    Background: Circumcision though not mentioned in the Quran is believed to be a compulsory practice among the Muslims. In Malaysia, although there are several methods of circumcision available and traditional circumcision is still popular.
    Methods: A cross-sectional survey was carried out in a small fishing village of Kedah to study the methods of circumcision available to the villagers. This was followed with an in-depth interview conducted with ‘Tok Mudim’, a practitioner of traditional method of circumcision.
    Results: Forty three of the eligible 71 subjects participated in the study giving the response rate as 60.5%. The most common age for circumcision was 9 years old. Despite private clinics being the most common place of circumcisions, there was an increasing number of boys going to the ‘Tok Mudim’ for circumcision. A Mass Circumcision Ceremony is traditionally practiced. The ‘Tok Mudim’ described the procedure in detail and was of the opinion that the reason traditional method is still popular was because of the fear of injections and impotency among the parents. Most common complication faced by the ‘Tok Mudim’ was bleeding and infection.
    Discussion and Conclusion: Till the community shifts entirely to using modern medicine, there is a need to integrate traditional practitioners into the system. Training the ‘Tok Mudim’ to use modern instruments and aseptic techniques should be considered.

    Polymeric nanoparticles gain a widespread interest in food and pharmaceutical industries as delivery systems that encapsulate, protect, and release lipophilic compounds such as omega-3 fatty acids, fat-soluble vitamins, carotenoids, carvedilol, cyclosporine, and ketoprofen. In this study, medium-chain-length poly-3-hydroxyalkanoate (mcl-PHA)-incorporated nanoparticle was developed via facile organic solvent-free nanoemulsion templating technique. The water content (W/surfactant-to-oil (S/O)), S/O, and Cremophor EL-to-Span 80 (Cremo/Sp80) ratios were first optimized using response surface methodology (RSM) to obtain nanoemulsion template prior to incorporation of mcl-PHA. Their effects on nanoemulsion formation were investigated. The mcl-PHA-incorporated nanoparticle system showed a good preservation capability of β-carotene and extended storage stability.

    Introduction: Managing adolescent pregnancy in the primary care setting is complex, as it requires doctors to navigate through a combination of medical, social, financial and legal needs. Objective: This study explores the perspectives of private general practitioners on their roles and challenges in managing adolescent pregnancy in Malaysia.

    Methods: Nineteen private general practitioners in Selangor and Kuala Lumpur participated in in-depth interviews in 2015. A topic guide was used for interview navigation. Participants were asked to discuss their experiences and approaches in managing pregnant adolescents. We used purposive sampling to recruit consenting private general practitioners who had experience in managing adolescent pregnancy. The verbatim transcripts of the audio-recorded interviews were analyzed using thematic analysis. Data reached saturation at the nineteenth in-depth interview. Results: Two themes emerged. Under the theme 'inadvertent advocator,' participants described their tasks with regards to building trust, calming angry parents and delivering comprehensive counseling and care related to the sexual and reproductive health of adolescents, including requests for abortions. Theme two, 'challenges of private general practitioners,' refers mainly to personal and religious conflicts arising from a request for an abortion and deficiencies in support and multidisciplinary integration within their practice settings.

    Conclusion: General practitioners practicing in the private sector identify themselves as active players in supporting pregnant adolescents but face many challenges arising from the personal, religious, professional and community levels. Addressing these challenges is important for optimal care delivery to pregnant adolescents in this community.

    Nasopharyngeal carcinoma (NPC) is one of the most difficult diseases to diagnose at an early stage. The clinical presentation of 122 patients with confirmed NPC is described and the findings analysed. The common modes of presentation and cases where detailed nasopharyngeal examination need to be performed are highlighted. We emphasize the importance of health education and training for primary care physicians for early detection of these cases.

    Milk-alkali syndrome (MAS) is one of the causes of hypercalcaemia. We report a case of a 75-year-old lady with a history of thyroidectomy, presented with an altered mental state and had an extremely high calcium concentration of 4.96mmol/L. The hypercalcemia was attributed to the ingestion of large doses of calcium supplements, including calcium carbonate and calcium lactate, leading to MAS. She was managed with intravenous fluids, diuretics and withdrawal of calcium supplements. The patient responded well to treatment and regained consciousness. Details of the case including clinical presentations, electrocardiogram (ECG) findings and treatment plan, are discussed in this article.

    Developing new treatments for emerging infectious diseases in infectious and noninfectious diseases has attracted a particular attention. The emergence of viral diseases is expected to accelerate; these data indicate the need for a proactive approach to develop widely active family specific and cross family therapies for future disease outbreaks. Viral disease such as pneumonia, severe acute respiratory syndrome type 2, HIV infection, and Hepatitis-C virus can cause directly and indirectly cardiovascular disease (CVD). Emphasis should be placed not only on the development of broad-spectrum molecules and antibodies but also on host factor therapy, including the reutilization of previously approved or developing drugs. Another new class of therapeutics with great antiviral therapeutic potential is molecular communication networks using deep learning autoencoder (DL-AEs). The use of DL-AEs for diagnosis and prognosis prediction of infectious and noninfectious diseases has attracted a particular attention. MCN is map to molecular signaling and communication that are found inside and outside the human body where the goal is to develop a new black box mechanism that can serve the future robust healthcare industry (HCI). MCN has the ability to characterize the signaling process between cells and infectious disease locations at various levels of the human body called point-to-point MCN through DL-AE and provide targeted drug delivery (TDD) environment. Through MCN, and DL-AE healthcare provider can remotely measure biological signals and control certain processes in the required organism for the maintenance of the patient's health state. We use biomicrodevices to promote the real-time monitoring of human health and storage of the gathered data in the cloud. In this paper, we use the DL-based AE approach to design and implement a new drug source and target for the MCN under white Gaussian noise. Simulation results show that transceiver executions for a given medium model that reduces the bit error rate which can be learned. Then, next development of molecular diagnosis such as heart sounds is classified. Furthermore, biohealth interface for the inside and outside human body mechanism is presented, comparative perspective with up-to-date current situation about MCN.

    Local ecological knowledge (LEK) increases understanding of certain species and the threats they face, especially little-studied taxa for which data on distribution and conservation are often lacking. We conducted 111 semi-structured interviews in Sarawak, Malaysia, to collect local knowledge about the behavior and distribution of the Philippine slow loris (Nycticebus menagensis) from two ethnic groups, the Iban and the Penan. Our study revealed that male Penan respondents, generally hunters, who frequently go into the forest were better at identifying animals from pictures. Overall, the Penan have a more detailed knowledge of slow loris behaviors, habitat, and distribution than the Iban. The two ethnic groups have different attitudes towards slow loris as the Penan hunt, eat, or keep them as pets while the Iban consider them sacred and signifiers of good luck. We advocate the use of LEK for providing complementary information to scientific methods in the study of cryptic animals.

    INTRODUCTION: Laparoscopic live donor nephrectomy (LLDN) continues to expand in minimally invasive surgery; nevertheless, the studies are inadequate to compare standard kidney extraction with TV-NOSE in less-developed countries. This study compared TV-NOSE with conventional kidney specimen extractions.

    OBJECTIVE: To examine the feasibility of TV-NOSE in live donor nephrectomy.

    MATERIALS AND METHODS: 53 patients received LDN surgery at our hospital from September 2017 to December 2021. Retrospectively, living donor nephrectomy with TV-NOSE was compared to three different surgical procedures with standard specimen extraction.

    RESULTS: 53 donor patients were included: 15 open (OLDN), 12 retroperitoneoscopic living donor nephrectomy (RPLDN), 10 transperitoneal living donor nephrectomy (TPLDN), and 16 standard laparoscopic living donor nephrectomy with transvaginal extraction (SLLDN TV-NOSE). SLLDN TVNOSE's longer operating time (p<0.0041) did not affect graft function. SLLDN TV-NOSE and RPLDN had shorter lengths of stay and better VAS trends than open LDN and TPLDN (p<0.05). SLLDN TV-NOSE donors reported acceptable surgical outcomes and unchanged sexual function. All patients had similar discharge creatinine levels, with 1-year transplant survival of 98% and just 1 graft loss in the TPLDN group.

    CONCLUSION: SLLDN TV-NOSE is equivalent to RPLDN and better than open LDN and TPLDN in terms of duration of stay, VAS score, surgical outcomes, and sexual function. TVNOSE is a safe surgical procedure with an acceptable donor complication. TV-NOSE may be safely conducted in both developed and developing countries with proper patient selection.

    Immunization with anti-idiotype (Id) antibodies represents a novel new approach to active immunotherapy. Extensive studies in animal tumor models have demonstrated the efficacy of anti-Id vaccines in preventing tumor growth and curing mice with established tumor. We have developed and characterized several murine monoclonal anti-Id antibodies (Ab2) which mimic distinct human tumor-associated antigens (TAA) and can be used as surrogate antigens for triggering active anti-tumor immunity in cancer patients. Encouraging results have been obtained in recent clinical trials. In this article, we will review the existing literature and summarize our own findings showing the potential of this approach for various human cancers. We will also discuss where anti-Id vaccines may perform better than traditional antigen vaccines.

    If asked to describe the term "anesthesiologist scholar", one may receive a variety of definitions depending on the individual's area of practice, location in the world, and the generation in which they trained. In this article, we review the roles of five core elements that make an anesthesiologist a "scholar": skills in critical appraisal, literature review, quality improvement, journal club participation, and presentation delivery. Although this list of scholarly components is not comprehensive, review of each element's role in the everyday practice and training of physicians will offer insight into their evolution and may offer a glimpse into the future of anesthesiologist scholars. Overall, through the dissemination, recognition, and support of scholarship through these practices, we will continue to achieve meaningful outcomes for our patients and promote a culture of collaboration worldwide. We should ensure that these topic areas become a bedrock of medical education globally, and we must foster opportunities for those who have already completed training to develop and master these skills as a part of their clinical and academic practice.

    Beta-thalassaemia is characterized by a decrease (β(+)) or absence (β(0)) in the synthesis of β-globin chains of human haemoglobin. The heterozygous state for β(+) or β(0) result in β-thalassaemia trait in which the hallmark is the presence of an elevated level of Haemoglobin (Hb) A(2) (α(2)δ(2)). In the past, the traditional methods such as cellulose acetate electrophoresis with elution and microcolumn chromatrography have been the techniques used by the majority of the laboratories in Malaysia for the estimation of (Hb) A(2) levels. The recommended method currently is high performance liquid chromatography which has only been introduced in a few laboratories in the country.

    Illness and injury have a significant impact on employees, their families and employers. The consequences faced by an injured worker could lead to disability, which could then lead to inability to work. This study examined the patterns of the Return to Work (RTW) using data from The Social Security Organisation (SOCSO) of Malaysia RTW database from 2010 to 2013. Factors of successful return to work, employees' salary upon returning to formal employment were also investigated. Gender, age, year of injury, industry, and job hierarchy were found to be significant predictors of employees' salary upon returning to work. Although there are other costs involved on the part of employers and employees, themselves, in the long term the financial returns that can be brought back by injured workers who have successfully returned to work combined with the qualitative benefits substantially outweighs the costs of RTW program.

    A study was undertaken in FELDA (Federal Land Development Authority) resettlement scheme areas in Pahang, Malaysia, to determine the effectiveness of group dietary counselling in motivating diabetic patients to achieve good dietary habits, and weight and diabetes control. Sixty-one non-insulin dependent diabetes mellitus (NIDDM) patients were randomly assigned to either the experimental or control group. The experimental group received six sessions of group dietary counselling over 5 months and the control group received mass media diabetes-educational program during the same period. The one hour group dietary counselling sessions discussed general knowledge of diabetes, food groups for meal planning, the importance of dietary fibre-rich foods, types of fat in food, exercise and weight control. The experimental group met monthly with a dietitian as a counsellor. Effectiveness was assessed by improvement in food choice, and decline in percentage glycated haemoglobin (total HbA1) or body mass index (BMI). Measurements were made at a baseline visit, every two months during the six month program, and six months afterwards. Patients in the experimental group improved their food choices, resulting in a healthier diet high in unrefined carbohydrates and dietary fibre rich foods, and low in fat. There were significant reductions of their percentage total HbA1 levels and BMI following the counselling sessions, which decreased further six months after the program compared with patients in the control group. Thus group dietary counselling is effective in motivating NIDDM patients to achieve better food choice, and related weight and glycaemic control in a Malaysian setting.

    Detailed geophysical investigations have been carried out using integrated geophysical methods with a view to characterising the subsurface lithologic features that might indicate suitable places for structural developments. An overview of the subsurface resistivity distribution has been achieved employing 8Vertical Electrical Soundings with the Schlumberger array and 4 2D resistivity imaging using Wenner array. In order to constrain the results of the electrical resistivity methods, we carried out a ground magnetic survey along E-W direction using the Proton precession magnetometer at 1m sampling interval. Analysis of well logs data available and VES results showed 4 to 5 geoelectric layers corresponding to sand, clayey sand, clay, silty sand and sandy clay. The 2D resistivity imaging sections showed relative decrease of apparent resistivity with depth implying a geological transition from sand with high resistivity value of about 508Ωm to clay with low resistivity value 16Ωm at depths of 0-20m and 25-50m respectively. The magnetic profiles showed that the study area was characterised by short wavelengths and amplitudes ranging from –3800 to 700 nT. The highs and lows of the magnetic responses occasioned by lithological variations and structural features were magnetically resolved. In view of the identified subsurface structures, the suggested depth to the competent layer is about 20m for low to medium structures while above 50m would be suitable for heavy or massive engineering structures. The use of integrated geophysical methods for the delineation, identification and imaging of the subsurface geological structures which could provide clues to the nature and type of foundation suitable for the development of the study area has been successfully achieved.

    This study sought to investigate understanding of obesity among registered nurse e-PJJ student semester 9 UiTM in Puncak Alam, in Selangor, Malaysia. A survey research design was used for the study. 100 nurses were randomly selected from 130 nurses in e-PJJ student semester 9. Study of this population was done by systematic sampling. The target groups for this study are matrons, sisters and staff nurses. 100% nurses showed understanding of obesity. Eating habits of the nurses contributed to this obesity. It was recommended among others, that nurses should practice theoretical knowledge base and the need to more opportunities for physical activities at hospital sites was emphasized.

    Distribution of gold nanoparticles (AuNPs) on a substrate becomes crucial in nanotechnology applications. This work
    describes a route to fabricate AuNPs directly on silicon substrates by using an aluminum template in hydrothermal reaction
    at 80°C for 1 h. The effect of aluminum nitrate (Al(NO3
    )3
    ) concentration in the hydrothermal bath was investigated. The
    properties of AuNPs were studied using field-emission scanning electron microscope (FESEM), x-ray diffractometer (XRD)
    and semiconductor characterization system (SCS). Two distinct sizes of AuNPs were observed by FESEM. XRD analysis
    proved the formation of AuNPs directly on the substrate. AuNPs were embedded between polymethylsilsesquioxane
    (PMSSQ) in order to investigate their effect on memory properties. The sample grown in 0.1 M Al(NO3
    )3
    exhibited the
    largest hysteresis window (2.6 V) and the lowest Vth (2.2 V) to turn ‘ON’ the memory device. This indicated that good
    distribution of FCC structure AuNPs with 80±4 nm and 42±7 nm of large and small particles produced better charge storage
    capability. Charge transport mechanisms of AuNPs embedded in PMSSQ were explained in details whereby electrons from
    Si are transported across the barrier by thermionic effects via field-assisted lowering at the Si-PMSSQ interface with the
    combination of the Schottky and Poole Frenkel emission effect in Region 1. Trapped charge limited current (TCLC) and
    space charge limited current (SCLC) transport mechanism occurred in Region 2 and Region 3.

    Mangosteen (Garcinia mangostana Linn.) is a highly beneficial fruit, containing potent bioactive compounds such as xanthones. However, the metabolite comparisons of mangosteen pericarp, aril and seed have not been described in detail previously. A comparative approach was undertaken in the present work to evaluate the effects of different ratios of solvent combination in the metabolite extraction of mangosteen. Gas chromatography-based metabolomics approach was performed to evaluate the efficiency of two metabolite extraction methods utilising different solvent extraction ratios (3/1/1 v/v or 2/1/2 v/v of methanol/chloroform/water) in determining the primary metabolite composition of mangosteen fruit tissues (pericarp, aril and seed). Cumulatively, 43 known metabolites were putatively identified from the mangosteen fruit tissues. Due to the higher ratio of polar solvent (methanol and water) used in method 2 as compared to method 1, the former method preferentially extracted a higher number of polar metabolites. Conversely, the higher ratio of methanol solvent in method 1 also contributed to the identification of more alcohol metabolites. Additionally, the multivariate analysis revealed that mangosteen pericarp was mainly localised by ribonic acid, arabinopyranose, β-hydroxypyruvic acid, L-(+)-tartaric acid and galacturonic acid. Meanwhile, thymol-α-D- glucopyranoside and D-ribofuranose contributed to the separation of mangosteen aril, whereas mangosteen seed contained high levels of β-D-galactofuranose, L-threonic acid, butanoic acid, glycoside, malic acid and myo-inositol. Results suggested that the differing solvent ratios can highly influence the types and levels of the extracted metabolites. This finding highlights the influence of metabolite solvent extraction methods towards the end results of the extraction as well as the localisation of primary metabolites in different mangosteen fruit tissues. Hence, the present work is vital in revealing important spatial information of various metabolites toward a better understanding of the mangosteen fruit ripening process.
