    Introduction: Vaccine-preventable diseases such as pertussis are re-emerging in Malaysia during recent years.

    Objective: This research aims to study the local incidence of clinical pertussis among paediatric patients admitted to Hospital Tuanku Fauziah, Perlis over two-year period.

    Method: A cross-sectional study was designed in Department of Paediatrics, Hospital Tuanku Fauziah, Perlis from 1 January 2013 till 31 January 2015. Data were collected from medical and laboratory record of cases admitted for clinical pertussis. Analyses for descriptive data were done using frequency and percentage; Pearson chi-square or Fisher exact was used to test association.

    Results: 81 cases of clinical pertussis were included in the study. Out of this, there were a total of 28 confirmed cases of pertussis. There was a steady increment in the incidence of pertussis throughout the study period. Cyanosis emerged as the only clinical sign significantly associated with pertussis (p = 0.011). Majority of the confirmed pertussis cases were too young to be immunized (n = 13, 46.4%).

    Conclusion: Reappraisal of local health system to strengthen herd immunity in the community is warranted to control disease spread.

    Dengue incidence has grown dramatically in recent decades and become a global incidence especially in the
    urban area. Ministry of Health (MOH) Malaysia reported that the number of dengue cases since January
    2014 until June 2014 was 38. 411 cases which was higher compared to the number reported in year 2013 (10,
    910 cases). The objective of the study was to determine the knowledge and preventive practices of dengue
    among residents in Flat Pandan Indah, Ampang Selangor. A cross sectional study design with convenience
    sampling method was adopted in this study. A total of 298 respondents living in Flat Pandan Indah were
    recruited based on the inclusion criteria. A validated questionnaire that measures knowledge and preventive
    practices of dengue was completed by respondents. More than half of the respondents had scored 61.7%
    with good knowledge regarding dengue. However, 50.3% of the respondents had a limited extent of dengue
    practice. Almost all of the respondents (95.6%) reported that television or radio as the main source for
    information on dengue. The common preventive measures used by the respondents were cleaning the
    house (n=148, 49.7%), eliminating standing water around the house (n=123, 41.3%) and covering water
    containers in the home (n=120, 40.a3%). There was no correlation between dengue knowledge and
    preventive practice score (p = 0.109). The study revealed that majority of the respondents had good
    knowledge regarding dengue but the practices to prevent dengue are poor. Thus, health education program
    focusing on dengue prevention practices should be done on the community members in the future.

    Multiple myeloma is a blood dyscrasias that accounts of almost 10% of all hematological malignancy. The presentation of myeloma kidney is highly variable and it often presents as renal insufficiency, renal tubular dysfunction and proteinuria of various types. In Malaysia the true incidence of myeloma kidney is unknown. Often the diagnosis of myeloma kidney was missed out despite the patient has sought medical treatment early. A high index of suspicion is required when the middle to elderly age patients present with unexplained renal impairment and enlarged kidneys. We present here the presentation of a rare subtype of myeloma in a relatively young patient whereby the patient presented with nephrotic syndrome and aoztemia.

    BACKGROUND: Epidemiologic measures of the dengue burden such as prevalence and incidence are important for policy-making and monitoring the progress of disease control. It is a common practice where epidemiologic and economic research estimate dengue burden based on notification data. However, a basic challenge in estimating the incidence of dengue is that a significant proportion of infected population are asymptomatic. It can be overcome by using mathematical models that relate observed prevalence and mortality to incidence. In this study, we estimate the trend of dengue incidence and hospitalization in Malaysia.

    METHODS: This study is based entirely on the available secondary data sources on dengue in Malaysia. The age-specific incidence of dengue between 2001 and 2013 was estimated using the prevalence and mortality estimates in an incidence-prevalence-mortality (IPM) model. Data on dengue prevalence were extracted from six sero-surveys conducted in Malaysia between 2001 and 2013; while statistics on dengue notification and Case Fatality Rate were derived from National Dengue Surveillance System. Dengue hospitalization data for the years 2009 to 2013 were extracted from the Health Informatics Centre and the volumes of dengue hospitalization for hospitals with missing data were estimated with Poisson models.

    RESULTS: The dengue incidence in Malaysia varied from 69.9 to 93.4 per 1000 population (pkp) between 2001 and 2013.The temporal trend in incidence rate was decreasing since 2001. It has been reducing at an average rate of 2.57 pkp per year from 2001 to 2013 (p = 0.011). The age-specific incidence of dengue decreased steadily with dengue incidence reaching zero by age > 70 years. Dengue notification rate has remained stable since 2001 and the number of notified cases each year was only a small fraction of the incident cases (0.7 to 2.3%). Similarly, the dengue hospitalization was larger but still a small fraction of the incident cases (3.0 to 5.6%).

    CONCLUSION: Dengue incidence can be estimated with the use of sero-prevalence surveys and mortality data. This study highlights a reducing trend of dengue incidence in Malaysia and demonstrates the discrepancy between true dengue disease burden and cases reported by national surveillance system. Sero-prevalence studies with representative samples should be conducted regularly to allow better estimation of dengue burden in Malaysia.

    A total of 39 titles related to rheumatic fever or rheumatic heart disease in Malaysia were found with online literature search dating back to their inceptions and through 2014. Additional publications from conference journals were included. Nine papers were selected based on clinical relevance and future research implications. There were no population-based studies on the incidence or prevalence of ARF or RHD. In the 1980s, the incidence of admission due to ARF ranged from 2 to 21.1 per 100 000 paediatric admission per year. The burden of disease was significant in the adult population; 74.5% of patients with RHD were female, of which 77.1% were in the reproductive age group of 15-45 years old. Rheumatic mitral valve disease constituted almost half (46.7%) of all mitral valve repairs, ranging from 44.8 - 55.8 patients per year from 1997 - 2003. From 2010-2012, mitral valve interventions increased to 184 per year, of which 85.7% were mitral valve repair. In children with ARF, 25.4% - 41.7% had past history of rheumatic fever or RHD. In patients with rheumatic mitral valve disease undergoing surgical or medical interventions, only 6% reported history of ARF, none had history of GABHS pharyngitis or antibiotic prophylaxis. Only 44.7% of patients with RHD on follow-up were on intramuscular benzathine penicillin prophylaxis. Overall, there is scarcity of publications on ARF and RHD in Malaysia. Priority areas for research include determination of the incidence and prevalence of ARF and RHD, identification of high-risk populations, evaluation on the implementation and adherence of secondary preventive measures, identification of subclinical RHD especially amongst the high-risk population, and a surveillance system to monitor and evaluate preventive measures, disease progression and outcomes.

    Medication errors could bring serious consequences to patients. Reporting medication error is a strategy to
    mitigate such incidence from happening. Unfortunately, some nurses do no report the errors due to certain
    factors. Determining the factors influencing unreported medication errors will ensure imperative actions
    that are to be taken to curb this issue. The aim of this study is to determine the prevalence and perceived
    causes of unreported medication errors among nurses in a public hospital in Selangor. A descriptive and
    cross-sectional study was carried out in 26 wards from various disciplines and the sample involved 234
    nurses. The data were gathered through self-reported questionnaires consisting of three sections. The first
    section covered demographic characteristics, the second section aimed to obtain information on the
    frequency of medication error incidents and the last section aimed to obtain information on nurses'
    perceptions of barriers in reporting medication errors. The findings of this study indicated that there was a
    significant relationship between level of education and the nurses' perceptions of barriers in reporting
    medication errors. The study recommended that providing enough education, initiating a non-punitive
    culture may help increase voluntary reporting of medication errors among nurses to strengthen the
    reporting system and to avert medication errors in the future.

    Background: The literature contains diverse and sometimes contradicting results about wound seroma following thyroidectomy. This is probably due to the subjective clinical estimation of seroma, or due to failure to differentiate between the occurrence of subcutaneous (SC) and deep wound collections. This work aimed at objectively investigating the factors affecting subcutaneous and deep wound seroma after thyroidectomy.

    Methods: The relation between various operative and clinico-pathological factors and the collection formation was prospectively analyzed in a cohort of 100 patients after conventional thyroidectomy. Wound seroma was assessed clinically and via high-resolution ultrasonography at 24 h, 48 h and two weeks postoperatively. Sonographically detected collections were expressed as SC and/or deep wound collections according to the relation to strap muscles.

    Results: Operative duration was the only independent factor significantly affecting the incidence of clinical seroma. Older patients (>40ys) showed significantly larger volumes of early SC collections. Early postoperative pain was significantly related to drain insertion, to the occurrence of clinical seroma and to the volume of SC collections.Sonographically, suction drains and shorter operative durations resulted in significantly less amount of deep collections. Suction drains did not result in less amount of SC collections or in a lower incidence of clinical seroma.

    Conclusions: Operative duration is the only independent factor significantly related to clinically-detected postoperative seroma with its subsequent postoperative pain. Especially in elderly patients, a flapless technique would be recommended as these patients developed larger volumes of SC collections with subsequent higher pain scores, even if seroma was not clinically detected.

    Epidemiological evidence suggests that the incidence of ischaemic stroke in young adults (18-50 years) has increased substantially. These patients have a long life expectancy after stroke, and the costs of long-term care pose huge challenges to health-care systems. Although the current recommendations for treatment of young and old (>50 years) patients with stroke are similar, the optimal management of young adult patients with stroke is unknown. They are usually not included in trials, and specific subanalyses limited to young adult patients with stroke are usually not done, owing to lower incidence of stroke and lower prevalence of vascular risk factors in young adults. Progress has been made in identifying patients with a considerable risk of stroke occurrence, such as those with patent foramen ovale. Future prevention studies might result in a decrease in the incidence of stroke and its sequelae in young adults. The development of guidelines specifically devoted to the management of stroke in young adults will be an important step in achieving this aim.

    BACKGROUND: Postzygotic de novo mutations lead to the phenomenon of gene mosaicism. The 3 main types are called somatic, gonadal, and gonosomal mosaicism, which differ in terms of the body distribution of postzygotic mutations. Mosaicism has been reported occasionally in patients with primary immunodeficiency diseases (PIDs) since the early 1990s, but its real involvement has not been systematically addressed.

    OBJECTIVE: We sought to investigate the incidence of gene mosaicism in patients with PIDs.

    METHODS: The amplicon-based deep sequencing method was used in the 3 parts of the study that establish (1) the allele frequency of germline variants (n = 100), (2) the incidence of parental gonosomal mosaicism in families with PIDs with de novo mutations (n = 92), and (3) the incidence of mosaicism in families with PIDs with moderate-to-high suspicion of gene mosaicism (n = 36). Additional investigations evaluated body distribution of postzygotic mutations, their stability over time, and their characteristics.

    RESULTS: The range of allele frequency (44.1% to 55.6%) was established for germline variants. Those with minor allele frequencies of less than 44.1% were assumed to be postzygotic. Mosaicism was detected in 30 (23.4%) of 128 families with PIDs, with a variable minor allele frequency (0.8% to 40.5%). Parental gonosomal mosaicism was detected in 6 (6.5%) of 92 families with de novo mutations, and a high incidence of mosaicism (63.9%) was detected among families with moderate-to-high suspicion of gene mosaicism. In most analyzed cases mosaicism was found to be both uniformly distributed and stable over time.

    CONCLUSION: This study represents the largest performed to date to investigate mosaicism in patients with PIDs, revealing that it affects approximately 25% of enrolled families. Our results might have serious consequences regarding treatment and genetic counseling and reinforce the use of next-generation sequencing-based methods in the routine analyses of PIDs.

    Objective: This study aims to determine the incidence rate of phlebitis among patients with peripheral intravenous catheter. Methods: An observational study was conducted in one of the hospitals in East Coast Malaysia. There were 321 data collected among patients who had peripheral intravenous catheter in medical, gynecology and orthopedic wards. The incidence of phlebitis was evaluated using modified Visual Infusion Phlebitis score checklist. Results: The incidence of phlebitis, was found out to be 36.1% (n=116/321). Most patients who developed phlebitis had visual infusion phlebitis, with a score of two (34.9%) and the rest developed phlebitis with a score of three (1.2%). Conclusion: This high incidence of phlebitis indicated a worrying outcome. Therefore, the study findings suggested that a specific guideline on post insertion management of peripheral intravenous catheter should be revised which may help in reducing more incidence of phlebitis, subsequently reduce infection in ward, and provide more safety environment in hospital and reducing cost in managing infection control.

    Amoebiasis is a parasitic infection caused by the
    intestinal protozoan Entamoeba histolytica, most
    prevalent in developing countries. It results in 40,000 to
    100,000 deaths each year from amoebic colitis and extra
    intestinal infections. Amoebic liver abscess (ALA)
    is the most common extra intestinal site of infection
    with an incidence of between 3% and 9% of all cases of
    amoebiasis. Ultrasound which has a sensitivity of more
    than 90% for detecting ALA is highly recommended
    as an initial investigation followed by serological
    demonstration of circulating antibodies specific to
    Entamoeba histolytica.

    Background: Despite regularly participating in international and national level multisport events, there is still
    limited data on the pattern of injuries and illnesses and factors associated with injuries and illnesses in Malaysia.
    Such information is crucial to instil preventive measures because sustaining injuries during competition could
    hamper the athlete’s performance.

    Methods: This cross-sectional study investigated the incidence, patterns and risk factors of injuries and illnesses
    among athletes throughout SUKIPT 2018, from the 2nd to the 10th of February 2018. All injuries and illnesses
    treated by tournament medical personnel were reported using a standardised online injury reporting form.
    This form was adapted from the injury surveillance form used by the International Olympic Committee.

    Results: A total of 6071 athletes from 80 contingents took part in SUKIPT 2018. During the nine days of
    competition 323 injuries and 48 illnesses were reported, resulting in an incidence of 5.3 injuries and 0.8 illness
    per 100 athletes. Approximately 6% of the athletes sustained at least one injury or illness.

    Conclusion: In summary, the incidence of injuries and illnesses among athletes during SUKIPT was 5.3 and
    0.8 per 100 athletes, respectively. Muscle strain/rupture/tear was the most common pattern of injury while
    collision with another athlete was the most frequent mechanism of injury. Meanwhile, the respiratory system
    was the most commonly affected by illness and infection was the most prevalent cause of illness.

    This article provides violent crime data in Malaysia from 2006 to 2017. The violent crimes include murder, rape, gang robbery, robbery and voluntarily causing hurt cases. A total of 330,395 violent crime cases were reported in this 12 year period and the data were tabulated state by state for all thirteen states of Malaysia, including two states in Borneo (Sabah and Sarawak) and one federal territory (Kuala Lumpur). In general, violent crimes show a decreasing trend from 2006 to 2017 in Malaysia. However, armed gang robbery and armed robbery show a fluctuating pattern from 2008 to 2011. A similar pattern was also recorded for unarmed gang robbery from 2008 to 2010. The violent crime data deposited here are available for further analysis, e.g., for identifying risk factors such as demography, lifestyle, socio-economic status, government policies etc. which may be associated with violent crime incidence and pattern across the country.

    Harmful algal blooms (HABs) events have been increasingly reported in the country, not only of the frequency and severity of the events, but also involved more species than previously known. In this paper, a decadal review of HABs events in Malaysia is summarized. Bloom events caused by harmful dinoflagellate species including the shellfish poisoning events were highlighted. Paralytic shellfish poisoning (PSP) is no longer restricted to Sabah coasts and Pyrodnium bahamense. Bloom of Alexandrium minutum was reported for the first time in the Peninsula with six persons hospitalized including one casualty after consuming the contaminated benthic clams. Algal blooms that are associated with incidence of massive fish kills have been reported from both east and west coasts of the Peninsula in conjunction to finfish mariculture loses. The culprits of these bloom events have been identified as the dinoflagellates, Cochlodinium polykrikoides, Neoceratium furca, Prorocentrum minimum, Noctiluca scintillans and a raphidophyte, Chatonella ovata. In this paper, some of these HABs species were characterized morphologically and genetically, including their toxicity. Therefore, with the increase of coastal utilization and eutrophication, prevention, management and mitigation strategies, such as site selection, moving pens, clay spraying should be adopted to minimize the impact of these natural events.

    The determination of the high-risk area and clusters of typhoid cases is critical in typhoid control. The purpose of this study was to identify and describe the epidemiology and spatial distribution of typhoid in four selected districts in Kelantan using GIS (geographical information system). A total of 1215 (99%) of the cases were coordinated with GPS (global positioning system) and mapping was done using ArcGIS 9.2. Spatial analysis was performed to determine the cluster and high-risk area of typhoid. Results showed that typhoid incidence was not associated with race and sex. Most affected were from the age group of 5-14 followed by 15-24 year olds. Nine sub-districts were categorized as highly endemic. In addition typhoid has shown a significant tendency to cluster and a total of 22 hotspots were found in Kota Bharu, Bachok and Tumpat with a few sub districts identified as high risk for typhoid. No significant relationships between the treated water ratio and flood risk area were found with the cluster of cases. The cluster of typhoid cases in the endemic area did not appear to be related to environmental risk factors. Understanding the characteristics of these clusters would enable the prevention of typhoid disease in the future.

    In Malaysia, the incidence of Dengue Fever (DF) and Dengue Hemorrhagic Fever (DHF) have risen dramatically in the last twenty years. With the use of Geographical Information System an explanation for the spread and control of these diseases can be obtained. This study aims to develop a spatial modeling that can predict the risks for DF and DHF based on environmental factors such as physical surroundings, land use, rainfall, temperature and GIS application using logistic regression. A total of 16 variables were used in the process of spatial modeling development. At the significant level of 0.05, the results of logistic regression showed that only 10 out of 16 significant variables in the modeling process. The accuracy of the resulting model is 70.3%. A crucial feature of this study is a risk area map for incidence of DF and DHF in the study area. This study also highlights the application of spatial analysis in planning and implementing the process for the prevention and control activities of DF and DHF in Malaysia.

    Hypertensive disorders of pregnancy is a major cause of fetal and maternal morbidity. Epidemiologic observations have shown an inverse association between calcium intake and incidence of eclampsia. Calcium supplementation has been reported to decrease blood pressure in pregnant women, although this remains controversial. The mechanism responsible for the effect of calcium on hypertinsion in pregnant women is unknown. This paper is a review of some of the recent calcium supplementation trials that have been carried out and their effectiveness in reducing systolic and diastolic blood pressure and decreasing the incidence of hypertensive disorders of pregnancy.

    Malaysia was ranked at 148 by World Health Organization (WHO) in suicide statistic for 2012, where males had a higher
    tendency to commit suicide, recording a 4.7 per 100,000 in comparison to female (1.5 per 100,000). As hanging is one of
    the most common methods to commit suicide, this retrospective study was conducted to observe the incidence of suicide
    by hanging in Klang Valley from four different hospitals. The current study is conducted to provide local database of
    suicide occurrence by hanging in Klang Valley. A total of 893 suicide cases by hanging from 2007 to 2016 were recorded
    from post mortem records and analysed. Suicide rates in Klang Valley showed a rising trend from 2007 to 2016. Males
    from age group 30 to 34 years old and females from 20 to 24 years old have the highest number of suicide fatalities
    compared to other age groups. Meanwhile, fatalities from the age group of 10 to 14 years old and 80 years old and above
    showed similar result which is only 7%. 78% of the fatalities were recognized as Malaysian and half of them were Indian.
    Majority of the victims/cases (45%) who committed suicide by hanging are of Hindu religious denomination. Non-citizen
    in Malaysia that have the highest suicide rates were from Nepal, Indonesia and Myanmar. Victims also committed suicide
    more during the day and the peak hours being at 12.00 pm to 5.59 pm.

    Introduction: Football and futsal were the main sports in the Malaysian Games. However, they were associated with a risk of injury. The purpose of the study was to analyse the incidence, circumstances, and characteristics of football and futsal injuries during the Malaysian Games of 2018.

    Materials and method: During the tournament, 14 teams participated in men's football, 12 teams in men's futsal and 11 teams in women's futsal. The biannual event involved athletes aged under 21 years. A medical report form used by FIFA Medical Assessment and Research Centre (F-Marc), was provided to the physiotherapists and team doctors of all the teams to report all injuries after each match.

    Results: The response rate was 84.62% in football and 59.76% in futsal. A total of 48 injuries were reported from 26 football matches, equivalent to 64.64 injuries per 1000 match hours (95%CI 46.35 to 82.93). In futsal, a total of 48 injuries from 41 matches were reported, equivalent to 292.42 injuries per 1000 match hours (95% CI 209.7 to 375.14). The rate of injury in women futsal players was higher compared to men: 358.21 versus 247.04 injuries per 1000 match hours (p=0.224). Futsal recorded higher injuries per 1000 match hours than football (p<0.001).

    Conclusion: The rate of severe injury in futsal and football recorded in the study as compared to previous studies gave rise to serious concerns. Hence, there was an urgent need to pay more attention to injury prevention strategies.
