    Vitiligo is an acquired cutaneous disorder of pigmentation, with an incidence of 0.5% to 2% worldwide. There are three major hypotheses for the pathogenesis of vitiligo that are not exclusive of each other: biochemical/cytotoxic, neural and autoimmune. Recent data provide strong evidence supporting an autoimmune pathogenesis of vitiligo. As vitiligo can have a major effect on quality of life, treatment can be considered and should preferably begin early when then disease is active. Current treatment modalities are directed towards stopping progression of the disease and achieving repigmentation. Therapies include corticosteroids, topical immunomodulators, photo(chemo)therapy, surgery, combination therapies and depigmentation of normally pigmented skin. It seems that traditional Chinese medicine could be more effective than the current treatment for vitligo.

    Psoralen and ultraviolet light A (PUVA) was the mainstay of vitiligo treatment before the introduction of narrowband UVB(NB-UVB). Eighteen patients treated with NB-UVB in Hospital Kuala Lumpur were retrospectively analyzed. Eight patients had Fitzpatrick skin phototype III and 10 with phototype IV. The mean duration of disease was 7.3 +/- 5.4 years, and the mean body surface area affected was 19.3 +/- 16.2%. Mean duration of treatment was 14.3 +/- 9.6 months, number of session 113.5 +/- 88.6 times and cumulative dose 111.7 +/- 108.5 J/ cm2. Concomitant topical steroid was used in 88.9% patients. Repigmentation was moderate in 6 (40%) patients, good in 3 (20%), while 3 (20%) achieved excellent results. One patient (6.6%) showed no response and 2 (13.3%) showed poor repigmentation. Three patients developed side effects. NB-UVB phototherapy results in satisfactory repigmentation in our vitiligo patients and should be offered as a treatment option.

    Vitiligo is a cutaneous pigmentary disorder characterized by depigmented macules and patches that result from loss of epidermal melanocytes. Physician evaluates the efficacy of treatment by comparing the extent of vitiligo lesions before and after treatment based on the overall visual impression of the treatment response. This method is called the physician's global assessment (PGA) which is subjective. In this article, we present an innovative digital image processing method to determine vitiligo lesion area in an objective manner.

    An objective tool to quantify treatment response in vitiligo is currently lacking. This study aimed to objectively evaluate the treatment response in vitiligo by using a computerised digital imaging analysis system (C-DIAS) and to compare it with the physician's global assessment (PGA). Tacrolimus ointment 0.1% (Protopic; Astellas Pharma Tech,Toyama, Japan) was applied twice daily on selected lesions which were photographed every 6 weeks for 24 weeks. The primary efficacy end-point was the mean percentage of repigmentation (MPR), as assessed by the digital method (MPR-C-DIAS) or by the PGA. The response was categorised into none (0%), mild (1-25%), moderate (26-50%), good (51-75%) and excellent (76-100%).

    Vitiligo is a chronic disease and can have significant impairment on patients' quality of life (QoL). Studies have shown that limitations in QoL are similar in magnitude to patients with other chronic skin diseases like psoriasis. The objective of this study was to assess the quality of life (QoL) of patients with vitiligo attending the dermatology clinic of a tertiary hospital in Malaysia.

    In this paper, we describe an image processing scheme to analyze and determine areas of skin that have undergone repigmentation in particular, during the treatment of vitiligo. In vitiligo cases, areas of skin become pale or white due to the lack of skin pigment called melanin. Vitiligo treatment causes skin repigmentation resulting in a normal skin color. However, it is difficult to determine and quantify the amount of repigmentation visually during treatment because the repigmentation progress is slow and moreover changes in skin color can only be discerned over a longer time frame typically 6 months. Here, we develop a digital image analysis scheme that can identify and determine vitiligo skin areas and repigmentation progression on a shorter time period. The technique is based on principal component analysis and independent component analysis which converts the RGB skin image into a skin image that represent skin areas due to melanin and haemoglobin only, followed by segmentation process. Vitiligo skin lesions are identified as skin areas that lack melanin (non-melanin areas). In the initial studies of 4 patients, the method has been able to quantify repigmentation in vitiligo lesion. Hence it is now possible to determine repigmentation progression objectively and treatment efficacy on a shorter time cycle.

    This paper describes an image analysis technique that objectively measures skin repigmentation for the assessment of therapeutic response in vitiligo treatments. Skin pigment disorders due to the abnormality of melanin production, such as vitiligo, cause irregular pale patches of skin. The therapeutic response to treatment is repigmentation of the skin. However the repigmentation process is very slow and is only observable after a few months of treatment. Currently, there is no objective method to assess the therapeutic response of skin pigment disorder treatment, particularly for vitiligo treatment. In this work, we apply principal component analysis followed by independent component analysis to represent digital skin images in terms of melanin and haemoglobin composition respectively. Vitiligo skin areas are identified as skin areas that lack melanin (non-melanin areas). Results obtained using the technique have been verified by dermatologists. Based on 20 patients, the proposed technique effectively monitored the progression of repigmentation over a shorter time period of six weeks and can thus be used to evaluate treatment efficacy objectively and more effectively.

    Introduction: Since January 2010, the UMS Polyclinic, situated at Riverside Plaza, Kingfisher Park, Kota Kinabalu, Sabah, provided general medical consultation as well as specialist consultations comprising of Internal Medicine, Endocrinology (Diabetes and related problems), Dermatology, Rheumatology, Cardiology including ECG and ECHO services, Orthopedic, Ophthalmology, ENT (Ear, Nose and Throat), Obstetrics and Gynecology, Pediatrics and Psychiatry. The purpose of this study was to look into the Dermatology consultations provided by the UMS Polyclinic in order to improve health care services, by reporting on the demographic characteristics of the patients with reference to the provisional diagnosis and management offered, as well as referrals to tertiary health care centers for further managements. Methods: It was a retrospective, descriptive study which included 76 patients who visited the Dermatology Specialist Clinic at UMS Polyclinic from January to December 2018.
    Results: The 76 patients comprised of 50 males and 26 female patients with the age ranged from 2 years to 86 years, with 5 children (under the age of 12 years).The catchment population of the patients were mainly from Universiti Malaysia Sabah, Kingfisher, and also from wide areas of Kota Kinabalu with a few from Kudat,Tamparuli, Telipok, Keningau, Kinarut and Beaufort, etc. The consultations comprised of 19 varieties of dermatological problems to which were given appropraite management. The period of the year 2018 under study showed highest attendance in February with 21 patients (27.6%) followed by 13 patients (17.1%) in October with December showing 11 patients (14.5% ).
    Conclusion: A total of 32 varieties of Dermatological problems were found among the 76 patients consulted. Acne Vulgaris was found to be the highest variant with 12 cases(15.7%) with Seborrhoeic Dermatitis seen as 5 cases (6.5%) followed by 4 cases (5.2%) each of Contact Dermatitis, Allergy/Hypersensitivity, Photosensitive Dermatitis, and Vitiligo. Certain patients had to be referred to Dermatology Department at Hospital Queen Elizabeth for further management, like for skin biopsy, and also for Electro-cautery machine, Phototherapy, and Laser, as we are still waiting for few equipments.
