    β-wollastonite (β-CaSiO3) was synthesized from rice husk ash and calcium carbonate, and a study of the effects of the autoclaving and sintering steps is presented here. Autoclaving and sintering at 8h and 2h yielded the β-wollastonite phase in full, with improved crystallinity. Nucleation between rice husk ash and calcium oxide occurred around 135 °C, pressure 0.24 MPa, and growth proved to be more crystalline after the ripening period. For shorter processing times, and for both unsintered and unautoclaved samples, cristobalite and unstable tricalcium silicate phases were present. Crystallite size was increased by longer sintering times but reduced by longer autoclaving times. The β-wollastonite obtained had a random branch-like structure. In conclusion, the introduction of the autoclaving step successfully obtained β-wollastonite from a reaction between rice husk ash and calcium oxide.

    Sixty two patients with displaced femoral neck fractures treated by percutaneous pinning are reviewed. There were 37 females and 25 males with an average age of 63.7 years. There were 36 Garden grade III and 26 grade IV fractures. Twenty three patients had other medical conditions. Fourteen patients were operated under local anaesthesia. Superficial wound infection was found in three cases with no deep infection. Union occurred in 41 patients by eight months. Of the 21 non-unions 15 remained painless. The pins migrated outwards in seven cases but caused no clinical problem. Avascular necrosis was seen in 11 patients by 18 months. Avascular necrosis and non-union occurred together in six patients. Secondary hemiarthroplasty was performed in only ten patients. Routine hemiarthroplasty for femoral neck fractures is not always indicated. Femoral head preservation should be attempted before prosthetic replacement. The pinning surgery is simple, fast and can be performed under local anaesthesia. Hemiarthroplasty should be reserved for failed cases only.

    BACKGROUND: Epidermal growth factor receptor (EGFR) gene in lung adenocarcinoma is associated with good clinical response to EGFR-tyrosine kinase therapy. The two most common EGFR gene mutations, representing 80 to 90%, are the E746-A750 deletion in exon 19 and the L858R point mutation in exon 21.

    MATERIALS AND METHODS: We have conducted the study to evaluate immunohistochemistry's performance in detecting the E746-A750 deletion in exon 19 of the EGFR gene in primary lung adenocarcinoma cases. This study examined 133 cases of primary lung adenocarcinoma for three years duration. The selected cases were tested for EGFR gene mutations by real-time PCR by a reference laboratory. Most cases (124) were diagnosed by tissue biopsy, though nine used cell block cytology. We performed an immunohistochemistry test on 75 cases that contained adequate diagnostic material in the paraffin block.

    RESULTS: The test result was scored as 0 to 3+, based on the staining intensity and percentage of positive tumor cells. We evaluated the immunohistochemistry test's sensitivity and specificity compared to the EGFR gene mutations by real-time PCR. There was a significant association between gender, smoking status, and the EGFR gene mutations (P < 0.001). The overall sensitivity and specificity of the immunohistochemistry test were 40% and 100%, respectively. The positive predictive value and negative predictive values were 100% and 76.9%, each.

    CONCLUSIONS: The immunohistochemistry has high specificity but low sensitivity in the detection of E746-A750 deletion in exon 19 of the EGFR gene. The mutation-specific antibody used in this study was unable to detect other uncommon variants of exon 19 deletions. With high specificity value, immunohistochemistry may provide an adjunct to molecular testing for detecting the most common EGFR gene mutations in cases of a low cellularity sample, financially-limited situations, or in critically ill cases where urgent targeted therapy is needed.

    OBJECTIVE: The objective of this randomized controlled trial (RCT) was to investigate the effectiveness of the lower limb rehabilitation protocol (LLRP) combined with mobile health (mHealth) applications on knee pain, mobility, functional activity and activities of daily living (ADL) among knee osteoarthritis (OA) patients who were overweight and obese.

    METHODS: This study was a single-blind, RCT conducted at Teaching Bay of Rehmatul-Lil-Alameen Post Graduate Institute of Cardiology between February and November 2020. 114 knee OA patients who were overweight and obese were randomly divided by a computer-generated number into the rehabilitation group with mHealth (RGw-mHealth) to receive LLRP + instructions of daily care (IDC) combined with mHealth intervention, rehabilitation group without mHealth (RGwo-mHealth) to receive LLRP + IDC intervention and control group (CG) to receive IDC intervention. All three groups were also provided leaflets explaining about their intervention. The primary outcome measure was knee pain measured by the Western Ontario and McMaster Universities Osteoarthritis Index score. The secondary outcome measures were mobility measured by the Timed up and go (TUG) test, functional activity measured by the patient-specific functional scale (PSFS), and ADL measured by the Katz Index of independence in ADL scores.

    RESULTS: Among the 114 patients who were randomized (mean age, 53 years), 96 (84%) completed the trial. After 3-months of intervention, patients in all three groups had statistically significant knee pain reduction (RGw-mHealth: 2.54; RGwo-mHealth: 1.47; and CG: 0.37) within groups (P  0.05).     Context Familial hypercholesterolaemia (FH) is a common autosomal dominant disorder, causing elevated cholesterol from birth, premature heart disease, and early death. Objective This study explored primary care physicians' experiences and perspectives on identifying FH in Malaysian primary care. Study Design and Analysis A qualitative study involving semi-structured interviews and focus group discussions with 22 primary care physicians (PCPs) in two primary care clinic settings. The interviews and focus group discussions were audio recorded, and the recordings were transcribed verbatim. The data in the transcripts were analysed using thematic approach. Setting Primary Care Clinics Population Studied Primary Care Physicians in two primary care clinics. Intervention A qualitative study involving semi-structured interviews and focus group discussions Outcome Measures Primary Care Physicians' perceptions and experiences of identifying individuals at high risk of FH in their clinical practice, and the acceptability and perceived challenges of trying to do this were explored during the interviews and focus group discussions. Findings PCPs felt there was potential for FH to be identified earlier in primary care. They had some existing knowledge and awareness of diagnostic criteria for FH but highlighted several challenges. In their practice, this included limited time in routine clinical care, availability of medication and clinical expertise; and critical lack of family history and physical examination findings in health records. The barriers on a systemic level were shortage of lipid specialist services and the absence of local care pathways for FH. The PCPs recommended a user-friendly case-finding tool for FH, and establishing FH registry and clinical practice guideline in Malaysia, alongside a national FH screening strategy and awareness campaigns for both clinicians and general public. Conclusions PCPs are positive about improving the identification of FH in primary care. However greater support in their practice and wider system developments and change are needed.

    The identification of accounting fraud is an important measure to safeguard the interests of stakeholders and ensure the long-term development of the company. The current traditional methods for identifying accounting fraud rely on manual review and judgment, lacking objectivity and accuracy. In order to improve the accuracy of accounting fraud identification, improve identification efficiency and objectivity, this article combines smart city information technology to conduct in-depth research on data mining algorithms for accounting fraud identification. This article first provides a brief overview of smart cities and information technology, then introduces the basic theory of accounting fraud identification, and finally implements accounting fraud identification through k-means clustering mining algorithm. The data is divided into k clusters, and abnormal clusters are identified by checking the characteristics and attributes of each cluster. Compared with traditional rule-based and pattern based methods, this approach can more flexibly adapt to different types and forms of fraud, and can discover unknown patterns of fraud. In the experiment, this article used electronic data collection, analysis, and retrieval systems on the websites of the Shanghai Stock Exchange and Shenzhen Stock Exchange to collect 641 annual reports and financial characteristics from 62 listed companies that engaged in financial statement fraud and 84 companies that were not reported to have financial statement fraud from 2012 to 2021 as test samples. The results were tested and analyzed from several aspects, including the number of misjudgments, misjudgment rate, and ROC curve. The final test results show that compared to traditional accounting fraud identification methods, the comprehensive misjudgment rate of data mining algorithms based on smart cities has decreased by 3 %. The conclusion indicates that data mining algorithms used in smart city information technology to identify accounting fraud can help improve the accuracy of accounting fraud, improve audit objectivity and effectiveness.

    Tuberculous pleurisy is extra-pulmonary tuberculosis caused by Mycobacterium tuberculosis (MTB), which is one of the main cause of pleural effusions in developing countries. Intercostal chest catheter is useful for drainage of infected pleural fluid and facilitates sepsis control. However, management might be challenging in complex tuberculous pleural effusion as the septations within the effusion hinder pleural drainage. Intrapleural fibrinolysis therapy improved infected fluid drainage and septic parameter in parapneumonic effusions; however, there seems to be little data on its use in tuberculous pleurisy. In our case series of seven patients with complex tuberculous pleurisy, the use of intrapleural alteplase and deoxyribonuclease (DNase) facilitated fluid drainage which resulted in clinical and radiological improvement. These medications should not be confined to bacterial aetiology only as our case series highlights that in complex tuberculous pleurisy, intrapleural alteplase and DNase may be used as an adjunctive treatment which are proven to be successful and safe.

    Although a substantial body of evidence suggests that large and old trees have reduced metabolic levels, the search for the causes behind this observation has proved elusive. The strong coupling between age and size, commonly encountered in the field, precludes the isolation of the potential causes. We used standard propagation techniques (grafting and air-layering) to decouple the effects of size from those of age in affecting leaf structure, biochemistry and physiology of two broadleaved trees, Acer pseudoplatanus (a diffuse-porous species) and Fraxinus excelsior (a ring-porous species). The first year after establishment of the propagated plants, some of the measurements suggested the presence of age-related declines in metabolism, while other measurements either did not show any difference or suggested variability across treatments not associated with either age or size. During the second year after establishment, only one of the measured properties (specific leaf area) continued to show some evidence of an age-mediated decline (although much reduced compared to the field), whereas, for some properties (particularly for F. excelsior), even the opposite trend of age-related increases was apparent. We concluded that (1) our plants suffered from grafting shock during year 1 and they gradually recovered during year 2; (2) the results over 2 years do not support the statement that age directly mediates ageing in either species but instead suggest that size directly mediates ageing processes; and (3) neither shoots nor roots of A. pseudoplatanus showed any evidence of senescence.

    OBJECTIVES: This study aims to investigate the effectiveness of the lower limb rehabilitation protocol (LLRP) using mobile health (mHealth) on quality of life (QoL), functional strength, and functional capacity among knee OA patients who were overweight and obese.

    PATIENTS AND METHODS: Between August 2019 and November 2020, a total of 96 patients (42 males, 54 females; mean age; 52.9±4.8 years; range, 40 to 60 years) were randomized into either the rehabilitation group with mobile health (RGw-mHealth) receiving reminders by using mHealth to carry on the strengthening exercises of LLRP and instructions of daily care (IDC), the rehabilitation group without mobile health (RGwo-mHealth) following the strengthening exercises of LLRP and instructions of daily care (IDC) and control group (CG) only following the IDC for duration of 12 weeks. The reminders for using mHealth were provided two times a day for three days a week. Primary outcome measures were QoL assessed by the Western Ontario and McMaster Universities Osteoarthritis Index summary score, and functional strength by five-repetition sit-to-stand test. Secondary outcome measure was functional capacity assessed by the Gait Speed Test. The assessments of QoL, functional strength, and functional capacity were taken at baseline and post-test after 12 weeks of intervention.

    RESULTS: After 12 weeks of intervention, the patients in all three groups had a statistically significant improvement in QoL within groups (p<0.05). Patients in the RGw-mHealth and RGwo-mHealth had a statistically significant improvement in functional strength and walking gait speed within groups (p<0.05). The pairwise between-group comparisons (Bonferroni post-hoc test) of the mean changes in QoL, functional strength, and functional capacity at post-test assessments revealed that patients in the RGw-mHealth had a statistically significant greater mean change in QoL, functional strength and functional capacity relative to both the RGwo-mHealth and CG (p<0.001).

    CONCLUSION: The improvement in QoL, functional strength, and functional capacity was greater among patients in the RGw-mHealth compared to the RGwo-mHealth or CG.

    Introduction: Proper distribution of human resources is an important factor ensuring high-quality performance and sustained service quality. The aim of this study was determining the workload pressure among medical officers in health clinics (HCs) in Kelantan. Method: A record review survey was conducted between January and April 2019 using human resources data for 2018 involving HCs in Kelantan. It included all the HCs in Kelantan and excluded community clinics. Workload pressure was determined using a tool known as Workload Indicator of Staffing Needs, developed by World Health Organization. A high workload pressure was defined as a ratio between required and acquired medical officers of less than 1. The data were presented descriptively using as frequencies and percentages. Results: All 85 HCs in Kelantan were involved in the study; 90% (9/10) of the Kelantan districts recorded high work- load pressure. Moreover, 68.2% (58/85) HCs had high workload pressure. Tanah Merah, Tumpat, Pasir Mas, and Kota Bharu had the most HCs with high workload pressure, and most such HCs were found in areas with a high-den- sity population, requiring huge coverage. Conclusion: The Kelantan State Health Department should develop better human resource distribution strategies to ensure the sustainability of quality care in HCs.

    Introduction: Primary health care plays an important role in healthcare systems. In Malaysia, One (1) Malaysia Clinic (now known as Community Clinic since 2018) was established in 2010 aims to increase healthcare services accessibility within the poor urban population. The aim was to look at the effectiveness of One (1) Malaysia Clinic as a new form of public primary health clinic by looking at expectations and perceptions of the quality of the provided services by the patients. Method: It applied a cross sectional study involving nine clinics in of the busiest district in Kelantan namely, Kota Bharu district. 386 respondents which were Malaysian citizen, 18 years old and above were involved in this study. The survey used Malay validated SERVQUAL questionnaire. The Service Quality (SQ) gap was calculated. Result: The empathy dimension obtained the highest expectation and perception score, while the tangi- ble dimension has the lowest expectation and perception score. In all SERVQUAL dimension, the expectations and perceptions of the service provided was significantly different with P < 0.001. The most critical dimension identified was tangible dimension as it has the largest negative SQ gap score. Conclusion: Negative gaps for all SERVQUAL dimensions and individual items indicated that provided services at One (1) Malaysia Clinic still could not fulfilled the patient’s expectation. A continuous improvement program is needed especially on tangible dimension in deliv- ering a quality healthcare services.

    Family satisfaction has been identified as a quality indicator in critical care area. The impact of family satisfaction level was also found to be associated with symptoms of psychological distress such as anxiety, depression and stress among family members. We evaluated the satisfaction level and prevalence, risk factor and correlation of psychological distress symptoms with the satisfaction level among family members in Malaysian Intensive Care Unit (ICU). Materials and Method: This is a cross-sectional, multicentre study conducted in ICU at Hospital Sultanah Aminah Johor Bahru and International Islamic University Medical Centre. Family members were enrolled 3 days after ICU admission, and they completed a modified version of Critical Care Family Needs Inventory (CCFNI) and the Depression, Anxiety and Stress Scales (DASS). Results: A total of 176 family members were enrolled in this study. We found 116 (66%) of the family members scores ≥ 3 denoting satisfaction with the mean CCFNI score was 3.11 (SD=0.3). Prevalence of depression, anxiety and stress were of 30.1%, 41.4% and 28.9% respectively. Risk factor for psychological distress symptoms were female sex, spousal relationship, lower education, median income less than RM 4000, staying with patient and younger patient age. Negative correlation between depression, anxiety and stress with CCFNI score were found (p< 0.05) but with low correlation coefficient (r=-0.178 to -0.209). Family members without symptoms of psychological distress were more satisfied (higher CCFNI score) with ICU care compared to those with symptoms of psychological distress (p< 0.05). Conclusion: Family members of ICU patient were satisfied with the care provided in the ICU. High rates of psychological distress symptoms in this study and its correlation with the satisfaction level highlight the need to identify and implement preventive and management strategies for family members to improve the overall ICU care.

Organisasi ini bertanggungjawab membangun dan mempromosikan produk-produk pelancongan Islam di Malaysia kepada pelancong luar. Ini kerana Malaysia merupakan antara tumpuan utama yang menjadi destinasi pilihan hampir 25% pengunjung daripada keseluruhan pelancong Muslim di serata dunia. Pulau Besar merupakan antara lokasi tumpuan di Melaka yang banyak dikaitkan dengan kisah mistik dan lagenda. Para pelancong dari dalam dan luar negara sering melawati pulau ini atas pelbagai tujuan dan keperluan. Kajian ini bertujuan menilai potensi dan cabaran Pulau Besar sebagai salah satu lokasi tumpuan pelancongan Islam khususnya di Melaka. Kajian dijalankan secara kualitatif menerusi sumber primer dan sumber sekunder yang dianalisis secara induktif. Keindahan alam semula jadi dan kewujudan makam para ulama Nusantara yang masyhur seperti Syeikh Ismail, Syeikh Ibrahim dan Syeikh Yusuf telah menjadi tarikan para pelancong khususnya dari rantau Asia Tenggara. Namun, amalan-amalan khurafat yang masih berleluasa terutamanya di beberapa lokasi ‘keramat’ perlu diberi perhatian bagi memastikan potensi pulau ini sebagai salah satu hub pelancongan Islam berjaya direalisasikan.

    Liposarcoma is one of the most common mesenchymal tumour in adults but it is rare to occur in the breast. Our case was a 50 year old single nulliparous woman who presented with a right breast mass for one year duration. The mass was progressively increasing in size in the last few months. Breast examination showed a huge mass measuring 5 x 8 x 6 cm occupying the entire right breast. Mammogram showed a large homogenous soft tissue mass occupying the entire right breast with foci of calcification. A trucut biopsy showed a cellular tumour which was thought to be an invasive carcinoma. The patient underwent right modified radical mastectomy with axillary clearance. Macroscopy showed a well circumscribed lobulated solid haemorrhagic yellowish tumour mass measuring 180 x 110 x 50 mm. Microscopically the tumour was heterogenous comprising cellular round nonlipogenic mesenchymal cells and loose myxoid areas containing small cells. The typical arborizing ‘chicken wire’ capillaries were observed. Vacuolated lipoblasts were seen. All eleven axillary lymph nodes sampled showed no metastasis. A diagnosis of a myxoid liposarcoma was made. To raise the suspicion of a possible mesenchymal tumour, it is very important for clinicians to relay the clinical and radiological findings to the pathologist to avoid misdiagnosis in a trucut biopsy.

    Maternal obesity is significantly associated with less intention to breastfeed, low breastfeeding self-efficacy, and poor breastfeeding outcomes. This cross sectional study investigates the determinants of breastfeeding self-efficacy among overweight and obese pregnant mothers using self-administered validated questionnaires. There were 44.9% of overweight and obese from 200 recruited mothers. They had low breastfeeding self-efficacy and less intended to breastfeed as compared to normal-weight mothers. Past breastfeeding experience and exclusive breastfeeding inten- tion are predictors of breastfeeding self-efficacy among overweight and obese pregnant mothers. Effective strategies should be conducted targeting this population with regards to low self-efficacy and intention to breastfeed.

    Training at high altitude for prolonged periods can cause low oxygen tension which can developed complication of hypoxia. Hypoxia is a cascade activity from a level of down regulation and function of cell’s nucleus. Early detection of biomarker and physiological changes are important in prevent the hypoxia at high altitude. Hyperbaric medicine is a new treatment that were used an oxygen therapy to treat hypoxic and inflammatory driven conditions which patients are treated with 100% oxygen at pressure greater than atmospheric pressure. The review discusses physiological changes associated with hypoxia, the response of biomarker hypoxia changes in high altitude and the role of hyperbaric oxygen therapy can play as part of the treatment for pilots and athletes training at high altitudes that suffering from disease with underlying hypoxia.
