    INTRODUCTION: Despite limited clinical evidence, platelet-rich plasma (PRP) is currently used for the treatment of various soft tissue injuries, but optimal use of PRP has yet to be determined. In many instances, PRP is prepared using commercial devices that lack standardized preparation techniques and consistent quality of the PRP produced.

    OBJECTIVE: The aim of this study is to explore a simple, easy, economical method of PRP preparation that is practical for clinical use.

    MATERIALS AND METHODS: This cross-sectional study was conducted at the Sports Medicine Clinic at the University of Malaya Medical Centre, Malaysia. Participants were healthy postgraduate students and staff at the Sports Medicine Department. The PRP was prepared using a single centrifugation technique. Leukocyte and platelet levels were compared with that of a whole blood baseline and a commercial preparation kit.

    RESULTS: The PRP produced using this technique contained significantly higher mean platelet (1725.0 vs. 273.9 x 109/L) and leukocyte (33.6 vs. 7.7 x 109/L) levels compared with whole blood. There was no significant difference in the mean platelet and leukocyte levels between the PRP produced in this study and by a commercial PRP system.

    CONCLUSIONS: A single-centrifugation protocol using readily available materials in a typical clinical setting could produce PRP of comparable quality to those of a commercial PRP production system.

    This study assessed the effects of progressive resistance exercise programmes on self-reported health related quality of life, body composition and muscle strength among patients with HIV. Twenty-nine men with HIV were randomly assigned to progressive resistance exercise programme using elastic band (PRE group) (n=14) or resistance exercise programme without elastic band (comparative group) (n=15). Both groups underwent exercise programmes for 12 weeks. Selfreported health related quality of life, body compositions and isometric shoulder lift strength were assessed at baseline and at week 12. Ten and nine men in the intervention and comparative groups, respectively, completed the 12-week programme. Participants in the comparative group showed significant improvements in the domain of mental health (p<0.05). No significant changes in all body composition parameters were found in both groups with time. Both groups showed significant improvements in peak and average shoulder lift strength (intervention, p=0.001; p=0.001; control, p=0.008; p=0.016). Progressive resistance exercise programmes significantly improves shoulder lift strength, showed positive effects on self-reported health related quality of life scores among patients with HIV.

    BACKGROUND: Osteoarthritis (OA) of the knee is defined as a progressive disease of the synovial joints and is characterized by wear and tear of the cartilage and underlying bone. This study aimed to determine the short-term effects of the lower limb rehabilitation protocol (LLRP) on pain, stiffness, physical function, and body mass index (BMI) among knee OA participants who were overweight or obese. Methodology. A single-blinded randomized controlled trial of one-month duration was conducted at Rehmatul-Lil-Alameen Postgraduate Institute, Lahore, Pakistan. Fifty overweight or obese participants with knee OA were randomly divided into two groups by a computer-generated number. Participants in the rehabilitation protocol group (RPG) were provided with leaflets explaining the strengthening exercises of the LLRP and instruction of daily care (IDC), while the participants in the control group (CG) were provided with leaflets explaining the IDC only for a duration of four weeks. The primary outcome measures were the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores for pain, stiffness, and physical function. The secondary outcome measures were BMI, exercise adherence, and patients' satisfaction assessed by using the numeric rating scale ranging from 0 to 10. The paired-sample t-test was used to analyze the differences within groups from baseline to posttest evaluations. The analysis of variance 2 × 2 factor was used to analyze the differences in BMI, knee pain, stiffness, and physical function between the groups.

    RESULTS: Participants in the RPG and CG reported a statistically significant reduction in knee pain and stiffness (p ≤ 0.05) within the group. The reduction in the scores of knee pain was higher in participants in the RPG than that in participants in the CG (p=0.001). Additionally, participants in the RPG reported greater satisfaction (p=0.001) and higher self-reported exercise adherence (p=0.010) and coordinator-reported exercise adherence (p=0.046) than the participants in the CG.

    CONCLUSION: Short-term effects of the LLRP appear to reduce knee pain and stiffness only, but not physical function and BMI.

    Menstrual irregularity and disordered eating were previously identified as risk factors for low bone mineral density
    (BMD) in female athletes. Therefore, an investigation for more potential factors that could be associated with low BMD
    among Malaysian female athletes is required. The aim of this study was to assess the association of BMD with energy
    availability, body weight, body fat, estrogen level and bone loading status in female athletes and to what extent those
    factors predict BMD. Energy intake and expenditure, body weight, Bone Physical Activity Questionnaire (BPAQ) score
    and estrogen level were determined from 85 female athletes aged 18-30 years in this cross-sectional study. Bone mineral
    density was measured by using an ultrasound bone densitometry device at the calcaneus bone. The female athletes’ body
    weight and fat percentage were 56±9 kg and 27±6%, respectively and they were all eumenorrheic. Fifty three percent
    of them had low BMD (z- score < 0). Their mean energy intake (1291±33 kcal/day) was lower than energy expenditure
    (1807±34 kcal/day) which resulted in a low energy availability (29±1 kcal/day/kg FFM). BMD was positively associated
    with BPAQ score (r=0.4, p=0.000) and body weight (r=0.3, p=0.005) but negatively associated with energy expenditure
    (r=-0.4, p=0.000). There was no association of BMD with energy intake, energy availability, body fat percentage and
    estrogen level. In conclusion, BMD was associated with energy expenditure, body weight and BPAQ score among female
    athletes. Regression analysis showed that energy expenditure and BPAQ score were the main factors to predict BMD.

    Although more Malaysians are taking part in International Multisport Games, these athletes well-being at such events have not been fully explored. The purpose of this study was to examine the pattern of injury and illness among Malaysian athletes during the XVII Asian Games 2014. Clinical and socio-demographic information of athletes diagnosed with injury or illness during the centralised training camp and at the Games were recorded in a standardised report form. Throughout the study period, 83 injuries and 64 illnesses were recorded from 276 athletes. Muscle strains and tears were the most common injury followed by ligamentous injury and soft tissues contusion. The number of injuries was highest among badminton players followed by hockey and rugby. Significantly higher incidence of injuries was observed among men than women hockey players. Athletes in individual events had higher proportion of more severe injury than those in team events. Respiratory tract infection was the most frequent illness diagnosed among athletes. Most injuries and illnesses diagnosed among athletes were minor and did not result in time away from participation. The incidence of injuries and illnesses among Malaysian athletes at the XVII Asian Games were comparable with those reported by previous authors. Injury and illness rate were influenced by gender and sports. Fortunately, majority of injuries and illnesses were minor and did not prevent athletes from participation.

    INTRODUCTION: Acute muscle injury is one of the commonest injuries that often result in loss of training and competition time. The best management for muscle injury has not been identified. Sports medicine practitioners used several approaches in attempt to accelerate time to recovery from muscle injury. More recently growing interest focussed on autologous blood product injection.
    METHODS: A literature search was conducted systematically using OvidMEDLINE, PubMed, EMBASE, SPORTDiscus and CINAHL databases to retrieve articles published until December 2012. Controlled trials and controlled laboratory studies comparing different strategies to promote early recovery of muscle injury were included. The methodological quality of studies was assessed.
    RESULTS: There are limited studies on the effects of PRP therapy for muscle injury. Three in vivo laboratory studies and one pilot human study were reviewed. The laboratory studies reported histological evidence on significant acceleration of muscle healing in animals treated with autologous conditioned serum (ACS), platelet-rich plasma (PRP) and platelet rich fibrin matrix (PRFM). A pilot human study found athletes treated with repeated ACS injection recovers significantly faster than retrospective controls.
    CONCLUSION: Several in vivo laboratory studies suggest beneficial effects of ACS, PRP and PRFM in accelerating muscle recovery. Evidence to suggest similar effects on humans is however limited, as valuable information from robust human controlled trials is still not available at this moment. Hence, more studies of satisfactory methodological quality with platelet-rich plasma interventions on muscle injury are justified.

    [Purpose] The purpose of this study was to determine the reliability of the active knee extension (AKE) test among healthy adults. [Subjects] Fourteen healthy participants (10 men and 4 women) volunteered and gave informed consent. [Methods] Two raters conducted AKE tests independently with the aid of a simple and inexpensive stabilizing apparatus. Each knee was measured twice, and the AKE test was repeated one week later. [Results] The interrater reliability intraclass correlation coefficients (ICC2,1) were 0.87 for the dominant knee and 0.81 for the nondominant knee. In addition, the intrarater (test-retest) reliability ICC3,1 values range between 0.78-0.97 and 0.75-0.84 for raters 1 and 2 respectively. The percentages of agreement within 10° for AKE measurements were 93% for the dominant knee and 79% for the nondominant knee. [Conclusion] The finding suggests the current AKE test showed excellent interrater and intrarater reliability for assessing hamstring flexibility in healthy adults.

    Background: Sahrmann five-level core stability test protocol has been used to evaluate the ability of the core muscles to stabilize the spine. However, validation studies on the Sahrmann protocol are limited.

    Objective: The purpose of this study was to compare the different levels of Sahrmann five-level core stability (levels 1-5) on the muscle activity of rectus abdominis (RA), external oblique (EO), and transverse abdominis/internal oblique (TrA/IO).

    Conclusion: In conclusion, the Sahrmann five-level core stability test differs according to the level of Sahrmann tests. Significantly higher abdominal muscle activities were observed during levels 3 and 5. Therefore, the classification exchange in levels 3 and 4 of the Sahrmann five-level core stability test should be reconsidered in the future.

    OBJECTIVE: To evaluate the efficacy of dextrose prolotherapy injection for focal supraspinatus rotator cuff tendinosis via outcomes such as functional score, range of movement, and real-time ultrasound parameters.

    MATERIALS AND METHODS: 12 adult patients with focal supraspinatus tendinosis recruited after they had less than 30% improvement in functional (DASH) scores after one month of physiotherapy following initial presentation. Seven patients had 0.5-1.0 ml of prolotherapy injection (12.5% dextrose, 0.5% lignocaine) injected into the area of focal tendinosis under ultrasound guidance. Meanwhile, five patients continued standard physiotherapy with no intervention performed. Regional area of echogenicity in decibels, DASH, range of movements of the shoulder, pain, and sleep scores were measured at baseline and at 12 weeks.

    RESULTS: The prolotherapy group showed significant improvement in shoulder abduction (p=0.030) and an improvement in sleep score (p=0.027). The echogenicity of area of tendinosis significantly increases at the end of treatment (p=0.009). However, there was a nonsignificant reduction in pain score in the injection group (43.5%) and in the control group (25%) at 12 weeks (p > 0.005).

    CONCLUSION: Ultrasound-guided intratendinous prolotherapy injection significantly improves patient's range of abduction and improves sleep within 12 weeks of treatment compared to conventional physiotherapy management.

    Rhabdomyolysis is a clinical and biochemical syndrome resulting from skeletal muscle injury, which may ultimately lead to acute renal failure (ARF) and death. Exertional rhabdomyolysis refers to skeletal muscle injury that is usually induced by strenuous eccentric exercises in a hot and humid environment. It is usually seen in marathoners and military personnel. We present the case of a 32-year-old Malaysian man who had rhabdomyolysis and myoglobinuria without ARF after two episodes of unaccustomed swimming. He was treated conservatively, and recovered uneventfully. A brief discussion on the pathophysiology of rhabdomyolysis, the principles of management and recuperation is included.

    Physical absorption process is always nullified by the presence of cavitation under low frequency ultrasonic irradiation. In the present study, high frequency ultrasonic of 1.7MHz was used for the physical absorption of CO2 in a water batch system under elevated pressure. The parameters including ultrasonic power and initial feed pressure for the system have been varied from 0 to 18W and 6 to 41bar, respectively. The mass transfer coefficient has been determined via the dynamic pressure-step method. Besides, the actual ultrasonic power that transmitted to the liquid was measured based on calorimetric method prior to the absorption study. Subsequently, desorption study was conducted as a comparison with the absorption process. The mechanism for the ultrasonic assisted absorption has also been discussed. Based on the results, the mass transfer coefficient has increased with the increasing of ultrasonic power. It means that, the presence of streaming effect and the formation of liquid fountain is more favorable under high frequency ultrasonic irradiation for the absorption process. Therefore, high frequency ultrasonic irradiation is suggested to be one of the potential alternatives for the gas separation process with its promising absorption enhancement and compact design.

    A rotating packed bed (RPB) is an innovative intensification technology that improves its separation capabilities in high-gravity conditions. This process increases efficiency with smaller equipment size and footprint than conventional packed columns. Although significant advancements have been made regarding RPBs, most studies only focused on single or dual rotor configurations in addressing dry pressure drop. Hence, multiple rotor systems in industrial settings can enhance economic efficiency by minimizing the necessity for numerous RPBs. This study investigated the pressure drops and holdup in a three-stage rotor-based RPB under actual process conditions using natural gas as the feed. A novel pressure drop correlation was introduced based on the nitrogen removal process from the natural gas in continuous RPB distillation operations. Consequently, the correlation between centrifugal acceleration, turbulent, and momentum effects demonstrated remarkable accuracy within ±15%. This outcome also highlighted the importance of meticulous design considerations in RPB-based applications due to the complex correlation between centrifugal forces, liquid holdup, and gas flow rates. The reflux feed ratio, liquid holdup, rotating speed, and F-factor effects were examined to comprehend the RPB distillation process. Overall, the correlations between the critical parameters offered crucial insights to prevent process upsets (such as flooding), contributing to advancing RPBs in practical industrial settings.

    OBJECTIVES: Tualang honey (TH) contains antioxidants such as ascorbic acid, phenolic acids, and flavonoids that may be protective against oxidative stress of exercise. The aim of this study was to examine the postprandial antioxidant activity and oxidative stress after ingestion of high and low dosages of TH in female athletes.

    MATERIALS AND METHODS: Twenty female athletes (aged 21.3 [2.1] years; body weight [BW] 54.1 [5.7] kg) were randomly assigned into two groups and consumed either 1.5 g/kg BW TH (high honey; HH; n = 10) or 0.75 g/kg BW TH (low honey; LH; n = 10). Blood sample was collected at fasting and at 0.5, 1, 2, and 3 h after TH consumption. Plasma was analyzed for total phenolic content (TPC), antioxidant activity (ferric reducing antioxidant power [FRAP]), and oxidative stress biomarkers (malondialdehyde [MDA] and reactive oxygen species [ROS]).

    RESULTS: The 3-h area under the curve (AUC) for MDA was significantly lower in the LH group compared with HH group, suggesting less oxidative stress in the LH group. However, the AUCs for TPC, FRAP, and ROS were not affected by the dosages. The concentrations of TPC and FRAP increased from baseline to 2 and 1 h after TH consumption, respectively, and concentrations returned toward baseline at 3 h in both LH and HH groups.     In this study, series of non-ionic surfactants from Span and Tween are evaluated for their ability to affect the viscosity profile of cyclopentane hydrate slurry. The surfactants; Span 20, Span 40, Span 80, Tween 20, Tween 40 and Tween 80 were selected and tested to provide different hydrophilic-hydrophobic balance values and allow evaluation their solubility impact on hydrate formation and growth time. The study was performed by using a HAAKE ViscotesterTM 500 at 2 °C and a surfactant concentration ranging from 0.1 wt%-1 wt%. The solubility characteristic of the non-ionic surfactants changed the hydrate slurry in different ways with surfactants type and varying concentration. The rheological measurement suggested that oil-soluble Span surfactants was generally inhibitive to hydrate formation by extending the hydrate induction time. However, an opposite effect was observed for the Tween surfactants. On the other hand, both Span and Tween demonstrated promoting effect to accelerate hydrate growth time of cyclopentane hydrate formation. The average hydrate crystallization growth time of the blank sample was reduced by 86% and 68% by Tween and Span surfactants at 1 wt%, respectively. The findings in this study are useful to understand the rheological behavior of surfactants in hydrate slurry.

    BACKGROUND: Home-based exercise (HBE) and patient education (EDU) have been reported as beneficial additions to usual knee osteoarthritis (KOA) rehabilitation. However, previous trials mostly examined the effects of HBE and EDU separately. Thus, this study aimed to evaluate the effects of a structured combined HBE and EDU program in addition to usual KOA rehabilitation on pain score, functional mobility, and disability level.

    STUDY DESIGN: A parallel-group, single-blinded randomized controlled trial.

    METHODS: Eighty adults with KOA were randomly allocated to experimental (n=40) and control (n=40) groups. All participants underwent their usual physiotherapy care weekly for eight weeks. The experimental group received a structured HBE+EDU program to their usual care, while the control group performed home stretching exercises to equate treatment time. The Knee Injury and Osteoarthritis Outcome Score (KOOS) for the disability level, visual analogue scale (VAS) for pain, and timed up-and-go test (TUG) for mobility were measured pre-post intervention.

    RESULTS: After eight weeks, the experimental group demonstrated significant improvements in the KOOS (all subscales), pain VAS, and TUG scores compared to baseline (P<0.001); meanwhile, only KOOS (activities of daily living and sports subscales) was significant in the control group. Relative to the control, the experimental group presented higher improvements (P<0.001) by 22.2%, 44.1%, and 15.7% for KOOS, pain VAS, and TUG, respectively.

    CONCLUSION: Integrating the HBE+EDU program into usual KOA rehabilitation could reduce pain and disability, while it improved functional mobility. The finding of this study suggests a combination of a structured HBE and EDU program to be considered as part of mainstream KOA management.

    Melamine formaldehyde (MF) resins have been synthesized at different reaction temperature and pH values. Different molar ratios of melamine and formaldehyde were used to synthesize the corresponding resins. The prepared resin samples were characterized by using molecular weight determination viscometry and thermogravimetric analysis (TGA). The maximum percentage of solid content (69.7%) was obtained at pH 8.5 and 75°C temperature. The molecular weight of MF resin was increased with an increase of melamine monomer concentration. The highest residual weight 14.125 wt.% was obtained with sample 10.
