The expression « To run amok » describes bouts of murderous madness. This clinical entity was originally described in the Malay Peninsula. Associated with a form of exoticism, literary and historical descriptions are available. For about twenty years, amok has been included in psychiatric classifications as a culture-bound syndrome. In the last quarter of the 20th century, some parallels were observed between amok and mass shootings. Although the weapon of choice has changed from dagger to gun, common traits are clearly identifiable. Anger, feelings of prejudice and paranoid thoughts -dominate the picture. The absence of an ideological claim distinguishes the mass shooter from the terrorist. The psychiatrist may have a role in prevention, but these acting outs -remain difficult to anticipate.

Objectives: To study the relationship between psychopathology and characteristics of offenders with schizophrenia. Methods: In this cross sectional study, 70 consecutive offenders with schizophrenia (aged 18-65) admitted to Hospital Bahagia Ulu Kinta within a six-month period were studied. Subjects’ psychopathology was assessed using Positive and Negative Syndrome Scale (PANSS). Results: Offenders against person group received treatment at later age (p=0.043) compared to against property and drug or firearm-related offenders groups. Offenders in drug or firearm-related group had significantly higher PANSS negative scores (p=0.015). Unsound mind at the time of offense was significantly associated with high PANSS positive scores (p=0.011) and offenders against person or property groups (p=0.004). Conclusion: Offenders against person had a significantly later age of treatment and probably onset of illness. Unsoundness of mind was significantly associated with positive symptoms and more frequently reported among offenders against person or property. Offenders in drug or firearmrelated
group were significantly associated with negative symptoms.

BACKGROUND: Selective mutism (SM) is a childhood onset anxiety disorder, and the main symptom is not speaking in certain social situations. Knowledge about the duration and long-term outcomes of SM have been lacking and the aim of this systematic literature review was to address this gap in the literature. We investigated how long SM symptoms persisted as well as other psychiatric outcomes associated with SM in later life.

METHODS: The PubMed, PsycInfo, Web of Science, Cochrane Library and Embase databases were initially searched from inception to 11 September 2023. Studies were included if they were published in English and had followed up subjects with clinically diagnosed SM for at least two years. The review followed the Preferred Reporting Items of Systematic Reviews and Meta-analyses guidelines and the protocol was registered with the Open Science Framework. The papers were assessed using the Quality Assessment with Diverse Studies tool.

RESULTS: This review screened 2,432 papers and assessed 18 studies. Seven case series studies were excluded from discussion because of the low number of subjects and the fact that their findings could not be generalized to wider populations. In the end, nine clinical cohorts and two case control studies were reviewed. These provided a total of 292 subjects and the sample sizes ranged from 11-49. The overall quality of the studies was moderate. The review found that 190 of the 243 subjects in the studies that reported recovery rates showed moderate or total improvement from SM during follow up. Other anxiety disorders were the most common psychiatric disorders later in life, although these results should be interpreted with caution. Older age at baseline and parental psychopathology might predict greater impairment, but further studies are needed to confirm these results.

CONCLUSIONS: Most subjects with SM recovered from this disorder during adolescence, but anxiety disorders were common in later life. Early detection and treatment are needed to prevent symptoms from persisting and other psychiatric disorders from developing.

Objective: To study the demographic data of patients and the pattern of crime in the study group, to look at the outcome of assessment by psychiatrist on fitness to plea and insanity at time of offense and to assess association between the nature of crime with presence of psychopathology during the crime. Method: This is a retrospective study. All available case notes for 342 forensic admissions from January 2007 until March 2010 were reviewed. All related information on 135 patients that committed violent crime was taken for analysis. Result: Majority of patients were male, Malay, single, education up to secondary school and unemployed. Only 38% of patients had encountered psychiatric services, 20% had previous imprisonment and 49% had history of substance usage. In cases involving victims (87%), 64% the victims were known to the patient, 53% had hallucination or delusion or both at the time of crime and 90% was found to have some diagnosis of mental illness after psychiatrist assessment. Only 81% of formal forensic reports were available in the case note and it showed 94% were fit to plea and 82% were at sound mind at the time of offense. Conclusion: Substance abuse had high prevalence among the patient. Almost 2/3 never had any encounter with psychiatric services before the admission. Even with the high percentage of patient diagnosed with mental illness, they were still found to be fit to plea in court and are at sound mind at the time of offense

Given the increased popularity and use of Complementary and Alternative Medicine (CAM) in the treatment of psychiatric disorders, it is crucial for mental health professionals to be informed about this treatment approach. Accordingly, this paper critically reviews the pro and con arguments for the use of CAM treatment modalities in the treatment of psychiatric disorders. Given that there are potential dangers in adopting the CAM approach without appropriate supervision, it begs the question of what roles and responsibilities mental health professionals have in the matter. Thus, this article proceeds to address the roles and the recommended strategies for mental health professionals when working with patients or clients who use CAM for their treatment of psychopathological issues. Finally, it goes further to discuss the practice of integrating both conventional treatment and CAM to treat mental disorders and issues associated with such disorders. Importantly, this article highlights the need for knowledgeable, confident, and trained mental health professionals to provide patient counselling about the complementary and alternative approach.

Introduction: A number of researches suggest smoking serves as a form of self-medication to reduce the side effects of antipsychotic medications, to alleviate negative symptoms, and/or to ameliorate a number of cognitive deficits associated with schizophrenia. Objective: The aim of this study was to investigate the association of cigarette smoking with verbal working memory and psychopathology of patients with schizophrenia. Methods: Fifty-three patients with schizophrenia were assessed by a single rater using the Malay Version of Auditory Verbal Learning Test (MVAVLT) and Positive and Negative Syndrome Scale (PANSS). Smokers (n=30) were compared with nonsmokers (n=23) on socio-demographic, clinical, psychopathology and verbal memory variables. Single linear and multiple regression analysis were performed to determine factors associated with verbal memory performance.
Results: Verbal working memory performance is associated with lower number of admission to ward, lesser severity of the negative symptoms or general psychopathology of schizophrenia and use of atypical antipsychotics in all schizophrenic subjects. Smokers with schizophrenia scored higher than non- smoker in measures that reflect immediate memory, delayed recall and recognition memory. However, the association between verbal working
memory performance and smoking status was found to be not significant. Conclusion: Verbal working memory performance is associated with negative symptoms but not positive symptoms. This study failed to detect association of smoking on verbal working memory.

For a Malaysian sample, the current study used exploratory factor analysis (EFA) to determine the best model for parent ratings of the Strengths and Difficulties Questionnaire (SDQ), and then multiple-group confirmatory factor analysis (MCFA) to confirm this model, and to examine measurement invariance across different language versions (Malay and English), child's gender (boys and girls), informants (mothers and fathers), and racial groups (Malay, Chinese, and Indians). In all 1,407 Malaysian parents completed SDQ ratings of their children (age ranging from 5 to 13 years). The EFA showed most support for a two-factor model oblique model, with factors for a positive construal factor and a psychopathology factor. CFA confirmed this model, and MCFA showed full measurement invariance (configural, metric. and scalar) across the groups in the different comparisons. For all comparisons, there were equivalencies for latent mean scores. The implications of the findings for clinical and research practice involving the SDQ in Malaysia are discussed.

BACKGROUND: An estimated 1 in 5 adolescents experience a mental health disorder each year; yet because of barriers to accessing and seeking care, most remain undiagnosed and untreated. Furthermore, the early emergence of psychopathology contributes to a lifelong course of challenges across a broad set of functional domains, so addressing this early in the life course is essential. With increasing digital connectivity, including in low- and middle-income countries, digital health technologies are considered promising for addressing mental health among adolescents and young people. In recent years, a growing number of digital health interventions, including more than 2 million web-based mental health apps, have been developed to address a range of mental health issues.

OBJECTIVE: This review aims to synthesize the current evidence on digital health interventions targeting adolescents and young people with mental health conditions, aged between 10-24 years, with a focus on effectiveness, cost-effectiveness, and generalizability to low-resource settings (eg, low- and middle-income countries).

METHODS: We searched MEDLINE, PubMed, PsycINFO, and Cochrane databases between January 2010 and June 2020 for systematic reviews and meta-analyses on digital mental health interventions targeting adolescents and young people aged between 10-24 years. Two authors independently screened the studies, extracted data, and assessed the quality of the reviews.

RESULTS: In this systematic overview, we included 18 systematic reviews and meta-analyses. We found evidence on the effectiveness of computerized cognitive behavioral therapy on anxiety and depression, whereas the effectiveness of other digital mental health interventions remains inconclusive. Interventions with an in-person element with a professional, peer, or parent were associated with greater effectiveness, adherence, and lower dropout than fully automatized or self-administered interventions. Despite the proposed utility of digital interventions for increasing accessibility of treatment across settings, no study has reported sample-specific metrics of social context (eg, socioeconomic background) or focused on low-resource settings.

CONCLUSIONS: Although digital interventions for mental health can be effective for both supplementing and supplanting traditional mental health treatment, only a small proportion of existing digital platforms are evidence based. Furthermore, their cost-effectiveness and effectiveness, including in low- and middle-income countries, have been understudied. Widespread adoption and scale-up of digital mental health interventions, especially in settings with limited resources for health, will require more rigorous and consistent demonstrations of effectiveness and cost-effectiveness vis-à-vis the type of service provided, target population, and the current standard of care.
