Introduction: Chemical pesticide is commonly used among vegetable family farmers in Sabah. There is no system to monitor adverse health effects. This study is to determine the blood cholinesterase activity and associated factors among vegetable family farmers. Methods: A cross sectional study has involved 163 farmers aged 18-60 years old. Validated questionnaire was used to assess the associated factors. Lovibond AF267 Cholinesterase Kit was used to determine the blood cholinesterase activity. Chi square test was used to determine the association between blood cholinesterase activity and socio demographic factors and work practices. The lower percentage of water, sanitation and hygiene are the root causes of diarrhoea and cholera. Cholera is a sudden onset of acute watery diarrhoea which can progress to severe dehydration and death if untreated. The current pandemic, Vibrio Cholera O1 started in 1961. This study explores water, sanitation, hygiene and cholera and diarrhoea in three affected villages of Beluran District, Sabah Malaysia to support effective and timely public health intervention. This cross sectional study uses purposive sampling. All (114) households were interviewed and household water samples collected. The study reported lower coverage improved sanitation facilities (35.3% to 52.3%), no latrine at home (37% to 63%), improved water supply (52% to 60%), and prevalence of hand washing after toilet (57% - 74%). For water quality, Ecoli was present in household water (32% to 37%) but Vibrio cholerae was not isolated in any of the water samples tested. Statistically significant associations were found for; 1) occupation−nonagriculture and unimproved sanitation facility and 2) house ownership and correct knowledge of ORS preparation. Predictors for household water quality were: latrine at home, and improved household toilet. Aggressive strategies to improve water supply, sanitation and hygiene−hand washing after toilet−were recommended for future prevention of cholera and diarrhoea in the affected area.
Introduction: University-wide healthcare programme are difficult to implement without complete protocols. This paper describes a collaborative academia-nursing programme to design a quick, user-friendly primary care screen- ing toolkit, to be used at community level at each faculty in UMS. Methods: A Primary Care Condition Assessment Questionnaire was designed by family medicine physicians, mental health doctors, and public health physicians. The questionnaire was pilot tested in 2 different faculties. The Primary Care Condition Assessment Questionnaire was manualized, and a one-day intervention training programme was administered. Subsequently 19 nurses and assistant medical officers were trained in the questionnaire administration and given concurrent communication skills and collaborative practice training to operate the questionnaire. Qualitative assessments of abilities to perform common primary health assessments were performed. Results: Trained nurses qualitatively felt they were more con- fident to perform primary care screening of common healthcare conditions and were able to deliver advice and refer screen-positive individuals to appropriate referral pathways. Conclusion: Public health programmes like HUMS2U put healthcare into the hands of nurses, allowing task-shifting to adequately trained non-specialist professions, and empowers nurses in basic non-communicable disease training and management. Further research will be performed to assess efficacy of the programme at all 23 faculties and departments of the university.
