    Calcinosis cutis is a rare presentation and not many cases have been reported especially of idiopathic type. We are reporting a case of idiopathic calcinosis cutis of lower limbs in a 33-year old female who presented to our clinic for multiple painless swellings over her lower limbs for the past six months, without any history of trauma or infection. We have decided to observe her condition on regular follow-up and conservative management.

    INTRODUCTION: Psychosocial stressors appear to alter the state of mind and adoption of overeating behaviour, resulting in high body mass index. This study was conducted to determine the magnitude of psychosocial stressors on male employees' well-being.

    METHOD: This study used secondary data retrieved from a cross-sectional study involving 492 male employees' completed data. Eligible participants completed validated questionnaires of the Psychosocial Safety Climate (PSC-12) scale, short version Demand Induced Strain Compensation (DISQ 2.1), Oldenburg Burnout Inventory - Emotional Exhaustion domain and the Three Eating Factor Questionnaire (TEFQ) -Uncontrolled Eating domain; assessing psychosocial safety climate, job demands and job resources, emotional exhaustion, and uncontrolled eating behaviour, respectively. Body mass index (BMI) was calculated based on weight and height. The research statistical model was tested by two-steps of assessment replicating partial least squares structural equation modelling (PLS-SEM).

    RESULT: The results show that psychosocial stressors (psychosocial safety climate, job demands and job resources) had significant effects on emotional exhaustion (β= -0.149, p=0.004; β= 0.223, p<0.001; β= -0.127, p=0.013). Emotional exhaustion predicted by work stressors may act as a chain reaction which could result in uncontrolled eating (β=0.138, p=0.005) and high BMI (β=0.185, p<0.001). Emotional exhaustion does mediate the relationship between PSC and uncontrolled eating behaviour (β= -0.021 [95% boot CI bias corrected: -0.048, -0.002]).

    CONCLUSION: The psychosocial stressors at work are significant factors for emotional exhaustion, which further signifies the positive effect on uncontrolled eating behaviour and BMI among Malaysian male employees.

    Background: Varicella has been known to be a harmless childhood disease. However, it has been reported that severe complications have taken place following Varicella infection, in both immunocompetent, as well as immunocompromised, individuals. Cutaneous complications of Varicella may manifest as preseptal cellulitis, albeit rarely.

    Report: We present a case of a 4-year-old boy who presented with symptoms and signs of preseptal cellulitis following Varicella infection. He was referred to the otorhinolaryngology team for a nasoendoscopy to rule out sinusitis, in view of the fear that a child presenting with a swollen red eye may be a case of true orbital cellulitis. He was treated successfully with intravenous antibiotics and surgical drainage of the preseptal collection.

    Conclusion: It is imperative for clinicians to be aware that a simple Varicella infection may lead to cutaneous complications in the pediatric age group, especially in children who are 4 years and younger. They may develop preseptal cellulitis, whose presentation might mimic that of orbital cellulitis. Empirical treatment with antibiotics would be advantageous for the patient. A nasoendoscopic examination may also be warranted in these cases to rule out sinusitis as a cause of orbital cellulitis.

    Chlorhexidine is labelled as hidden allergen as the health care professionals (HCPs) are unaware of the wide range of products containing chlorhexidine. Adverse events from chlorhexidine allergy can be reduced by appropriate perioperative management especially heeding on positive history during preoperative assessment, awareness regarding this hidden allergen, and educating HCPs on possible chlorhexidine-containing products. The regulatory agencies all over the world have issued recommendations regarding safety and risk of hypersensitivity reactions with chlorhexidine-containing products. The onus lies on HCPs to disseminate this knowledge to the stakeholders. We present a brief update to combat chlorhexidine allergy in perioperative setting.

    Background: Colorectal cancer (CRC) is one of the major causes of morbidity and mortality. According to National Cancer Registry, the incidence of colorectal cancer in Peninsular Malaysia increases with age. The incidence is highest among Chinese population but lower among Indians and Malays. Many reviews have suggested that obesity may be associated with a higher risk (>50%) of colorectal cancer. Methods: This study collects a comprehensive data from the literature review available from respective journals on dietary intervention and the chemo-protective mechanisms of a few natural resources in obesity -associated colon cancer based on previous and current studies. Results: In obesity-associated colon cancer, the genes of interest and pathways that are mainly involved include NFκB, P13K/Akt, and MAPK pathways, and FTO, leptin, Cyclin D, MMPs, and STAT3 genes. Dietary modification is one of the alternative steps in early prevention of colon cancer. It has been proposed that the components present in certain foods may have the ability to protect against many diseases including the prevention of cancer. Conclusion: There are many factors that lead to obesity-associated colon cancer and the mechanisms behind it is still undergoing intensive research. This review aims to scrutinize research as well as reviews that have been previously reported on obesity associated colorectal cancer and the beneficial effects of including antioxidants-rich foods such as vegetables and fruits in the diet to reduce the risk of obesity associated colorectal cancer.

    Health has been the main driver for many urban environmental interventions, particularly in cases of significant health problems linked to poor urban environmental conditions. This paper examines empirically the links between climate change mitigation and health in urban areas, when health is the main driver for improvements. The paper aims to understand how systems of urban governance can enable or prevent the creation of health outcomes via continuous improvements in the environmental conditions in a city. The research draws on cases from two Indian cities where initiatives were undertaken in different sectors: Surat (waste) and Delhi (transportation). Using the literature on network effectiveness as an analytical framework, the paper compares the cases to identify the possible ways to strengthen the governance and policy making process in the urban system so that each intervention can intentionally realize multiple impacts for both local health and climate change mitigation in the long term as well as factors that may pose a threat to long-term progress and revert back to the previous situation after initial achievements.

    Postoperative cognitive dysfunction (POCD) is a significant concern, with incidences reported up to 70% following cardiac surgery. Therefore, we aim to evaluate the incidence of POCD after elective coronary artery bypass graft (CABG) surgery at our single centre over a one-year period from August 2021 to July 2022. We included 34 patients in the study and conducted serial cognitive assessments up to three months post-surgery. Interestingly, our findings indicated an absence of POCD among patients who underwent elective CABG. Reasons contributing to this outcome are multifactorial, which may include the patients' younger age, higher educational levels, lack of pre-existing neurological disorders, meticulous intraoperative cerebral saturation monitoring, and the duration of aortic crossclamp and cardiopulmonary bypass time.

    Soft tissue mycosis usually presents with a triad of tumefaction, suppuration and ulceration. We report an unusual case of soft tissue mycosis in a 42-year-old male teacher who presented with painless swelling over the anterolateral aspect of the right shin for 4 years duration.

    Majority consumers around the world have become increasingly concern and aware about their health and food safety. Recent food crisis and foodborne illness incidents showed the needs to assure the authenticity and traceability of foods in the market especially meat and meat products. These scandals have led to negative effect and perception to consumers, food companies and both supply and demand chain. Hence, the food industry needs an excellent and reliable traceability system to ensure that consumers are persistently well protected from unconscious consumption of unsafe foods. Therefore, traceability systems can support the claims by making it verifiable. However, the awareness among Malaysian consumers is still lacking due to the inadequate exposure towards concept and the importance of traceability systems particularly in meat and meat products. The aim of this study is to determine factors influencing consumers’ preferences towards traceability systems of meat and meat products in Malaysia. Primary data were collected using structured questionnaire via face to face interview with 503 respondents in Klang Valley, Malaysia. Data collected were analyzed using descriptive and factor analysis. The findings of descriptive analysis showed that most of the consumers preferred using traceability systems and aware of its importance when buying meat and meat products. Meanwhile, the factor analysis results discovered six factors that influenced consumers’ preferences towards meat and meat products with traceability systems namely Halal certificate, transparency, quality, confidence, food safety and knowledge. Therefore, implementation of traceability systems could raise standards of food safety throughout the meat production supply chain. Furthermore, the society will become more confident and they can benefit from the quality of purchase and consumption. The findings from this study are also able to contribute to the body of knowledge to the producers or marketers towards food safety issues and foodborne illness that recently happened in Malaysia.

    Interests in the nutraceutical or food supplement are growing around Asia in response to health and beauty concerns. Nutraceutical supplements seem to dominate the Malaysian market as the awareness of health conscious arise. The consumptions of these nutraceutical products are to cover insufficient nutrient in their diet intakes. Edible bird nest (EBN) is made from saliva produced by the male’s swiftlet and been consumed as a health supplement due to their high nutritional value. Despite an ever-growing number of EBN products and options available in the market, there are still fewer purchasers among Malaysian consumers as compared to other countries. This study aims to determine factors that influence consumers’ intention in purchasing EBN products. The number of complete response from the survey were 1310 samples. The questionnaire includes nine variables that are determined from the theory of planned behavior and marketing mix. Exploratory factor analysis with promax rotation is conducted to remove items with factor loading less than 0.5 and confirmatory factor analysis is constructed to measure the fitness of the model so that it can be used in predictive models. The three popular classification algorithms from predictive models which are decision tree, logistic regression, and artificial neural network will be used to analyze the dataset and determined the best model building. These comparisons are highly evaluated based on the prediction performance and it does not conclude that one method will be superior to other predictive methods. Results showed logistic regression outer perform other classifiers to develop the purchase intention model. There are five variables identified which consist of age, gender, price, accessibility and halal authorize. This study provides an input of consumers’ concern and interest that can be used as a strategic tool and give special attention to those elements to promote the EBN products.

    Objective: To determine the prevalence of depressive symptomatology in early postnatal period and its relationship with obstetric risk factors. Material and Methodology: This is a cross sectional study involving 293 women from postnatal ward Hospital Universiti Sains Malaysia (HUSM), Malaysia. They were screened for depression at day 1 to day 5 postpartum and 4 to 6 weeks postpartum using the Edinburgh Postnatal Depression Scale (EPDS). At the first visit, the women completed questionnaires on socio-demography and obstetric risk factors and Brief Patient Health Questionnaire (BPHQ-9). At the second visit which is 4-6 weeks postpartum, they only completed EPDS. Results: Two hundred and ninety-three women successfully completed the study (100% response rates). The prevalence of PND at 4-6 weeks postpartum using the EPDS score of 12 or more was 27.3%. Women with the history of abortion (p < 0.05) and early postnatal depression (p < 0.05) was significantly associated with PND. Conclusion: Postnatal depression among women in Hospital USM was more common compared to previously reported series in Malaysia. Women with the history of abortion and early postnatal depression were significantly associated with PND. © 2009 Japan International Cultural Exchange Foundation.

    Introduction: Metabolic syndrome (MetS) is a condition that includes the presence of a cluster of risk factors specific for cardiovascular disease (CVD). The criteria used to aid the diagnosis of MetS includes abdominal obesity, elevated triglycerides, low high density lipoprotein (HDL) cholesterol, hypertension or use of antihypertensive medication, elevated fasting blood glucose and other risk factors. Objective: This study aimed to determine the prevalence of metabolic syndrome (MetS) among postmenopausal women and its associated factors in a tertiary center in Malaysia Methods: This is a cross-sectional study done among 411 postmenopausal women attending Gynaecology clinic and Family Medicine clinic in a tertiary center in Malaysia. Socio demographic data, reproductive profile, menopausal profile and medical history were obtained. Then waist circumference (WC), weight, height and blood pressure (BP) were also recorded. A fasting blood sample was obtained for serum glucose and lipid profile determinations. Metabolic syndrome was defined according to the criteria of International Diabetes Federation. Results: The mean age of participants was 57.2 + 6.9 years. The prevalence of metabolic syndrome was 36.7%. The risk of MetS increased with the presence of obstetrics history of hypertension (HPT) (odds ratio (OR) 2.64, 95% (CI) 1.25-5.62), previous usage of contraception (odds ratio (OR) 1.56, 95% (CI) 1.02-2.42), family history of HPT (odds ratio (OR) 1.71, 95% (CI) 1.13-2.59) and obesity (odds ratio (OR) 2.59, 95% (CI) 1.08-6.23). Conclusion: There was a high prevalence of the metabolic syndrome in postmenopausal women seeking gynaecologic and primary health care in the tertiary center Malaysia. The associated factors of MetS include previous obstetrics history of HPT, family history of HPT and obesity.

    Abstract
    Objectives: This study aimed to assess intra- and inter-examiner reliability of International Caries Detection
    and Assessment System (ICDAS) and modified epidemiology ICDAS (MOD) code by undergraduate dental
    students with different clinical experiences.
    Methods: A total of 150 dental undergraduate students with varying clinical experiences (0, 1 and 2 years of
    clinical experience) were recruited. Participants received training through a theoretical lecture on ICDAS criteria
    by an experienced National Benchmark Group (NBG) examiner and underwent e-learning program prior to
    ICDAS calibration. Visual examination on extracted permanent teeth (N= 45) with different location and stages
    of caries progression ranging from ICDAS scores 0 to 6, was performed using the ICDAS criteria. The
    assessments were repeated after one hour. The data were analysed to evaluate inter-examiner and
    intra-examiner reliability in the form of kappa scores using SPSS 23 Software.
    Results: Mean kappa values for intra- and inter-examiner reliability for ICDAS code, were between 0.41 to
    0.60, and 0.61 to 0.80 respectively. For MOD code, mean kappa values for intra- and inter-examiner reliability
    were between 0.61 to 0.80. Good intra-examiner agreement (>0.61) was observed in both ICDAS and MOD
    code for all groups.
    Conclusion: All students performed similar agreement, therefore, clinical experience within 2 years does not
    influence the performance of visual inspection in detecting caries using ICDAS. The results of the study shows
    that ICDAS and modified epidemiology ICDAS codes has good reproducibility and is feasible to be used as a
    tool in clinical practice as well as patient education.

    Two cocoa bean fermentation methods (spontaneous fermentation and the use of starter culture) for 7 days fermentation were compared in terms of safety and quality fermented beans. Candida sp. was used as a starter culture in this study. The safety of the fermented cocoa beans were measured by the growth colonies of pathogenic microorganisms namely Bacillus cereus, Escherichia coli, Salmonella sp., Staphylococcus aureus, and Pseudomonas sp., on Bacillus cereus agar, eosin-methylene blue (EMB) agar, xylose lysine deoxycholate (XLD) agar, Baird-Parker agar (BPA), and Pseudomonas agar, respectively. B. cereus, E. coli and Salmonella sp. were early present in both fermentations. Candida sp.-fermentation showed detection of B. cereus at 5.34 log10 CFU/g and absence after 24 hours of fermentation while in spontaneous-fermentation B. cereus was too few to count. Moreover, the log10 E. coli number in Candida sp.-fermentation and spontaneous-fermentation were reduced from 5.72 to 3.66 and from 7.15 to 4.46 on day 1 to day 3, respectively. There were no presences of pathogenic microorganisms on day 5 and day 7 for both fermentations. In term of quality, proximate analysis of spontaneous-fermentation resulted that the content of moisture, ash, fat, crude protein, crude fibre and carbohydrate was 56.47%, 2.32%, 3.17%, 7.02%, 28.14% and 2.88%, meanwhile for the Candida sp.-fermentation was 53.96%, 2.19%, 3.44%, 8.25%, 25.46% and 6.70%, respectively. This study showed that both fermentations are considered to be safe and there is no significant difference in proximate value in fermented cocoa beans from spontaneous-fermentation and Candida sp.-fermentation.

    Information about the quality of drinking water, together with analysis of knowledge, attitude and practice (KAP) analysis and health risk assessment (HRA) remain limited. The aims of this study were: (1) to ascertain the level of KAP regarding heavy metal contamination of drinking water in Pasir Mas; (2) to determine the concentration of heavy metals (Al, Cr, Cu, Fe, Ni, Pb, Zn and Cd) in drinking water in Pasir Mas; and (3) to estimate the health risks (non-carcinogenic and carcinogenic) caused by heavy metal exposure through drinking water using hazard quotient and lifetime cancer risk.

    According to recent researches, angle shear connectors are appropriate to transfer longitudinal shear forces across the steel-concrete interface. Angle steel profile has been used in different positions as L-shaped or C-shaped shear connectors. The application of angle shear connectors in tilted positions is of interest in this study. This study investigates the behaviour of tilted-shaped angle shear connectors under monotonic loading using experimental push out tests. Eight push-out specimens are tested to investigate the effects of different angle parameters on the ultimate load capacity of connectors. Two different tilted angles of 112.5 and 135 degrees between the angle leg and steel beam are considered. In addition, angle sizes and lengths are varied. Two different failure modes were observed consisting of concrete crushing-splitting and connector fracture. By increasing the size of connector, the maximum load increased for most cases. In general, the 135 degrees tilted angle shear connectors have a higher strength and stiffness than the 112.5 degrees type.
