Introduction: There is an emergent increase of ultra-processed food consumption in developing countries including Malaysia which carries a likelihood towards the increase of obesity. However, few studies have been done in relating between ultra-processed consumption and obesity, especially in Malaysia. Therefore, this study aims to determine the consumption of ultra-processed foods and its relationship with BMI and body fat percentage among university committee in Kuala Nerus, Terengganu. Methods: A cross-sectional study was performed among 167 individuals aged 18 to 59 years old in Kuala Nerus. Data were collected using a researcher-administered questionnaire which consisted of three different sections: socio-demographic profile, two days 24-hour dietary recall, and nutritional status assessment on BMI and body fat percentage. Physical Education (PE) teachers in schools have difficulties in attracting students to participate in activities that cause students to feel bored to play during the PE class due to the short time of teaching and the number of games to be learned in various ways. This study aims to evaluate the effectiveness of teaching methods based on the TGfU model towardssports science students through the Volleyball Camp 2016 (KBT2016). KBT2016 involves the Faculty of Sports & Recreation Science (FSR), UiTM in collaboration with SMK Puncak Alam. The respondents were selected based on sampling involving 31 Sports Science students taking the Acquisition of Movement Skills (SPS465) subject. Respondents were divided into four groups: two male groups and two female groups. KBT2016 received positive feedbacks from respondents. All respondents were able to learn the basics skills of volleyball with the right techniques in exciting situations.
Introduction: The metabolic syndrome comprises a collection of cardiovascular disease risks, which has been demonstrated to predict type 2 diabetes mellitus and cardiovascular disease. Metabolic syndrome is a crucial health concern in Malaysia, with a prevalence of about 42.5% in the general population based on the ‘Harmonized’ definition. The aim of this study was to ascertain the association between socioeconomic status among Malaysian government employees with metabolic syndrome, compared with those without metabolic syndrome. Furthermore, this study also aimed to ascertain the associated obesity indicators for metabolic syndrome among employees—explicitly body mass index, waist circumference, waist-to-hip ratio, body fat percentage, fat mass index, and waist-to-height ratio. Methods: This cross-sectional study was
undertaken at government agencies in Putrajaya, Malaysia, via multi-stage random sampling. A total of 675 government employees were randomly sampled from a list of 3,173 government employees working in five government agencies under five geographical areas. Data on socioeconomic status, anthropometric, biochemical, and clinical assessments were collected. Results: Employees who were males had higher metabolic syndrome prevalence compared to their counterparts (p=0.019). In addition, employees aged between 20 to younger than 30 years had lowest metabolic syndrome prevalence (p=0.002). Introduction: Identification of remains recovered at advanced stages of decomposition can be problematic due to the lack of physical evidence. Nonetheless, human dentition is least susceptible to decomposition and as such carry a significant value in personal identification of decomposed remains. Demirjian’s method of age estimation was developed specifically for children with developing dentition. In this article, a method on adapting the Demirjian’s method for Malay ethnic-specific age estimation using the third molar development is presented. Methods: Orthop- antomograms of Malay subjects aged 18 to 25 were obtained from UiTM Sungai Buloh. Total of 318 samples were taken, comprising of 123 and 195 images from male and female subjects. Development of right and left mandibular third molar was classified according to the eight stages of development as illustrated in Demirjian’s method. Data obtained were subjected to statistical analysis such as descriptive statistics and analysis of variance. The intra- and inter-gender variation between left and right mandibular third molar was evaluated using independent student t-test and analysis of variance, respectively. Results: Intra-gender comparison analysis revealed a significant difference in the female and male with a p-value of 0.000 and 0.003, respectively. Regression equation to estimate age based on third molar development were formulated according to dental age and maturity score. Conclusion: The Demirjian’s method was successfully adapted for age estimation of individuals of Malay ethnicity. These findings can help with victim identification in cases of poor skeletal framework recovery and highly decomposed remains.
Introduction: Illegal immigrants refer to the migration of people into a country in ways that violates its immigra-tion laws. A stateless person is a person who is not considered as a national by any state under the operation of its law. Interestingly, many who are stateless have never even crossed an international border. The term illegal is very loosely used in places like Sabah where it is interchanged with statelessness. As of 13 November 2018, it was estimated that there are about 12 million stateless people in the world. This review seeks to understand the current demographic situation in Sabah along with the public health repercussions of this demographic change and also to look into the success stories from around the world along with the recommendations from United Nation in solving this matter. Methods: The method used is reviewing of literature of previous studies conducted on statelessness and illegals. Local as well as international studies were reviewed. The data base used was ProQuest. Results: This review identified that one third of Sabah’s population consists of non-locals and in the past 27 years, about half a million illegal immigrants have been deported from Sabah. These numbers pose major public health repercussions from the economic, crime and health point of view. The way forward involves incorporating the United Nation Action plan with the local requirements and settings. Despite challenges, Sabah is trying its best to curb this issue and the Public health repercussions through various initiatives. We have also identified that more public health actions can be taken to reduce the negative effects. Conclusion: Dealing with the stateless and illegals is a delicate matter and there is no one way to solve it. Every country and every state are unique therefore the methods used must be tailor made. This is not something that can be solved within a short period of time and therefore persistence and perseverance is very much needed to tackle this global issue.
Introduction: One of the aims outlined in Malaysia’s Health Vision 2020 is to be a nation of healthy individuals, families, and communities through an equitable, affordable, efficient, environmentally adaptable, and consumer friendly healthcare system. Sabah faces tremendous challenges to provide the best care for patients. For example, Sabah's unique geographical location and landscape, such as steep hills and rivers, is one of the challenges that health staff faces. Objectives of this study aimed to examine the prevalence of geographical accessibility, types of healthcare services, and the types of health seeking behaviour in 2 northern rural areas of Sabah to assess the geographic accessibility and availability of healthcare services.
Materials and Methods: A community-based cross-sectional study was conducted in two rural areas in Sabah—Kudat and Pitas. Data collection was done by using questionnaire and face–to-face interviews
Results: It was found that 48% of the study population sought healthcare and they mainly chose healthcare services from hospitals and health clinics.
Conclusion: Half of the population in the areas studied used healthcare in the last year. The choice of using a public hospital or community health clinics was determined by distance from residence
Food insecurity is normally associated with lower socioeconomic status and improper feeding practices which may consequently affect growth and development of young children. This study aims to assess household food security status and its association with child feeding practices and children’s weight status among low income mothers in Terengganu. A cross-sectional study using convenience sampling was conducted at four public maternal and child health clinics (MCH) located in Terengganu. A total of 107 of low-income mothers between ages of 18 to 45 years with net household income less than RM 3000 who has at least one child aged two years and above were recruited. The instruments used were Household Food Insecurity Access Scale (HFIAS), Child Feeding Questionnaire (CFQ) and weight-for-age child growth chart for children’ weight status at aged one year. Descriptive statistic and non-parametric tests were employed (SPSS 20). Majority of the households were food secure (85.0%). Yet, it is also important to note that about 15% of the household had experienced some degree of food insecurity. There was no significant correlation between household food security and all child feeding practice domains. Most of the mothers had children with normal weight (mean Z-score =-0.72+0.99). There was statistically significant association between household food security status and weight of children at one year of age (p=0.01). In conclusion, majority of the households experienced food secure. Household food security were associated with children’s weight status at early age but not correlated with parent’s child feeding practice.
Introduction: Dengue fever is endemic in Malaysia. It is a major public health challenge that has caused significant morbidity and mortality. A dengue outbreak is contributed by entomological factors, epidemiological and environmental factors. Outbreak response is crucial to reduce cases and death. This study is a descriptive dengue outbreak report in an urban residential area in Kota Kinabalu, Sabah Malaysia. Methods: This is a report of a dengue outbreak that started in middle of November 2019 and ended early December 2019. A dengue outbreak in Malaysia is defined when more than one dengue cases is reported in the same locality within 14 days from the date of notification of the first case. Cases were analysed descriptively looking at epidemiology and vector control database. Results: This outbreak involved 6 cases. The source reduction activities by elimination of breeding sites were conducted 3 times covering an area of 200-meter radius. A total of 110 premises were checked with coverageof 86%. The number of containers inspected was923, with 454 outside the premises and 469 inside the premises with no positive results for dengue larvae. Insecticide space spraying via thermal fogging was conducted with added ultra-low volume (ULV) spraying done within the 400 meterradius with coverageof 100%. Conclusion: Effective preventive measures have to be paired with responsible communities, as both play very vital roles in the control of dengue.
Colorectal cancer (CRC) is the second most common cancer in Malaysia. In general, patients aged > 65 years comprise the majority of the population with CRC. However, recent reports indicate its increasing incidence in younger populations. We describe 4 cases of early onset colorectal cancer in Kuantan. Materials and method: This is a descriptive study of a small series of patients. Data between 2018 and 2019 were obtained from medical charts. Results: We diagnosed four patients under the age of 50 with colon cancer (75% male), the median age was 36 (23-47), with no comorbidities. None of the patient had family history of colorectal cancer. The majority of our patients presented with complication of tumour (2 perforation, 1 obstruction) required emergency surgeries. Only one patient presented with with rectal bleeding and was found to have tumor of the rectosigmoid junction. Two patients had tumor at descending colon and one at the splenic flexure. From this case series, we found there were one patient for every stage of colorectal cancer. Carcinoembryonic (CEA) levels were elevated at baseline in only two patients. Conclusion: Incidence of colon cancer rises among young patients in Kuantan. Further studies are needed to clarify the clinical and biological characteristics of colon cancer, improve its treatment strategies, and promote better outcomes in young patients.
An exploratory study to ascertain the public’s awareness of community pharmacy
and pharmacists in a selected subset of the Malaysian population was undertaken,
utilising an interviewer-administered structured questionnaire approach. A total
score was computed for each respondent, ranging from a possible minimum of 0 and
a maximum of 24. The scores achieved were arbitrarily categorised into poor (19) levels of general knowledge
regarding community pharmacy and pharmacists. The scores achieved ranged from
3 to 21, with an average “fair” score of 13.7. The results showed that 93.6% of the
respondents (n = 561) interviewed had heard of the term “pharmacist” before.
Interestingly, 17.5% of the respondents were of the opinion that pharmacists
worked on farms. A significant 77.4% perceived that a pharmacist served in a
doctor’s clinic. It was noted that 84.1% of those surveyed would go to doctors for
advice on medicine, while only 49.4% would seek a pharmacist. A majority (76.7%)
of the respondents interviewed chose to go to a doctor’s clinic for a screening test.
The study amplifies the need for a more aggressive projection of the pharmacist’s
image in the community in order to be recognized and accepted by the public as an
integral partner in the health care profession.
Information regarding water fluoridation (WF) in Malaysia has been substantially documented, but is scattered in various government publications and may be lost to the stakeholders. This paper is a review of water fluoridation in Malaysia and its effect on oral health: a history of WF in Malaysia, the current policy, the evidence of its effectiveness, the challenges and the future directions. A search for relevant physical and electronic documents of WF in Malaysia resulted in the identification of 70 documents for review. WF was gazetted as national policy with an optimal fluoride level of 0.7 parts-per-million (ppm) in 1972, with a reduction of the level to 0.5 ppm in 2005. Evidence showed that WF effectively reduced population dental caries while fluorosis was not a prevalent public health concern. Strong collaboration between stakeholders and the extensive network of piped water supplies resulted in 80% of the population receiving WF in 2013. However, the coverage was reduced to 74.1% in 2018, largely due to the cessation of WF in Pahang. The key challenges in WF included a lack of funding, weak legislation, use of reverse osmosis water filtration system, difficulty to maintaining an optimal level of fluoride in the water, and lack of local data on the impact of WF cessation on oral health, and its cost-effectiveness. This review will provide dental health professionals with scientific evidence on WF and oral health in Malaysia and assist them in answering relevant questions about WF raised by the public.
Introduction: Chronic obstructive pulmonary disease (COPD) patients usually have dietary problems leading to malnutrition issues. Therefore, this study aimed to determine macro and micronutrient intakes and meal-related sit- uations among outpatients COPD elderly and its adequacy according to their requirements. Methods: 140 patients were included in this cross-sectional study at Respiratory Clinics of Hospital Serdang and Institut Perubatan Respira- tori. Socio-demographic and health status data were collected by interviewing patients and reviewing their medical records. A three-day diet history (two-day on weekdays, one day on weekend) was analysed using Nutritionist Pro and compared against their requirements. Meal-related situation was assessed using three open-ended questions that related to food shopping, cooking and eating and analysed using content analysis. Results: Patients were 70±7 years old, 97% male, 59% Malay, 48% had primary education, 75% married, 72% ex-smokers and 54% presented with comorbidities. Majority of them had no episode of exacerbation for the past one year and in moderate stage of the COPD severity. Mean energy intake was 916±221 kcal/day with 98% of them have under-reported their intake. Almost all patients had inadequate macro and micronutrient intake; energy (97%), protein (97%), carbohydrate (86%), fat (99%), vitamin A (95%), C (86%), D (99%) and E (99%). The meal-related situation reported patients had difficulties with food shopping and preparation and problems during mealtime. Conclusion: COPD elderly reported inadequate intake of macro and micronutrients and had problems during mealtime. This indicates the need for Ma- laysian nutrition guidelines specific to COPD patients and nutrition intervention in the primary setting.
Introduction: This study aimed to compare the nutritional characteristics and clinical outcomes among critically ill patients with diabetes (DM) and without diabetes (WDM). Methods: Mechanically ventilated, critically ill patients who were admitted into the intensive care unit (ICU) within 48 hours and remained in ICU 72 hours were prospec- tively recruited and followed for up to 12 days. They were stratified to DM or WDM, depending on their diabetes status at ICU admission and comparison were made for nutritional characteristics and clinical outcomes including 60-day mortality. Results: A total of 154 patients were included with 73 (47.4%) DM patients. In comparison to WDM, patients with DM were older, more severely ill, had higher nutritional risk and body mass index, presented with a higher blood glucose level, and required more insulin. DM was fed relatively earlier but had lower energy adequacy. They experienced more frequent EN interruption. Both groups had comparable ICU and hospital stay, ventilation support duration and mortality. In multivariable logistic regression, no association was found between diabetes status and for ICU and hospital mortality. However, There was a trend towards an increase in 60-day mor- tality in DM patients (Odds Ratio: 2.220, 95% Confidence Interval: 0.764-6.452; p=0.143). Conclusion: Critically ill patients with DM had higher nutritional risks, were fed relatively earlier, but with frequent EN interruption leading to lower energy adequacy than patients WDM. Diabetes status does not affect clinical outcomes.
Assessment of instructors by students is needed for assessing the teaching quality of a lecturer towards achieving the objectives of a course. This paper aims to examine the techniques used in sentiment analysis for assessing the effectiveness of a lecturer’s or a teacher’s teaching style in the learning process at a university or school. In addition, the effectiveness of sentiment analysis techniques in assisting the teaching evaluation process is also discussed. The challenges for assessing the quality of teaching of National Defence University of Malaysia (UPNM) lecturers are also discussed in this paper. The sentiment analysis technology is capable of analysing views or opinions on a matter, regardless of whether they are positive or negative. Data from the sentiment analysis can be used by specific parties or anyone else to rectify any weakness or to improve any aspect that the user commented on. The purpose of this study is not to find the weakness of the lecturer, but rather the results of this assessment process can be useful to the management for rectifying weaknesses and for improving the teaching process.
Introduction: It is widely recognised that health-related quality of life (HRQOL) is
impaired in chronic obstructive pulmonary disease (COPD) patients. Most previous
studies were conducted among the younger adult population and there is limited
information on the elderly population. This study aimed to determine the COPD
assessment test (CAT) score in the elderly with COPD. Methods: This was a crosssectional study involving 140 elderly patients who had been diagnosed with COPD
at two selected government institutions. Data on socio-demographic and health
status were collected by interviewing patients and reviewing their medical records.
The HRQOL was measured using CAT. The independence sample t-test and one-way
ANOVA were conducted to compare the CAT scores with socio-demographic and
health status. Results: The socio-demographic and health characteristics of the
patients were as follows: majority (54%) were aged 60-70 years, male (97%), Malay
(59%), married (75%), ex-smokers (72%), had attained primary education (48%),
had co-morbidities (54%), no history of hospitalisation or visits to the emergency
department due to COPD (57%), and were in moderate stage of airflow obstruction
(53%). The mean of CAT score was 21.87±6.85 and the majority of the patients were
classified as having worst symptoms of COPD (93%) as they had high CAT score
which was ≥ 10. Conclusion: Majority of the patients in this study had high scores
of CAT, which indicated poor HRQOL. Ex-smokers had higher scores compared
to non-smokers. More attention need to be given to these subgroups in order to
increase their quality of life.
Introduction: Validation of a culturally specific FFQ is important in assessing habitual dietary intake of ethnic population groups. This study aimed to assess the validity and reproducibility of a FFQ developed specifically for determining the dietary intake of Malaysian Punjabis.
Methods: Subjects were approached through voluntary participation for the development and validation phase in the Klang Valley. A list of foods consumed by participants (n=100) was documented through a 3-day dietary recall. The validation process was conducted by verifying the developed FFQ against another group of Punjabi adult volunteers (n=101) who kept a 2-day dietary record. Macronutrients, dietary fibre, fatty acids, cholesterol, three types of minerals (calcium, sodium, iron) and four vitamins (B12, folate, C, A) were included in the analysis. Reproducibility was shown with intraclass correlation (ICC) values between FFQ1 vs FFQ2 that were administered 6 months apart among 32 participants.
Results: In the validation study, the FFQ1 was found to have over-estimated almost all nutrients compared to those in the dietary records. The Spearman correlation coefficients for energy, carbohydrate, protein and fat intake based on the FFQ1 and 2-day dietary records were 0.54, 0.38, 0.47, and 0.31, respectively. The classification into the same and adjacent quartiles was between 61- 84% for the nutrients consumed. Bland Altman plots showed relatively good agreement (between ±2 standard deviation) for both the dietary methods used. Reproducibility analysis of ICC (FFQ1 vs FFQ2) was between 0.46-0.76 for macronutrients and 0.20-0.92 for micronutrients.
Conclusion: The developed FFQ could be used as a valid tool for assessing dietary intake of Malaysian Punjabis, as it showed a moderate agreement with dietary record for intake of energy and macronutrients.
Introduction: This study aimed to investigate the effect of saffron extract and crocin on blood biomarkers associated with obesity using the rat model.
Methods: Obesity was induced by feeding a high-fat diet to 42 male Sprague-Dawley rats for 12 weeks, after which they were equally distributed into seven groups. Three groups served as controls namely, normal diet (ND), high-fat diet (HFD), and high-fat diet plus orlistat (HFD + ORL), while the remaining four treatment groups consisted of HFD added low or high dose (40 and 80 mg/kg/day) of either saffron extract or crocin in the food. At the end of 8 weeks, blood samples were collected by cardiac puncture for biochemical analysis.
Results: Obese rats treated with a high dose of saffron extract and crocin showed significantly lower plasma glucose levels (5.26 and 5.67 mmol/L respectively) than the HFD rats (6.92 mmol/L). Saffron extract and crocin at a high dose showed significantly lower levels of plasma insulin (3.97 and 3.88 ng/mL respctively) compared to HFD control (5.41 ng/mL). Adiponectin levels significantly increased in obese rats fed saffron extract and crocin at high doses (7.44 and 7.92 µg/mL respectively) compared to HFD control (5.34 µg/mL). Ghrelin level significantly increased from 419.10 to 284.10 pg/mL,while leptin level significantly decreased from 8.08 to 5.68 ng/mL for the high dose crocin groups compared to HFD control. No significant differences in plasma serotonin levels were found among the groups.
Conclusion: Saffron extract and crocin show potential in reducing blood biomarkers associated with obesity as well as anti-inflammatory and regulatory potential of adipocytokines in an animal model.
Introduction: Chinese Herbal Medicine (CHM) is becoming increasingly popular among cancer patients worldwide. While health-related quality of life (HRQoL) in relation to cancer outcomes has attracted global attention, there are few studies on CHM use and HRQoL among cancer patients in Malaysia. This study attempted to determine the association between use of CHM, including the types, reasons, and beliefs, and HRQoL among Malaysian cancer patients.
Methods: A cross-sectional study was conducted among 120 cancer patients (60 male and 60 female) recruited from government oncology clinics in Johor state. A purposive non-probability sampling was applied to recruit respondents.
Results: Use of CHM was reported by about half of the patients (49.2%). Common types of CHM used included Chinese herbal extracts (27.5%), Sabah snake grass (12.2%), and ginger (11.5%). The median score for overall belief in CHM significantly differed between
CHM users (71.7%) and non-users (65.0%) (p<0.001). The HRQoL was generally reported as ‘good’ with high scores for overall functioning scales and low scores for symptom scales. However, no significant difference in HRQoL between CHM users and non-users was noted. ‘Social Functioning’ scores between both groups showed a significant difference (p < 0.001) with respect to sex, with females scoring higher than males. No significant association was noted between CHM use and socio-demographic characteristics except for sex.
Conclusion: There is a high prevalence of self-prescription of CHM among the cancer patients studied. Nonetheless use of CHM did not show any significant difference in terms of quality of life among CHM users.

Orlistat is a widely used drug in treating obesity as it promotes weight reduction. The aim of this study was to determine the protective effects of orlistat (10 mg/kg/day) on cardiovascular parameters and oxidative stress biomarkers in high-fat diet (HFD)-induced obese rats. Methods:Twenty-four male rats Sprague Dawley rats were divided into three groups and fed with normal diet (N), HFD and HFD with orlistat (HFD+O). Orlistat was administered daily by oral gavage and after six weeks, all rats were sacrificed. Results:Administration of orlistat along with HFD (HFD+O) has brought significant decreases in Lee obesity index and LDL level compared to HFD group. Activities of cardiac superoxide dismutase (SOD), glutathione peroxidase (GPx) and catalase (CAT) were significantly higher, whereas level of oxidised LDL was significantly lower in HFD+O group compared to HFD group. HFD group had significantly higher necrotic patch area in myocardium while minimal histological changes were seen in HFD+O group. Conclusion:This study may suggest that administration of orlistat at 10 mg/kg/day for 6 weeks may have protective effects against the changes on Lee obesity index, lipid profiles, cardiac oxidative stress biomarkers and histology of myocardium in HFD-induced obese rats possibly through its hypolipidaemic and antioxidant actions.
Environmental factors such as temperature, lighting and noise have very significant impact to workers’ health, safety, comfort, performance and productivity. In an ergonomically design industrial work environment, these factors need to be control at their optimum levels. The main objective of this study is to find the effect of temperature, illuminance and sound pressure level on workers’ productivity in automotive industry. To perform this study a workstation in an automotive component manufacturing was selected as the location of the study. Results of data analysis showed there were relationships between temperature, illuminance and noise on workers’ productivity. Later, the authors’ developed multiple linear equation models to represent the relationships between temperature, illuminance and noise on the workers’ productivity. These multiple linear equation models could be used to predict the production rate for the workstation by referring to the value of temperature, illuminance and noise level.
