Chronic low back pain is a common preventable occupational health illness affecting most workers. Large amount of financial and benefit cost had been spent by the developed countries to prevent, treat and rehabilitate a large number of workers who are exposed to hazards that are attributed to low back pain. Efforts on primary prevention of low back pain had been challenging due to difficulties in affirming work- relatedness of chronic back pain among workers. As such, efforts have to be focused on existing literatures to propose acceptable variables to develop the definition of workrelatedness specific to chronic low back pain. Such identified variables or factors could be used to develop a set of criteria in defining work- related chronic back pain. Literature search using specific work- related and chronic low back pain key words were used. Comparable articles were judged and a summarized result was obtained. These variables could be grouped into individual characteristics, health behaviours, physical conditions at work, work organizations and ergonomic factors. With proper methodology and statistical analysis, tools could be developed to aid physicians in determining work- related chronic low back pain among employees.
Substance misuse, in particular heroin addiction contributes to health and social problems. Although effective medical treatment was available, earlier efforts confined the treatment of heroin addicts to in-house rehabilitation which required them to be estranged from the community and their families for 2 years. The in-house rehabilitative programme, implemented for at least three decades has produced low abstinence rates. On the other hand, being ‘away’ meant that many heroin addicts faced employment problems and family relationship difficulties upon completing the in-house rehabilitation. However, recently, the concerted efforts by various government and non-government organisations, and the acknowledgement that heroin addiction is a medical illness has resulted in a revamp to approaching treatment of heroin addiction. At present, methadone substitution programmes have been offered as part of treatment programme for heroin addicts in Malaysia. This new programme has been shown to be effective in treating heroin addiction and would need support and cooperation from all groups involved.
Dental caries is the main oral health problem in hearing-impaired (HI) children and remains the most neglected need. The present study aimed to determine caries prevalence and treatment needs in HI children and the association with salivary parameters. A cross-sectional study was conducted on 63 HI children aged between 7-14 years who attended a special school for the deaf. Clinical oral examination was done and salivary parameters (resting flow rate and resting pH) were measured. Caries experience was charted using the index of decay-filled teeth (dft) and Decay-Missing-Filled Teeth (DMFT) for primary and permanent dentition respectively. Data were analysed using SPSS version 12.0. The mean age was 11.5 (SD 2.39) years and 53.8% were female. Dental caries prevalence was 88.0% (95% Cl: 73.0, 100.0) in primary dentition and 85.0% (95% Cl: 73.0, 96.0) in permanent dentition. The mean dft was 6.1 (SD 4.14) and the mean DMFT was 4.9 (SD 3.28). The mean resting flow rate was 0.14 (SD 0.08) ml/min while mean pH was 6.8 (SD 0.79). Both pit and fissure sealants and restorations were the highest (83.1%) treatment needs. Only 3.1% of the children did not require any treatment. There were no significant association between both salivary flow rate and salivary pH with caries experience in the primary (p=0.342, p=0.610 respectively) and permanent (p= 0.99, p=0.70 respectively) teeth. In conclusion, children with HI have high caries prevalence and unmet need for dental treatments. Salivary pH and resting flow rate of the children were not associated with their caries experience.

The objective of this survey was to understand the current trend of readership of professional dental journals among Malaysian dentists. A total of 225 questionnaires were sent out to Malaysian dentists who attended various dental related conferences throughout Peninsular Malaysia from February 2006 to July 2006. Questionnaires comprised of questions relating to dentists’ socio-demographic status and a list of journal(s) read by them. Malaysian dentists’ view on the content and quality of a particular dental journal, i.e. the Malaysian Dental Journal (MDJ) was also enquired. The details of this finding are highlighted in Part II of this study. A total of 156 questionnaires were returned; the respondents were made up of 61 male and 91 female dentists. Almost 80% of the respondents aged between 20-49 year-old and most respondents (n= 132; 84.62%) only had a basic Bachelor of Dental Surgery or equivalent degree while another 19 (12.18%) had in addition, a post-graduate degree. Almost equal numbers of respondents were working in the Ministry of Health (MOH) or Armed Force (n=73; 46.8%) and private practice (n=74; 47.4%). Also, equal number of respondents (n=67; 42.95%) were found to be working as single-handed practitioner and in a partnership/assistant/working-with-other specialties type of practice Almost two-thirds (n=103; 66%) of the respondents read more than one professional journal, and a majority of them worked in the private sector. The percentage of readers reading more than one journal from the private practice (n=67, 60.0%) was close to twice of that from the MOH (n=36, 35.0%). No specific age-group pattern was present but the least number of subscribers were from those 60 year-old and above (n=3), whereby none of them subscribed to any professional dental journal/magazine. The highest percentage of subscribers were from those in the age group of 40-49 year-old, whereby 86.49% (n=32) of dentists in this age-group subscribed to at least one professional dental journal/magazine. Out of the list of journals/magazines provided, it was found that the MDJ has the most number of readers. The MDJ was most read by dentists in the private practice while the Annals of Dentistry of the University of Malaya was most read by dentists in the MOH. In conclusion, it was found that almost two-third of the respondents read more than one professional journal, with the MDJ receiving the most number of readers. More dentists in the private practice read professional dental journals than dentists in the MOH.
The objective of this part of the study was to understand the current trend on readership of the Malaysian Dental Journal (MDJ) among Malaysian dentists. Their views on the contents and quality of the Malaysian Dental Journal were enquired. We also enquired the reasons they chose-to/chose-not-to read the MDJ. Of the 225 dentists surveyed, the number of MDJ readers was 101; with only 24.75% reading all issues published. The editorial section was rated as “useful” by 70.3% of readers, while 79.2%, 87.1%, 87.1% and 80.2% of readers rated the research article section, the review article section, the case reports section and book recommendation section similarly respectively. Feedback from readers indicated that they wanted more case reports, more review articles on “how to do it” and on medical problems in dentistry. More than half (55.45%) of the MDJ readers preferred to receive the journal in both hard and soft copies. For the non-readers, the most common reasons cited for not reading the MDJ was not being able to access to the journal, followed by not having time to read. Our finding suggested that the respondents preferred to learn from colleagues’ experience and to read article that can improve their clinical knowledge and skill.
Introduction: In Malaysia, medical specialisation training is mainly carried out by the public universities. Methods: A survey was carried out to explore the views of medical specialists in the country on issues such as structure of medical specialty training, availability of human resource, public/private sector competition, competency and apprenticeship, and its impact on assurance and quality of medical specialty training. Results: Altogether 238 medical specialists from 30 hospitals and medical institutions in the country participated in the survey. Conclusion: Among the findings, competition for human resource between public and private sector and lack of uniformity on medical specialty training across universities in the country are among the issues found to be of concern. There is also a need to address governance issue which necessitates to clearly delineating what constitutes medical specialty and what constitutes a subspecialty so that an agreed uniformed nomenclature is exercised across all stakeholders. The respondents also strongly agreed on the need to ensure competence in medical specialist training.
Stress is on one of the important hazards faced by nursing staff. Although various intervention methods to reduce stress have been developed, its effectiveness among Malaysian nurses has yet to be established. This study involved 185 health nursing staff working at health districts Kemaman and Dungun in Terengganu. Health nursing staff from Kemaman (n=94) was selected as intervention group whereas those from Dungun (n=91) were chosen as controls. Members of the intervention group were given stress awareness education. They were taught how to reduce stress by relaxation methods. Teaching was conducted through lectures, practical training and providing a stress guidebook. A stress reliever card and letter reminding respondents to practice stress reduction methods were sent every two weeks. The total intervention period was 6 weeks. The control did not receive any education on stress reduction or encouragement to practice any stress reduction techniques throughout this period. The effectiveness of the intervention programme was measured by comparing changes in stress symptoms scores. Stress symptoms were measured using a modified Personal Stress Inventory questionnaire. The study found significant reduction of stress symptoms (42.66 ± 20.36 to 35.51 ± 22.60) in the intervention group after intervention compared to before intervention. However there was no significant difference in change of mean score of stress symptoms between both groups. Further studies need to be conducted to establish an intervention programme, which is suitable and effective in reducing stress among health nursing staff.
Perkembangan industri permotoran telah menyebabkan pencemaran udara yang menjadi faktor kepada peningkatan simptom masalah respiratori dalam kalangan kanak-kanak sekolah rendah. Kajian ini mengkaji perkaitan antara peningkatan simptom masalah kesihatan respiratori dalam kalangan kanak-kanak sekolah rendah dengan pendedahan terhadap pencemaran udara akibat lalu lintas. Kajian school-based jenis keratan lintang telah dilakukan di dua buah sekolah di daerah Kota Bharu, Kelantan iaitu Sekolah Kebangsaan Langgar dan Sekolah Kebangsaan Demit dengan melibatkan pelajar berumur 10, 11 dan 12 tahun. Saiz sampel seramai 124 orang (n = 124) dipilih menggunakan corak persampelan kebarangkalian iaitu persampelan rawak mudah. Responden disoal menggunakan instrumen yang telah disahihkan dan diadaptasi dari International Study of Asthma and Allergies in Childhood (ISAAC). Analisis data dilakukan dengan menggunakan Statistical Package For Social Sciences (SPSS). Hasil kajian menunjukkan peningkatan simptom masalah respiratori yang dihadapi oleh pelajar di dua buah sekolah rendah di Kota Bharu adalah berkait rapat dengan pencemaran udara yang berpunca dari lalu lintas dengan p < 0.05. Kajian mendapati pendedahan kepada bahan pencemar udara yang berpunca dari lalu lintas telah meningkatkan simptom masalah respiratori dalam kalangan kanak-kanak sekolah di rendah lokasi kajian.
This retrospective study is aimed at evaluating the role of CT scan in predicting parametrium involvement in early stage of cervical carcinoma. It was conducted in a Gynaecologic Oncology Centre, Hospital Alor Star from January 2004 till December 2008. All patients with operable stage I and II cervical cancer had pelvic CT scan for evaluation of parametrium involvement before undergoing radical hysterectomy and pelvic lymphadenectomy. Parametrial streakiness or presence of infiltration suggested local invasion. Following radical hysterectomy, the specimens sent for histological confirmation and the correlation between the CT scan finding and the histopathology result was studied. The result revealed a total of 104 patients with operable stage cervical carcinoma had pelvic CT scan. The sensitivity and the specificity of CT scan in assessing parametrial involvement was 33.3% and 84.8%, respectively. In conclusion, CT scan had high specificity but low sensitivity in determining parametrial involvement in early stage of cervical cancer. Hence, routine preoperative pelvic CT scan has a limited role in assessing parametrial involvement in early stage cervical carcinoma.

Bioglass 45S5 in mole percentages (46.13% SiO2, 26.91% CaO, 24.35% Na2O and 2.60% P2O5)was synthesizedthrough melt-derived and characterized usingX-ray diffraction (XRD) and Fourier transform infrared spectroscopy (FTIR)to confirm and identify its properties.SHEDwere used to evaluate the biocompatibility of 45S5 by exposing the cells to various concentration of BG-conditioned medium (1-10 mg/ml) using alamarBlue assay. The BG produced has an amorphous structureas shown by XRD analysis. TheSi-O-Si bending, asymmetric Si-O stretching and asymmetricSi-O-Si stretchingbands were observed in the BG structure supporting the presenceof silicate network. For alamarBlue assay, SHED cultured in BG-conditioned medium showed high proliferation rate when subjected to minimal powder content in the DMEM cell culture medium.Hence, it can be concluded that SHED cultured in lower powder content of the BG-conditioned media showedhigh proliferative activity suggesting the potential of the BG for dental usage.

This paper aim is to design an education kit for wastewater system that can maintain
the standard parameters of neutralized wastewater by maintaining the suitable pH
(Potential Hydronium) level and temperature of the wastewater from industry by using
fuzzy controller. This study is capable to control the unwanted bacteria by automatic
regulatory and monitoring the temperature, pH and water level. Fuzzy logic method is
use to control and monitor pH level as well as the temperature during clarifying process
because pH control process is a complex physical-chemistry process of strong
individuality of time-varying and non-linearity properties. Pumps used in the prototype
need to be controlled precisely to enable either acid or base to be pumped into mix
tank of the wastewater treatment. The control and monitoring system, which has been
designed through LabVIEW front panel will ease end user in inspection of the
parameters involve in wastewater treatment. The entire system output could be
observed remotely in Data Dashboard application in smartphone or tablet. The GUI
was designed and interfaced with the prototype constructed to carry out the process
of controlling and monitoring the required parameters. Few tests were conducted
repetitively to analyse the performance of the system parameters. It was found that
the controlled set point fixed within the range of pH 7.6-8.4, temperature 25-29.44
Celsius and water level of 20cm in this research that was effectively achieved in the
entire test conducted. In addition, the wastewater system accuracy and performance
is 96.72% and 90.22% respectively.
Lightning is among the most deadly natural phenomena to mankind. This phenomenon
is seen to increase globally as well as in Malaysia. Lightning does strike open areas
such as playing fields and playgrounds and these areas are places people gather.
Sensors that can detect the early occurrence of lightning have been developed for
detecting approaching lightning activity in this project. The main objective is to provide
early lightning warning system to the public and hence to reduce the number of
fatalities due to lightning strike. The warning circuit was designed and simulated using
Multism11. Basic operational method of the circuit is based on the comparative
voltage method using LM339N integrated circuit comparator (IC). Light Emitting Diodes
(LEDs) were used as indicators to indicate if the incoming voltage level is higher or
lower than that of the safety level.
One of the concerns in power system preventive control and security assessment is to find the point where the voltage and frequency collapse and when the system forces a severe disturbance. Identifying the weakest bus in a power system is an essential aspect of planning, optimising and post-event analysing procedures. This paper proposes an approach to identify the weakest bus from the frequency security viewpoint. The transient frequency deviation index for the individual buses is used as the weakest bus identification as well as a frequency security indicator. This approach will help to determine the bus with the worst deviation, which helps to analyse the system disturbance, takes proper control action to prevent frequency failure, and most importantly, observes consumer frequency. The approach is applied to the WSCC 9 bus test system to show the feasibility of the method.
Combining Mel Frequency Cepstral Coefficient with wavelet transform for feature extraction is not new. This paper proposes a new architecture to help in increasing the accuracy of speaker recognition compared with conventional architecture. In conventional speaker model, the voice will undergo noise elimination first before feature extraction. The proposed architecture however, will extract the features and eliminate noise simultaneously. The MFCC is used to extract the voice features while wavelet de-noising technique is used to eliminate the noise contained in the speech signals. Thus, the new architecture achieves two outcomes in one single process: ex-tracting voice feature and elimination of noise.
Due to high energy demand worldwide, finding an alternative renewable and
sustainable energy source is of great interest. Plant microbial fuel cell (P-MFC) is one
of the most promising methods to generate green energy. In P-MFC, a plant is placed
into the anode compartment. Mutual interaction between plant root rhizodeposits
and bacterial community results in the biofilm formation at the vicinity of the
rhizosphere area in plant root could be utilized to generate electricity. Indeed, in PMFC,
bacteria metabolize rhizodeposits into electrons and protons. These electrons
could be then converted into green electricity. The objectives of this research are to
utilize Epipremnum aureum plant collected from Kota Tinggi’s lake to generate
electricity and observe current generation by different resistors, to characterize
immobilized bacteria attached on the anode surface then identify the optimum growth
temperature for isolated bacteria. Five plant microbial fuel cells were constructed in a
H-shape (dual- chambers) configuration in the plastic container. Maximum current
density for 20 days for P-MFC by external resistance of 100k Ω was 0.1 µA/cm2
with
maximum power density of 0.85 µW/cm2 and the open circuit voltage (OCV) was
measured at 195 mV. Besides, fresh biomass averages increased 5g after 20 days of
experiments below and above ground as compared to the initial fresh biomass. Five
isolated bacterial strains from the graphite felt surface found on the anode were
screened by nine biochemical tests such as catalase, TSI (triple sugar iron agar), gelatin
and etc. The immobilized bacteria attached to anode electrode in P-MFC were further
examined with Fast Electron Scanning Electron Microscopy (FESEM). The isolated
bacterial growth curves were determined at two different temperatures of 25 °C and
37 °C. The optimum growth temperature predominantly for them was 37 °C.
Multimode multiband connectivity has become a de-facto requirement for smartphones with 3G
WCDMA/4G LTE applications. In transceivers, multiband operation is achieved by selecting an output from two or more signal path targeting for a specific frequency range in parallel or by using switched capacitor/inductor. In this paper, design methodology of 280nm CMOS switch is presented. Design optimization of RF CMOS switch is presented which is deciding proper selection of CMOS transistor parameters and switch size as per external circuit parameters. The CMOS switch of a 5-transistor stack with W/L=1200μm/280nm provides insertion loss < 0.6dB and isolation loss >14dB. The switches designed when implemented in a multiband power amplifier (PA) exhibits 36dB gain at 1900MHz high-band and 34.5dB gain at 900MHz low-band with 27.5dBm peak power at both bands. The switch design methodologies presented in this paper should be of use in designing various blocks in emerging multiband transceiver applications.
Scleral buckle placement is a well-established technique for the treatment of primary rhegmatogenous retinal
detachment. Complications associated with scleral buckle are uncommon and its presentations can be vary. We
report a case of recurrent orbital cellulitis with anterior segment ischemia following a forgotten episode of previous
scleral buckling surgery, presenting with blurring of vision, redness and swelling of the lids. The presence of scleral
buckle was detected by detailed examination and confirmed by orbital imaging. Orbital infection and rubeosis iridis
were successfully treated with scleral buckle removal, intravenous antibiotics and intracameral ranibizumab.
However, the retinal detachment recurred and the visual acuity deteriorated to light perception. There was no further
intervention as the family declined in view of her old age. In cases of recurrent orbital infection, detailed clinical
examination is important to look for evidence of ocular prostheses as a source of infection. Orbital imaging is an
adjunct for making the diagnosis especially in cases where history is unreliable. Anterior segment ischemia due to
scleral buckle responds well to buckle removal with ranibizumab injection.
