anaerobes. Simplex P with added cefuroxime was effective against 8 out of 13 strains. Cefuroxime is stable during exothermic polymerisation of the cement, and is released from the cement at concentrations high enough to inhibit the growth of most organisms encountered after joint arthroplasty.

    HP0986 protein of Helicobacter pylori has been shown to trigger induction of proinflammatory cytokines (IL-8 and TNF-α) through the activation of NF-κB and also to induce Fas mediated apoptosis of human macrophage cells (THP-1). In this study, we unravel mechanistic details of the biological effects of this protein in a murine macrophage environment. Up regulation of MCP-1 and TNF-α in HP0986-induced RAW 264.7 cells occurred subsequent to the activation and translocation of NF-κB to the cell nucleus. Further, HP0986 induced apoptosis of RAW 264.7 cells through Fas activation and this was in agreement with previous observations made with THP-1 cells. Our studies indicated activation of TNFR1 through interaction with HP0986 and this elicited the aforementioned responses independent of TLR2, TLR4 or TNFR2. We found that mouse TNFR1 activation by HP0986 facilitates formation of a complex comprising of TNFR1, TRADD and TRAF2, and this occurs upstream of NF-κB activation. Furthermore, FADD also forms a second complex, at a later stage, together with TNFR1 and TRADD, resulting in caspase-8 activation and thereby the apoptosis of RAW 264.7 cells. In summary, our observations reveal finer details of the functional activity of HP0986 protein in relation to its behavior in a murine macrophage cell environment. These findings reconfirm the proinflammatory and apoptotic role of HP0986 signifying it to be an important trigger of innate responses. These observations form much needed baseline data entailing future in vivo studies of the functions of HP0986 in a murine model.

    A case of traumatic posterior cruciate ligament (PCL) avulsion fracture presenting with unusual radiographic findings is described. CT scan of the right knee showed features suggestive of combined ACL and PCL avulsion fractures. Arthroscopic findings showed that the injury was in fact a PCL avulsion fracture that was displaced anteriorly so as to mimic an ACL avulsion fracture on CT scan.

    This study was conducted to evaluate the efficacy of intra-articular injection of hyaluronic acid for the treatment of knee osteoarthritis. Patients with knee osteoarthritis were followed for a period of six months to assess the efficacy of intra-articular injection of hyaluronic acid given three times in three consecutive weeks. Fifty patients were reviewed at two, eight and 24 weeks post-injection. The average age was 60.9 years and female to male ratio was 3:1. Patients were assessed using the Lequesne Algofunctional Index for function, and the visual analogue score for pain and side effects. We found that the knee pain reduced and the function improved in most patients and these beneficial effects maintained for till the last follow up. The only side effect noted was one case of acute non-septic joint effusion after the 3rd injection. We concluded that intra-articular injection of hyaluronic acid can produce pain relief and functional improvement for up to 6 months.

    Venous thromboembolism is a rising concern in Asia especially among patients after surgery where this complication is readily preventable. Despite the availability of several treatment options, the acceptance of prophylaxis and usage of these methods remain low. A possible explanation to this behavior is the limitations attached to the available treatment options: narrow therapeutic window of warfarin and parenteral administration of low molecular weight as well as unfractionated heparins. Newer agents have been researched and introduced to overcome these limitations in the hope of improving the adaptation towards post surgical thromboprophylaxis. Dabigatran and rivaroxaban are two such new agents that are promising in view of efficacy and ease of administration.

    Acute traumatic transphyseal fracture of the capital femoral epiphysis is a rare but serious injury. The injury is typically inflicted by a severe trauma. Because of the vulnerability and predisposed anatomy of the femoral epiphysis in relation to its blood supply, the fracture has been designated to have poor prognosis with inevitable osteonecrosis and eventual deformity of the hip. We report a case of such fracture in a 13-year-old child in view to highlight some of the anticipated problems in the management of such injury.

    COVID-19 pandemic has created an extreme pressure on the global healthcare services. Fast, reliable, and early clinical assessment of the severity of the disease can help in allocating and prioritizing resources to reduce mortality. In order to study the important blood biomarkers for predicting disease mortality, a retrospective study was conducted on a dataset made public by Yan et al. in [1] of 375 COVID-19 positive patients admitted to Tongji Hospital (China) from January 10 to February 18, 2020. Demographic and clinical characteristics and patient outcomes were investigated using machine learning tools to identify key biomarkers to predict the mortality of individual patient. A nomogram was developed for predicting the mortality risk among COVID-19 patients. Lactate dehydrogenase, neutrophils (%), lymphocyte (%), high-sensitivity C-reactive protein, and age (LNLCA)-acquired at hospital admission-were identified as key predictors of death by multi-tree XGBoost model. The area under curve (AUC) of the nomogram for the derivation and validation cohort were 0.961 and 0.991, respectively. An integrated score (LNLCA) was calculated with the corresponding death probability. COVID-19 patients were divided into three subgroups: low-, moderate-, and high-risk groups using LNLCA cutoff values of 10.4 and 12.65 with the death probability less than 5%, 5-50%, and above 50%, respectively. The prognostic model, nomogram, and LNLCA score can help in early detection of high mortality risk of COVID-19 patients, which will help doctors to improve the management of patient stratification.

    The coronavirus disease 2019 (COVID-19) after outbreaking in Wuhan increasingly spread throughout the world. Fast, reliable, and easily accessible clinical assessment of the severity of the disease can help in allocating and prioritizing resources to reduce mortality. The objective of the study was to develop and validate an early scoring tool to stratify the risk of death using readily available complete blood count (CBC) biomarkers. A retrospective study was conducted on twenty-three CBC blood biomarkers for predicting disease mortality for 375 COVID-19 patients admitted to Tongji Hospital, China from January 10 to February 18, 2020. Machine learning based key biomarkers among the CBC parameters as the mortality predictors were identified. A multivariate logistic regression-based nomogram and a scoring system was developed to categorize the patients in three risk groups (low, moderate, and high) for predicting the mortality risk among COVID-19 patients. Lymphocyte count, neutrophils count, age, white blood cell count, monocytes (%), platelet count, red blood cell distribution width parameters collected at hospital admission were selected as important biomarkers for death prediction using random forest feature selection technique. A CBC score was devised for calculating the death probability of the patients and was used to categorize the patients into three sub-risk groups: low (<=5%), moderate (>5% and <=50%), and high (>50%), respectively. The area under the curve (AUC) of the model for the development and internal validation cohort were 0.961 and 0.88, respectively. The proposed model was further validated with an external cohort of 103 patients of Dhaka Medical College, Bangladesh, which exhibits in an AUC of 0.963. The proposed CBC parameter-based prognostic model and the associated web-application, can help the medical doctors to improve the management by early prediction of mortality risk of the COVID-19 patients in the low-resource countries.
