    The family Sterculiaceae is one of the most important families among flowering plants. Many of its members demonstrate medicinal properties and have been used for the treatment of various ailments and wounds. A wide range of compounds including alkaloids, phenyl propanoids, flavonoids, terpenoids and other types of compounds including hydrocarbons, sugars, quinones, phenolic acids, lactones, lignans, amine and amides have been isolated from several species in this family. Few studies have reported that some extracts and single compounds isolated from this family exhibited several biological activities, such as antimicrobial, anti-inflammatory, antioxidant and cytotoxic activities. The present review is an effort to provide information about the traditional uses, phytochemistry and pharmacology of species from family Sterculiaceae, and to uncover the gaps and potentials requiring further research opportunities regarding the chemistry and pharmacy of this family.

    The main objective of this work was to determine the effectiveness of various biofouling reducers (BFRs) to operational condition in hybrid membrane bioreactor (MBR) of palm oil mill effluent (POME). A series of tests involving three bench scale (100 L) hybrid MBR were operated at sludge retention times (SRTs) of 30 days with biofouling reducer (BFR). Three different biofouling reducers (BFRs) were powdered actived carbon (PAC), zeolite (Ze), and Moringa oleifera (Mo) with doses of 4, 8 and 12 g L(-1) respectively were used. Short-term filtration trials and critical flux tests were conducted. Results showed that, all BFRs successfully removed soluble microbial products (SMP), for PAC, Ze, and Mo at 58%, 42%, and 48%, respectively. At their optimum dosages, PAC provided above 70% reductions and 85% in fouling rates during the short-term filtration and critical flux tests.

    Thrips are key pests of mango, Mangifera indica (L.), in Malaysia, including the Northern Peninsular. As Penang has year-round equatorial climate and high of rainfall, the populations of thrips may be subject to variations in composition and size. With a goal of developing an appropriate control strategy, a survey was conducted in Penang to determine species composition and abundance in relation to some environmental factors. Sprayed and unsprayed orchards were sampled on weekly basis through two flowering seasons of 2009 using CO(2) collection technique. Larval population falling into the ground to pupate and adults emerging from the soil were investigated in both orchards. Thrips hawaiiensis (Morgan) and Scirtothrips dorsalis (Hood) were the most prevalent species in the sprayed and the unsprayed orchards, respectively. The abundance of thrips was high during the flowering period of the dry season and decreased during the flowering period of the rainy season. This latter period coincided with decreased temperature and increased relative humidity. Percentage of adult emergence from the soil was lower in the rainy season than recorded in the dry season in both orchards. Taken together, these observations suggest that T. hawaiiensis and S. dorsalis are the main thrips species pests of mango panicles in Penang. Direct control with insecticides focusing on these two species may help to reduce cosmetic injuries and other damages on mango fruits.

    Coconut cadong-cadong viroid (CCCVd) causes the Lethal cadang-cadang disease of coconut palms in the Philippines and it is recently reported to be associated with the orange spotting disease on oil palm in Malaysia. The low concentration of the viroid RNA in oil palm as well as the high content of polyphenols and polysaccharides in this plant which interfere with the purification steps makes it difficult to extract and detect this viroid from oil palm. A previously described method was modified and optimized for extraction and detection of CCCVd from infected oil palms. Briefly, 7 g of leaf material was homogenized in a mortar or a blender using liquid nitrogen. 10 ml of extraction buffer (100 mM Tris-HCl pH 7.5, 100 mM NaCl, 10 mM EDTA) along with 100 mM 2-mercaptoethanol and 10 ml water saturated phenol was added to the frozen powder. After centrifuging at 4 degrees C, 4000 g for 30 min, the aqueous phase was extracted once more with phenol then once with chloroform-isoamyl alcohol (24:1). After adding sodium acetate, pH 5.6 to 200 mM, the mixture was precipitated with 2.5 vol ethanol overnight in -20 freezer and then the pellet was washed with 70% ethanol and air-dried. One milliliter of 8 M LiCl was added to the dried pellet and after shaking overnight at 4 degrees C and another centrifugation step the supernatant was collected and precipitated again with ethanol and then the resulting pellet was washed and air-dried. To carry out northern blotting, samples equivalent to 40 g of plant tissue were mixed with formamide buffer and loaded onto a 12% polyacrylamide gel containing 7 M urea and after separation by electrophoresis, were electroblotted onto membrane and fixed by UV cross-linking. Pre-hybridization and hybridization using hybridization buffer (50% formamide, 25%SSPE, 0.1% Ficol and PVP, 0.1 % SDS, 0.02 % DNA (5mg/ml)) was carried out at 45 degrees C for 90 min and 16 h, respectively followed by two low stringency washes (0.5 X SSC, 0.1% SDS, at room temperature for 5 min) and one high stringency wash (0.1X SSC, 0.1% SDS at 60 degrees C for 1 hour). In vitro synthesized DIG-labeled full-length CCCVd(-) RNA probe was used in hybridization step. DIG Nucleic Acid Detection Kit (Roche) instructions were followed for detection procedure and as a result the blue bands corresponding to the position of the viroid were appeared on the membrane. The result of this study showed the ability of DIG labeled probe in detection of the viroid and also provided a suitable extraction and hybridization method for the detection of CCCVd from oil palm.

    Historically, the recommended treatment for paranasal sinus mucoceles is the complete excision of through an open approach to achieve a cure. Though with the advent of Endoscopic sinus surgery, transnasal Endoscopic sinus surgery has gained more attention in order to manage the sinus mucocele. The aim of this study is to present the efficacy of the Endoscopic marsupialization of sinus mucoceles. From 2001 to 2005, 18 patients with paranasal sinus mucoceles were treated endoscopically. This series includes 6 fronto-ethmoidal, 2 maxillary, 4 ethmoid, 2 sphenoid, and 4 middle turbinate. The presenting signs, symptoms, and radiological findings were reviewed. All patients underwent endoscopic-wide marsupialization of the mucocele; the mean follow up was 13 months. There are 10 male and 8 female subjects who were of an age range of 29-72 years. Patients were treated with endoscopic marsupialization of the mucocele. There were no recurrences in the mean 13-month follow-ups in 17(94%) of patients. Only one patient needed revision endoscopic surgery. Mucocele happens to be the most commonly benign lesion, which causes the paranasal sinus to expand. There is increasing evidence that endoscopic marsupialization of sinus mucocele results in long-term control with very low recurrence rate at or close to 0%. Thus this technique is safe and less invasive than external approaches.

    Paraesophageal hernias have been historically associated with risk of substantial morbidity and mortality. We report a case of a 92 year old lady with acute gastric outlet obstruction due to a chronic paraesophageal hernia that was successfully treated by minimal invasive surgery. Anterior gastropexy was performed after the stomach was reduced. The hiatal opening was enlarged to reduce the risk of obstruction in the future. She was discharged well on the third day.

    Metal implants such as titanium, stainless steel and Co-Cr-Mo are used for load bearing purposes such as hip joint prostheses, fixing plates and dental root implants. For practical application, plasma-sprayed coatings of hydroxyapatite (HA) on metal implants are applied to promote early formation of strong bonds between metal implant and living bone. Plasma spray coating involves heating of HA material to a semi-molten or molten state and then propels its to a metal substrate. The plasma flame temperature is in the range of 6,000 degrees C to 16,000 degrees C but the surface temperature of the substrate rarely exceeds 150 degrees C. The HA materials are feed into the spray gun in the form of powders. Furthermore, this paper will discuss the processes of plasma-sprayed coating of HA on various types of metal implants.

    Cefotaxime [HR 756], a third generation cephalosporin with pronounced antibacterial activity
    against the Enterobacteriaceae, was assessed in serious and problem antibiotic resistant infection. Good clinical success was achieved without observed untoward effects. The study suggests that due to its properties, cefotaxime could be used as a first-line antibiotic provided that the clinical situation warrants the use of a cephalosporin or aminoglycoside.

The Kiefer-Neumann model of labor market search is adapted to marital search, the aim being to explain both the positive sorting on educational levels for spouses, and the positive relationship between female schooling and age at 1st marriage. World Fertility Survey data for Indonesia, Korea, and Malaysia are employed in the analyses.

    Results from a 6-year study of solar ultraviolet A (UVA) radiation measurements at the equatorial location of Penang (5 degrees N) are presented. On clear days, the diurnal flux reaches a very high dosage of about 3.0 x 10(-2) KWHM-2 around midday. The average daily total flux is in the range of 1.6 x 10(-1) KWHM-2 and does not change much seasonally. The high 83% cloud cover only reduces the incoming flux to about half. The radiation flux represents a lower limit of the incident UVA radiation applicable to much of the equatorial/tropical region.

    INTRODUCTION: Cyclooxygenase-2 (COX-2) promotes carcinogenesis by inducing proliferation and angiogenesis while decreasing apoptosis and immunosuppressive activity. It is overexpressed in many malignancies including renal cell carcinoma (RCC). The aim of this study was to investigate COX-2 expression in clear cell RCC and its association with tumour grades and demographic parameters.

    MATERIALS AND METHODS: Thirty-six clear cell RCC cases were selected. There were 21 (58.3%) men and 15 (41.7%) women with median age of 56.6 years (range: 16-74 years). Chinese constituted 16 (44.4%) of the cases; Malays 14 (38.9%) cases and Indian 6 (16.7%) cases. There were 6 (16.7%) grade 1, 20 (55.6%) grade 2, 10 (27.8%) grade 3 and none was grade 4. The paraffin embedded tissues were cut at 4 μm thick and stained with COX-2 monoclonal antibody.

    RESULTS: Eighteen (50%) of the RCC cases were immunopositive, of which all showed strong positivity. The immunopositive cases showed cytoplasmic membrane positivity.

    CONCLUSION: There was no significant association between COX-2 expression with grade, age, sex and ethnicity (p=0.457, p=0.054, p=0.389 and p=0.568 respectively). Strong positivity of COX-2 suggest that COX-2 may play a role in cell proliferation and in carcinogenesis.

    BACKGROUND: Published reports by the Social Welfare Department of Malaysia suggests that child abuse and neglect cases has been steadily increasing. There is a lack of basic data and qualitative study on child maltreatment in Malaysia.

    OBJECTIVE: The aim is to describe the pattern and demographic features of all suspected child abuse and neglect (SCAN) cases seen in a single tertiary hospital in Malaysia over a period of five years.

    METHODS: A retrospective descriptive review of children suspected of maltreatment, aged 0 to 18 years old who presented to the Hospital Serdang (HS), Selangor, Malaysia from January 2014 to December 2018 was done. A list of registered SCAN cases obtained from One Stop Crisis Centre (OSCC) HS census. Clinical information of patients was retrieved from the computerised database.

    RESULTS: In all, there were a total of 391 SCAN cases over five years with almost a 3-fold increase in the number of cases from 2014 to 2018. Physical abuse was the most common (55%) seen followed by sexual abuse (34%) and neglect (10%). There were four deaths, all involving infants <1 year old who were physically abused by babysitters. The main perpetrator in sexual abuse were people known to the victim. Ninety-three percent of patients were neglected by their biological parents and more than 2/3rd of neglect cases occurred due to inadequate supervision.

    DISCUSSION: Child maltreatment is an inevitable burden to our health system and infants are the most vulnerable group to sustain significant injuries leading to death and disabilities.

    Crude glycoside extracts from the plant, Sarcolobus globosus, were tested on the rat phrenic nerve-diaphragm, chick biventer cervicis and frog rectus abdominis preparations. Nerve-stimulated twitches were inhibited by the extract. The muscle paralysis was not similar to that by curare-like blockers as it was not reversed by neostigmine or by a tetanus. Although contractures to acetylcholine or carbachol were not affected by 0.6 mg/ml of the extract, higher concentration of the extracts (3 mg/ml) depressed the log dose-response curve of acetylcholine and carbachol. The results suggest that the neuromuscular blocking effect of the extracts is either dose-dependent or due to a mixture of toxins with presynaptic or postsynaptic actions.

    The first carpometacarpal (CMC) joint, located at the base of the thumb and formed by the junction between the first metacarpal and trapezium, is a common site for osteoarthritis of the hand. The shape of both the first metacarpal and trapezium contributes to the intrinsic bony stability of the joint, and variability in the morphology of both these bones can affect the joint's function. The objectives of this study were to quantify the morphological variation in the complete metacarpal and trapezium and determine any correlation between anatomical features of these two components of the first CMC joint. A multi-object statistical shape modelling pipeline, consisting of scaling, hierarchical rigid registration, non-rigid registration and projection pursuit principal component analysis, was implemented. Four anatomical measures were quantified from the shape model, namely the first metacarpal articular tilt and torsion angles and the trapezium length and width.
