    Acupuncture is a form of complementary medicine that has been practiced in China for thousands of years. Adverse effect of acupuncture is rarely reported in local literature. This is a case of a patient who developed bilateral pneumothorax following an acupuncture session. A 63-year-old lady with no significant medical illness presented with sudden onset of shortness of breath half an hour following acupuncture and massage session by traditional medicine practitioner. On examination, she was tachypnoiec and there was reduced air entry bilaterally on lung auscultation. Urgent portable chest X-ray was done and it showed bilateral pneumothorax. Bilateral chest tubes were inserted. Patient was discharged well following five days of hospitalization. As acupuncture is gaining popularity among Malaysian population, medical practitioners need to increase their awareness and knowledge regarding the adverse effect of such alternative practice.

    Nephelium lappaceum is a popular tropical fruit belonging to the Sapindaceae family. The plant originated in Malaysia and Indonesia and is commonly called rambutan. Because of its refreshing flavor and exotic appearance, rambutan is widely accepted in the World. Due to its significant medicinal properties, the fruit has also been employed in traditional medicine for centuries. The chloroplast genome of rambutan was sequenced, assembled, and annotated in the present study. The chloroplast genome length was 161,356 bp and contained 132 genes, including 87 protein-coding genes, 37 transfer RNA (tRNA) genes, and 8 ribosomal RNA (rRNA) genes. It possessed the typical quadripartite circle structure with a large single-copy region (86,009 bp), a small single-copy region (18,153 bp), and two inverted repeat regions (28,597 bp). A total of 35 SSR markers were found in the chloroplast genome of Nephelium lappaceum, of which 33 were monomer, 1 was dimer and 1 was tetramer. Phylogenetic analysis based on the complete chloroplast genome sequences of 21 plant species showed that rambutan was closely related to Pometia tomentosa. These results provide a foundation for further phylogenetic and evolutionary studies of the Sapindaceae family.

    Recent studies suggested that combined treatment approaches can be used to improve anticancer potency and circumvent the limitations of high-dose tocotrienols administration. Acalypha wilkesiana is a medicinal plant that has been used as an adjunct treatment for cancers in traditional medicine. Herein, the effects of single and combined treatments of β-, γ- and δ-tocotrienols and ethyl acetate extract (9EA) of Acalypha wilkesiana on lung (A549) and brain (U87MG) cancer cells were investigated. γ- and δ-tocotrienols exhibited higher potent antiproliferative effects against A549 (12.1 μg/ml and 13.6 μg/ml) and U87MG cells (3.3 μg/ml and 5.2 μg/ml) compared to β-tocotrienols (9.4 μg/ml and 92.4 μg/ml), respectively. Whereas, 9EA induced potent antiproliferative effects against U87MG cells only (2.0 μg/ml). Combined treatments of tocotrienols and 9EA induced a synergistic growth inhibition with up to 8.4-fold reduction in potent doses of β-, γ- and δ-tocotrienols on A549 cells. Apoptotic features were also evidenced on A549 cells receiving single and combined treatments. The synergism may greatly improve the therapeutic outcome for lung cancer.

    Chinese medicine is one of the most famous traditional medicines in the world with a glorious and long written history of at least 2000 years. Recently, acupuncture and the use of other herbal medicine are being gradually accepted globally. In 2011,the International Medical University (IMU) started the Chinese Medicine programme which is the first of its kind in a western medicine university in Malaysia.The author introduced the background of Chinese medicine and the curriculum of the Chinese Medicine programme established in IMU, analyzed the situation regarding the quality of lectures given by internal and
    external lecturers in this programme and also discussed on ways to integrate western and traditional medicine in IMU or in Malaysia. The launching of Chinese medicine in IMU is a great step in the development of IMU and also an important step in the development of medical education in Malaysia or even in South-east Asia.

    Henna plant (Lawsonia inermis) is an Indian medicinal plant used in traditional medicine for the treatment of various diseases, besides its popularity as a natural dye to colour hand and hair. Research in the recent past has accumulated enormous evidence revealing henna plant to be an excellent source of antioxidants such as total phenolics. In this study, the extraction of total phenolics from henna stems was evaluated using the Folin-Ciocalteu assay. A set of single factor experiments was carried out for identifying the optimum condition of each independent variable affecting total phenolic content (TPC) extraction efficiency of henna stems, namely the solvent type, solvent concentration (v/v, %), extraction time (min) and extraction temperature (oC). Generally, high extraction yield was obtained using aqueous acetone (about 40%) as solvent and the extraction yield could further be increased using a prolonged time of 270 min and a higher incubation temperature of 55°C. Under these optimized conditions, the experimental maximum yield of TPC of 5554.15 ± 73.04 mg GAE/100 g DW was obtained.

    This paper attempts to examine the real values of Malaysian herbal products in the aspects of quality, safety and efficacy as curing agents. In so doing it will also determine the driving force behind the intense public interest for herbal medicine as alternative or complementary to conventional medicine. Most herbal products in the Malaysian market are not sufficiently provided with information on their ingredients, indications, dosage, pharmacology, contraindications and possible side-effects. Most published information on the products on evidence of safety and efficacy is not supported with scientific evidence. The present practice of traditional medicine still depend heavily on information obtained through ethnopharmacological experiences. However, the herbal product market is experiencing a tremendous growth and there is an increased trend of incorporating herbal therapy into modern medical practice by many mainstream health professionals. Unfortunately, the popularity of herbal products is more associated with consumer attitudes and the ability of the herbalists to influence rather than their true quality as medicinal agents. Many people are exploited due to ignorance on the real value of herbals as therapeutic agents. It is especially disappointing when popular media, promotional literature and talk shows by individuals promoting quackery are given much publicity. Even worse, many individuals trained in the health sciences are promoting quackery. This has often left the consumers with the perception that the authority condones the improper use of herbal products. Thus, it is important for pharmacists and physicians to be trained in traditional herbal medicine so that they can educate the public on the benefits, quality, safety and proper use of herbal products.

    Dichloromethane and methanolic extracts of each plant were tested for their antiplasmodial activity on chloroquineresistant strain of Plasmodium falciparum (FCB strain), based on lactate dehydrogenase activity. Cytotoxicity was assessed with the MTT test on MRC-5 human diploid embryonic lung cells. Most extracts of ten selected plants used in Malay traditional medicine in Malaysia had activity in vitro. This supports continued investigations of traditional medicine in the search for new antimalarial agent. The compounds responsible for the observed antiplasmodial effects are under investigation.

    Natural products are often perceived as safe due to the absence of artificial or chemical materials in its content. However, many are unaware that some of these compounds, albeit natural in existence, can cause harm. Cinnamon and lemon are two commonly used home remedies for acne. Both these naturally existing ingredients are capable of producing dermatitis upon contact with the human skin. The aim of this article is to create awareness among physicians that natural remedies are not free from harm hence, should look out for any possible untoward reactions that these products may cause. Physicians need to explore the possible use of homemade remedies to treat common or minor ailments during history taking as this information may not be given voluntarily. Early identification of the offending agent, adequate management and future avoidance could help prevent further episodes of contact dermatitis and its complications.

    ETHNOPHARMACOLOGICAL RELEVANCE: Yahom is a traditional Thai medicine used to treat syncope and abdominal discomfort.

    AIM OF THE STUDY: This study aimed to systematically review all available evidence which purports to support these claims.

    MATERIAL AND METHODS: The systematic review accorded with the Cochrane Collaboration framework and PRISMA reporting. Databases including MEDLINE, Excerpta Medica Database (EMBASE), Cochrane library database, and Google Scholar were searched by keywords, Yahom and Ya-hom. Pharmacological and toxicity data from non-animal and animal studies were included.

    RESULTS: Twenty-four articles: 2 on in vitro cell lines or bacteria, 3 in vitro cell-free, 5 in vitro animal, 13 in vivo and 1 human mainly reported (A) Cardiovascular effects (i) transient hypotension (0.2-0.8g/kg, intravenous injection (i.v.)), increased cerebral blood flow (2g/kg, single oral) and vascular dilatation/relaxation (ii) elevated blood pressure (BP) (0.2-0.8g/kg, i.v. or 2-4g/kg oral) and vasocontraction. Single Yahom doses (3g) given to healthy volunteers had no effect on cutaneous blood flow, ECG or systolic BP although marginally increased diastolic BP was claimed. (B) Yahom (2-4g/kg) completely inhibited gastric acid secretion evoked by gastric secretagogues. (C) Toxicity: Chronic oral doses of selected Yahoms to rodents (0.001-1g/kg) supports its status as generally regarded as safe.

    CONCLUSIONS: Most studies supported declared objectives relating to perceived Yahom actions, but lacked background demonstrating clinical efficacy, and mechanistic data that would validate conclusions. Our study suggests that research into traditional medicinal herbs needs underpinning by appropriate clinical interventions and pharmacovigilance, thereby optimising efficacy and minimizing toxicity by combining traditional wisdom and modern testing.

    parts of the world for centuries. It was used in the world as folk medicine to cure different kinds of diseases. This plant has been considered as one of the main sources of nutrition and healthcare for humans as well as animals. It has been perceived as Kalonji; it is a southwest Asian plan t that flowers annually. The seeds and oil of this plant have been used in food; in addition, it has a long history in the making of medicines. In addition to its being a model plant for better realization of gene and chromosome relationship, the plant species is also significant cytogenetically. Plant based system has not been absorbed fully for human health care despite the remarkable advancements in the field of pharmacology. Cumin, as one of the medicinal plants gifted to humans by nature, has a number of potential uses. It has been proved to be a
    useful herbal medicines that can be used for human health and therefore has been extensively studied and investigated to further discover the advantages of this plant.

    Ganoderma lucidum has been recognized as a precious fungus in both Chinese and Japanese traditional medicine for
    centuries. It contains many bioactive ingredients such as triterpenoids and polysaccharides. The present study used
    supercritical carbon dioxide (SC-CO2
    ) fractionation to fractionate Ganoderma lucidum extract into four fractions (R,
    F1, F2, & F3) and evaluate the correlation between the content of functional components and their antioxidant ability.
    Relatively high concentrations of the three types of bioactive constituents were simultaneously partitioned into different
    fractionation collecting vessels. The free radical scavenging ability was greatest in F1. The IC50 of DPPH scavenging ability
    was 0.90 mg/mL and that of ABTS radicals scavenging activity was 0.45 mg/mL. The correlation analysis of antioxidant
    ability with total triterpenoids and total polyphenols showed a positive relationship. In conclusion, this study showed
    that fractionation of Ganoderma lucidum extract using SC-CO2 fractionation technology was able to effectively partition
    its bioactive components including triterpenoids, polysaccharides and phenolic compounds and also to increase the
    antioxidant activities of the fractions.

    Ficus carica, a native plant to the Middle East and Western Asia, is of high value in folk medicine. The therapeutic potential
    of Ficus carica has led to the extensive studies in recent years, focusing on evaluating and validating its pharmacological
    effect. The present systematic review summarizes the effectiveness of Ficus carica on promoting bone health focusing on
    osteoporosis and rheumatoid arthritis via mineral contents and RANKL pathway. The search was done with Medline via
    Ebscohost, Scopus and Google Scholar databases to obtain relevant articles published between 1946 and December
    2016. The main inclusion criteria were research articles published in English that reported effect of Ficus carica on
    bone health. The literature search returned 716 potentially relevant articles, whereby 5 met the inclusion criteria. This
    systematic review concludes Ficus carica plays an important role in the promotion of bone health and can be a potential
    pharmaceutical product in the future.

    There has been accumulating interest in the application of medicinal plants as alternative medicine to treat various diseases and/or to develop modern medicines. Vitex negundo is one of such medicinal plants that has been of interest to many researchers and has been of use in traditional medicine. V. negundo is found in Sri Lanka, Madagascar, Malaysia, India, China, The Philippines and East Africa. Therapeutic properties of V. negundo have previously been reviewed. Different parts, preparations and bioactive components of V. negundo possess potential protective and therapeutic effects against cardiovascular disease and related conditions as demonstrated in previous studies. We review the present state of scientific knowledge on the potential use of V. negundo and some of its bioactive components in protecting against cardiovascular diseases and related pathologies. Previous studies in animal and non-animal experimental models, although limited in number and vary in design, seem to support the cardioprotective effect of V. negundo and some of its active components. However, there is need for further preclinical and clinical studies to validate the use of V. negundo and its active constituents in protection and treatment of cardiovascular diseases. Additionally, since only a few V. negundo compounds have been evaluated, specific cardioprotective effects or mechanisms and possible side effects of other V. negundo compounds need to be extensively evaluated.

    BACKGROUND: Zingiber zerumbet (L.) Roscoe ex Sm. (family, Zingiberaceae) is a potent medicinal herb widely known as shampoo ginger and its rhizome is used in numerous ethnomedicinal applications including antipyretic, anti-inflammatory, antibacterial, anti-diarrheal, antidiabetics, carminative, and diuretic. The aim of this review was to bring together all the scientific updates on the phytochemistry and pharmacological activities of this herb, including their toxicological studies, and critically analyzed the outcomes to provide directions for future research on the herb as potential source of bioactive metabolites for pharmaceutical and nutraceutical applications.

    METHODS: A structured electronic search on worldwide accepted scientific databases (Web of Science, PubMed, Google Scholar, Science Direct, SciFinder, Wiley Online Library) was carried out to compile the relevant information. Some information was obtained from books and database on medicinal plants used in various countries.

    RESULTS: About 60 metabolites, mainly polyphenols, and terpenoids have been isolated and identified. However, most of the reported pharmacological studies were based on crude extracts, and only a few of those isolated metabolites, particularly zerumbone have been investigated for biological and pharmacological activities. Many of the mechanistic studies to understand the pharmacological effects of the plant are limited by many considerations with regard to design, experimentation and interpretation.

    CONCLUSION: The bioactive metabolites should be further investigated on their safety and more elaborate preclinical studies before clinical trials can be undertaken.
